About the role 

Our Social Prescribing service supports patients referred by GP practices across Central Islington.Working within a fast, paced, person-centred environment, Social Prescribing Link Workers support individuals to identify what matters most to them and connects with community services, activities and support that improves their well-being and addresses the wider determinants of health.

The service benefits from close collaboration with our Connect service and access to over 120 referral partners, offering a diverse range of opportunities to support residents’ health, wellbeing and independence.

What You’ll Be Doing  

As part of a dedicated team of four Social Prescribing Link Workers, seven Connect service colleagues and external partners working in primary care teams, you will: 

• Work with patients referred by GP practices to provide personalised, strengths based support 

• Build trusted relationships and support individuals to overcome, social, practical and emotional barriers 

• Support people to access community-based services, activities and networks 

• Manage a varied caseload, maintaining accurate and timely records 

• Work collaboratively with GP practices, primary care teams, colleagues and external partners

• Passionate about creating a fairer society and reducing health inequalities 

• Have excellent interpersonal and communication skills 

• Have experience of providing person-centred or personalised support (paid or unpaid) 

• Can work at pace and work across different data-systems and maintain accurate records 

• Can manage a caseload in an organised and methodical way 

• Are comfortable balancing face to face work with administrative responsibilities

About you

You will provide high-quality education, care, and family support, ensuring children experience a safe, nurturing, and inclusive environment where they can thrive. Working as part of a multidisciplinary team, you will bring strong leadership, early years knowledge, and a commitment to creating enabling spaces that support children’s learning, development, wellbeing, and family engagement.

Role Title: Early Year Professional (added area of responsibility) / Senior Practioner 

Hours: 35 hours per week, 7.5 hours per day Monday to Friday (Hours will include some afterschool wraparound, occasional evenings and weekends).

Salary: £28,392 (FTE) depending on experience and up to an extra £3,640 per year incentive for added area of responsibility. Potential FTE £32,032.

You will have experience of:

• Planning, delivering, and evaluating high-quality learning experiences in line with the Early Years Foundation Stage (EYFS) framework, supporting children’s individual development, curiosity, and wellbeing.

• Working collaboratively as part of an early years professional team to maintain and continuously improve the quality of practice, provision, and outcomes for children.

• Operating within a Key Person system, building secure, nurturing relationships with a designated group of children and supporting their individual needs, development, and family relationships.

Main duties include:

• Contribute to the responsibility for providing a high quality of education and learning, ensuring that staff are properly deployed, and to offer appropriate stimulation and support to the children attending the setting.

• Contribute to the responsibility towards drawing up long-term, medium-term and sessional curriculum plans which take into account the requirements of the Early Years Foundation Stage (EYFS), and to monitor the effectiveness of the setting’s curriculum; this may include working with external professionals.

• Advocate for early years curriculum based on outdoor learning through play. 

• Contribute to the responsibility of drawing up and implementing the daily programme of activities and events.

• Comply with current Ofsted inspection requirements for achieving a ‘Good’ or above rating.

• Contribute and support the key person system, ensuring parents/carers are aware of their child’s key worker and that key workers regularly and effectively engage with their key children’s parent/carer.

• Contribute to the responsibility for the implementation of systems for observation and record keeping so that children’s progress and achievements are effectively and regularly assessed and to monitor the effectiveness of assessment procedures.

• Understand and appreciate the importance of monitoring and evaluation in a preschool based setting.

What you’ll be doing? 

The Better Together service delivers a community-based outreach, awareness-raising and personalised support programme to address gambling-related harm. As an Engagement Advice Worker, you will play a key role in delivering both strands of the service by raising awareness of gambling-related harm across Lambeth and providing personalised, strengths-based support to individuals affected by gambling.

1. Deliver a programme of community outreach across Lambeth by attending community events, local groups and venues where residents naturally gather, including faith settings, shopping areas, sporting venues, barbershops and culturally specific community spaces.

2. Build relationships with residents, community leaders and partner organisations to increase awareness of gambling-related harm, reduce stigma and promote the Better Together Service.

3. Deliver presentations, information sessions and informal engagement activities to residents and professionals, promoting early identification of gambling-related harm and available support.

4. Support the planning and delivery of conferences, networking events and professional education sessions, promoting the service and contributing to wider system change.

5. Provide holistic, person-centred one-to-one support to adults affected directly or indirectly by gambling-related harm and agreeing personalised goals to build on their strengths, access community resources and develop sustainable strategies that improve their health and wellbeing.

6. Support clients to access specialist gambling treatment where appropriate, including referrals to the NHS Primary Care Gambling Service, alongside wider support for issues such as debt, housing, benefits, mental health, loneliness, bereavement and social isolation.

• Experience of delivering outreach work, including presentations to small groups and at larger events.

• Experience of networking and proactively liaising with external agencies and community groups to build strong working relationships.

• Experience of recording data and keeping records up to date and accurate.

• Excellent interpersonal skills including verbal and written communication.

• Able to work independently and manage a varied caseload of clients and the ability to prioritise different areas of work according to need and deadlines, making efficient use of time and resources.

• Able to  work in a flexible manner to meet the demands of a developing service.

Islington Mind is recruiting a Team Leader for our Mental Health Recovery Pathway service, a trauma-informed mental health provision based across three sites in Islington and responding to the mental health needs of residents through a range of therapeutic interventions and community engagement.
