Job description

This role will deliver healthcare advice, education and support planning to customers being discharged from hospital and those living in temporary accommodation.  The post holder will deliver health advice ‘drop-in’ sessions and one-to-one support to customers living in temporary accommodation, including low-level first aid care and lifestyle advice on issues such as smoking, sexual health, mental health issues and substance misuse.

The H4 Hospital Project provides discharge support, housing and health advocacy, and ongoing wellbeing support to homeless patients in Stockport. The project is delivered by the charity H3 that supports people experiencing homelessness within Stockport to build independence and wellbeing. 

The role is focussed on preventing repeat admissions to hospital.  This will include assisting patients to access on-going and appropriate primary and secondary health care. The post holder will support with arranging and transportation to customer’s health appointments and liaison between health services.
