The Bexley Health & Wellbeing Coaching Service is delivered through a partnership of voluntary and community organisations working together to improve health and wellbeing outcomes across the borough. Bringing together a diverse range of local charities and community organisations, the partnership combines specialist knowledge, trusted community relationships, and local expertise to support residents to live healthier, more independent lives.

While coaches will be employed and hosted by partner organisations, they will form part of a borough-wide Health & Wellbeing Coaching team.

Working with households, the CHWW will build trusted relationships with individuals and families, identify health and social needs, provide personalised health promotion and behaviour change support, and connect residents to appropriate health, social care and community services.

About the role 

Our Social Prescribing service supports patients referred by GP practices across Central Islington.Working within a fast, paced, person-centred environment, Social Prescribing Link Workers support individuals to identify what matters most to them and connects with community services, activities and support that improves their well-being and addresses the wider determinants of health.

The service benefits from close collaboration with our Connect service and access to over 120 referral partners, offering a diverse range of opportunities to support residents’ health, wellbeing and independence.

What You’ll Be Doing  

As part of a dedicated team of four Social Prescribing Link Workers, seven Connect service colleagues and external partners working in primary care teams, you will: 

• Work with patients referred by GP practices to provide personalised, strengths based support 

• Build trusted relationships and support individuals to overcome, social, practical and emotional barriers 

• Support people to access community-based services, activities and networks 

• Manage a varied caseload, maintaining accurate and timely records 

• Work collaboratively with GP practices, primary care teams, colleagues and external partners

• Passionate about creating a fairer society and reducing health inequalities 

• Have excellent interpersonal and communication skills 

• Have experience of providing person-centred or personalised support (paid or unpaid) 

• Can work at pace and work across different data-systems and maintain accurate records

• Are comfortable balancing face to face work with administrative responsibilities

We are seeking a flexible, reliable, and enthusiastic Sessional Worker to join our Young Carers team. The primary focus of this role is to provide consistent support at our weekly Young Carers After School Club, which takes place every Thursday during term time from 4:30pm – 6:00pm. The club is delivered across two age groups (5–11 and 12–16 years), and regular attendance and commitment to supporting both groups each week is an essential requirement of the role. 

You will work alongside the Young Carers team to deliver engaging, age-appropriate activities that promote wellbeing, reduce isolation, and provide children and young people with a fun, safe, and supportive break from their caring responsibilities. You will encourage participation, build positive relationships, support behaviour where required, and help ensure every young person feels welcomed, included, and valued. 

You will also play an active role in planning and preparing for the After School Club as well as assisting with the set-up before each club and the clear-up afterwards to ensure the space is safe, organised and welcoming. 

In addition to the weekly After School Club, you will support the delivery of our school holiday programme, including trips, workshops, activity days, and special events, subject to your availability. This will include contacting families, promoting upcoming activities, recruiting and booking young carers onto both the After School Club and holiday activities, supporting attendance, and helping to ensure activities are well attended by those who would benefit most. 

From time to time, you may also support outreach and engagement activities, including attending community events, working with local schools and partner organisations, and helping to identify hidden Young Carers so they can access the support and opportunities available through our service.

  This is a sessional role, therefore hours are not guaranteed but will be offered based on project need.

• Experience working with children, young people, or vulnerable groups 

• Ability to communicate sensitively and effectively with young people and their families

You’ll build real relationships with clients, supporting them to stabilise, develop life skills and move towards greater independence, while navigating the challenges that can come with housing, health and wider support needs.
