Job description

The role

DHI is an award winning charity that works with socially excluded communities to help turn lives around. We are currently recruiting for 2 full time (37.5 hours) and 1 part time (15 hours) Shared Care Workers to support people who use heroin and other opioids to overcome their dependencies and live life to the fullest potential. We are looking for inspiring and creative individuals who are able to create opportunities for people who are often marginalised from the resources and opportunities that society has to offer.

The Shared Care Team will manage a caseload of service users and will work in close collaboration with GPs, partner agencies and other health and social care professionals. They will also help broker a package of holistic support to help build resilience and improve peoples ability to live independently.
