We are looking for a compassionate and proactive Assertive Outreach Worker to support people experiencing multiple disadvantage, including mental health needs, substance use, homelessness, social isolation, or other complex challenges. This role will involve building trusting relationships with people who may find it difficult to engage with services, providing practical and emotional support, and working closely with partner agencies to help individuals access the right support at the right time.
The role focuses on building trust with individuals who may be reluctant to engage with traditional services, helping to reduce health inequalities, improve wellbeing, strengthen safety, build confidence and support sustainable recovery and independence.
The post holder will deliver community-based outreach, complete needs and risk assessments, co-produce support and safety plans, maintain accurate case records and work closely with partner agencies including housing, health, social care, domestic abuse services, drug and alcohol services and criminal justice partners.
This opportunity would suit someone with experience of supporting vulnerable adults, particularly in areas such as mental health, substance misuse, homelessness, domestic abuse, exploitation, safeguarding or multiple disadvantage. Specialist Behaviour Support Workers are responsible for supporting people with learning disabilities and behavioural needs to live full lives, develop skills and maintain their wellbeing.
The working pattern for this role consist of:

The Comedy School draws on the power of comedy to improve wellbeing across community, health, education and criminal justice settings. Providence Row has supported people facing homelessness in East London since 1860. Our Day Centre is often the first place people come when they need help. It is a space where people can eat, shower, speak to staff, and be welcomed with dignity.
As a Day Centre Assessment & Reconnection Worker, you will play a key role at that first point of contact. You will ensure people are welcomed calmly, understood quickly, and supported to access the right pathway. This is a role focused on assessment, decision-making, and connection rather than ongoing casework.
You will also support geographical reconnection where appropriate, including accompanying clients to local authorities across London, ensuring plans are realistic, safe, and grounded in dignity.

Location: This role is based in Balham. The service is a short walk from Clapham South Station (Northern Line Underground). Unfortunately, this service does not have step free access.

Shift Pattern: 37.5 hours per week Monday to Sunday on a rota between 08:00 - 21:30. You may be required to work bank holidays and evenings as required. You may be required to work additional hours outside of the hours to meet service and resident needs.
About the Role
We're hiring a Complex Needs Recovery Worker to join our team based in Penrose Focus. In this role, you will support residents who have a history of forensic mental health issues with co morbidity, often alongside co-occurring substance misuse disorders. The residents range from high risk to low support and our aim is to provide tailored support to help them reintegrate into the community.
As a Complex Needs Recovery Worker, you will identify individual resident needs, and provide person centred, tailored support to residents to help them overcome their personal challenges and achieve their goals. We're looking for a genuine passion for supporting people who may have complex needs which includes forensic mental health. Experience in mental health or support work is desirable but not essential, as full training will be provided. You will be able to work collaboratively as part of a team, and will have alignment with our SIG values. You will be keen to support our residents to achieve their individual goals and support them to be the best versions of themselves.

At Project 41, you will support individuals with complex needs to build stability, confidence and independence while working towards successful move-on into suitable accommodation. You will build trusting, trauma-informed relationships, create person-centred support plans and work closely with partner agencies including health, substance use, probation and social care services. Main Purpose Of Job:
Project 41 accommodates individuals within the homeless pathway and as part of Oxfordshire’s Homelessness Alliance. These individuals have support needs including substance use, mental health and offending histories, but are able to live in dispersed accommodation in Oxford with a higher level of independence than in our hostels. The successful candidate will understand the trauma people using our services have experienced, and have a compassionate and assertive approach to building relationships. At Project 41 you will provide high quality support service provision, work with clients to ensure suitable and timely move on, ensure the health and safety and safeguarding of service users and colleagues, work in a psychologically informed way, and develop and maintain effective partnerships with relevant agencies.
Main Areas of Responsibility:
To ensure the effective delivery of high quality support, housing management and resettlement opportunities.
To develop effective professional relationships with service users that is trauma informed and compassionate, and which empowers and motivates our service users to make positive changes in their lives.
To work in partnership with support agencies to coordinate a holistic response to an individuals needs; this may include substance use services, probation, health professionals and adult social care.
To lead on identifying and applying for suitable move on accommodation, and providing support throughout the move and whilst settling in to new accommodation.
To assess risk and suitability of referrals for the service.
To be responsible for creating and actioning person centred, strengths based support plans, and identifying and working towards suitable move on.
To assess and manage risk for individuals and in the service, creating and maintaining risk assessments for all clients.
Reporting any safeguarding concerns without delay in line with procedures.
Work with the Team Manager and colleagues to achieve agreed contractual performance targets and organisational targets and objectives.
Maintain a safe, clean and welcoming environment across accommodation services; this may including room cleaning and clearance.
Manage challenging situations in line with procedures and to ensure the safety of yourself and others.
Ensure that IT systems are updated as required and concise record keeping is completed promptly.
