Project
GPIMHS is an emotional and wellbeing service that supports adults aged 18+ in primary care who are experiencing a range of mental health difficulties. Clients are offered extended consultation times, quick and easy access to practical advice, and tailored support for their mental health needs from a multidisciplinary team.
Clients can explore the situation affecting their wellbeing – Whether that is an ongoing mental or physical health problem, loneliness, debt, or other issues. They can then be guided to appropriate resources that may help, including talking therapies, benefits advice, or an introduction to a local community group.
The role of the Community Connector is fundamental to the development of these innovative teams and mental health services based within networks of GP Practices (Primary Care Networks – PCNs). The role supports the ongoing development and mobilisation of integrated primary care mental health services. GPIMHS is delivered in partnership between Surrey and Borders Partnership NHS Foundation Trust (SABP) and the voluntary, community and social enterprise (VCSE) sector.
The Role
As part of a multidisciplinary team based in primary care, the Community Connector will:
- Collaborate with the Mental Health Practitioner to conduct assessments (including risk assessments), assist with triaging incoming referrals, support clients in identifying socially determined needs and goals, provide self-management tools, and facilitate personal support plans.
- Deliver motivational and structured psychosocial interventions to promote recovery and wellbeing.
- Encourage independence through an asset-based approach that builds on individuals’ strengths, preferences and natural support networks.
- Establish and maintain effective relationships with GPs, practice staff, health practitioners, and a wide range of agencies to ensure coordinated support and continuity of care.
- Support clients in accessing community services including Community Connections, housing providers, family and carer support, debt advice, and employment services.
- Facilitate engagement with key services such as Safe Havens, CMHRS (Community Mental Health Recovery Services), and local wellbeing activities.
- Connect clients with peer support and volunteering opportunities and provide support to peer support workers within the PCN.
- Assist with community resource mapping, maintain a database of local resources, identify gaps in provision, and collaborate with VCSE organisations to develop needed services.
- Broker and build partnerships between public and voluntary sector agencies to enhance service delivery and access.
- Complete quality outcome measures with clients and conduct regular reviews (i.e. DIALOG+).
- Attend regular supervision with the Mental Health Practitioner (case management) and Team Manager (line management).
About You
Qualifications and Experience
- A diploma or degree in an appropriate subject e.g. counselling, psychology, social work, probation, mental health, Health and Social Care (Level 3) or NVQ Level 3+.
- A minimum of 2 years’ experience of mental health work.
- Proficient in Microsoft Word, Outlook, and basic Excel skills.
- Experience of using electronic patient record systems/case management systems (e.g. SystmOne, Halo, IAPTus) and/or ability to learn how to use the electronic patient record system.
- Ability to enter and manipulate data on a database as required by the Service and Commissioners to include in monitoring and reports.
- Ability to work from home and maintain confidentiality and professionalism when and if required using Microsoft Teams and other virtual platforms.
Knowledge and Skills
- Ability to interact effectively with clients, colleagues and other professionals whilst retaining clear boundaries.
- Understanding and ability to work to the confidentiality, consent, information sharing, GDPR and safeguarding policies of the integrated service.

This is a hands-on role for an experienced organiser who will lead key aspects of our community organising practice. About the role
Reporting in to the Director of Organisational Effectiveness, the Head of Health, Safety, Security & Safeguarding is a key role for Christian Aid and will inspire, design and drive Christian Aid's Global Safe strategy, in respect of safety, security, and safeguarding, ensuring that staff in the UK, overseas offices, and partner organisations operate in a safe, secure, and well-supported environment.

This is an exciting opportunity for an experienced leader, with a passion for giving children the best possible start and with a strong belief in the power of communities. The role will work both locally and nationally, as part of the wider team.
The Director will manage our client’s existing programme in Redcar and Cleveland with a talented and committed team (10 staff). They will also develop and manage the new programme in Middlesbrough, currently in Discovery phase, and recruit a staff team of 8- 10 people, with support from the charity’s central team. Once set up, the Director will lead the following across both areas:

As part of our mission to break down barriers to university access and address broader educational inequalities, the Social Mobility & Widening Participation department has developed a dedicated strand of community organising within our work, which is becoming increasingly impactful across King’s and recognised in the sector. Through our Access & Participation Plan 2025–2029, we have made ambitious commitments to expand of this work to improve young people’s educational outcomes and address challenges to university access and broader inequalities.
We work to address local barriers to education such as mental health, pathways to citizenship, access to English language provision and issues around employment and fair wages. We do this through our partnership with South London Citizens. This role leads on the development and delivery of community organising initiatives and supports the wider integration of these methods across King’s. Through community organising, the role empowers school leaders, young people and parents to design, launch and sustain impactful campaigns. The post holder will work closely with Citizens UK organisers and colleagues across King’s to build strong partnerships and drive meaningful change, including engagement in broader, high profile local, regional and national initiatives.
The postholder will use community organising methodology to lead and expand our core community organising programmes which are currently, Parent Power South London, Empoderando Familias (in partnership with Citizens UK) and Empower ESOL. A key focus will be driving the strategic growth of our flagship Parent Power programme into new regions in collaboration with The Brilliant Club, where we have a commitment to support the launch of a new chapter each year through to 2028/29. This initiative empowers parents and carers to engage actively in their children’s education while building strong networks of community organisers who drive meaningful change at both local and national levels. The postholder will act as the main liaison with Citizens UK, The Brilliant Club and work closely with internal key stakeholders to support wider community organising initiatives and deliver training for SMWP staff.

The Role

The Team
IPS Grow is a national programme designed to support the expansion of Individual Placement and Support (IPS) services in mental health, primary care, and drug and alcohol teams across England. Job Description
The Youth Violence Intervention Practitioner role is to be part of the youth work team within Redthread with a primary focus on youth violence interventions in a health care setting. In this role you will be part of the Youth Violence Intervention Programme team at Queen Elizabeth Hospital. You will support young people aged, 11-25 years old, who present to hospital following an incident of violence of for whom them there are concerns around exploitation or extra-familial farm. You will work with the young person in the hospital as well as supporting them post discharge to ensure they have the support in place that they need and want. Other tasks include:
- Hold a caseload and work with young people in a range of settings, including in A&E and on the wards, in the community, in face-to-face and group contexts, and promote their personal, educational, health and social development through all interactions.
- Assist with the on-going development of the service model to ensure that clients gain the maximum benefit from Redthread’s interventions.
- Ensure information resources such as client forms, databases and contact files are kept fully up-to-date and secure, in line with GDPR, the Data Protection Act and our Confidentiality Policy and Consent Policy. Maintain high quality recording of interventions with young people on our database, Threads, in line with Redthread’s best-practice requirements. (Training will be provided.)
- Collect and record evidence of project outcomes and young people’s achievements, to ensure the completion of monitoring forms and project progress reports as required.

Qualifications
- 5 GCSE’s (grade C or above), or equivalent.
- Experience of working with children and young people, with complex needs or vulnerabilities.
- Preferable: Working knowledge of supporting children and young people in one or more of the following areas: substance misuse, child exploitation, missing from home.

Working with the Chief Executive and the Executive Committee of Directors, you will act as Company Secretary and as head of a small Legal and Governance team. This will involve providing high quality advice on legal, risk, compliance and governance matters across the UK organisation and progressing strategic priorities to ensure first class management of these areas.
You will respond to a broad range of legal queries and develop robust policies and procedures to advise on compliance with legal, regulatory and donor requirements, voluntary standards and best practice. You will be confident in working independently to provide high quality advice, drafting and reviewing contracts, preparing and delivering training, working with the Board of Trustees and instructing external counsel where appropriate. Are you passionate about centring lived experience in mental health work and ensuring people’s voices are heard at every level of a charity?
We’re looking for someone to coordinate and champion our Lived Experience Advisory Group, a vital part of how we shape services, policies, and strategy at West Central London Mind. You’ll play a key role in ensuring people with lived experience of mental health challenges are meaningfully involved across the organisation, while also finding new ways to engage young people under 18 in a safe, inclusive and ethical way.
This is a rewarding and hands-on role where you’ll be the bridge between people with lived experience and our senior leadership, service teams and Trustees. From running group meetings and sharing feedback across the organisation, to safeguarding participants and making sure diverse voices are represented, no two days will be the same.
You’ll also contribute to our wider equality, diversity and inclusion (EDI) work, including supporting the delivery of the Patient and Carer Race Equality Framework (PCREF), helping to ensure our services are shaped by those most affected by inequality and injustice.
You’ll be the go-to person for:
- Coordinating and promoting the Lived Experience Advisory Group, ensuring it runs smoothly and inclusively
- Supporting group members to participate safely and meaningfully, with appropriate safeguarding and GDPR procedures in place
- Working with the senior leadership team and clinical leads to embed lived experience perspectives into services, campaigns, and strategy
- Recruiting and onboarding new members to the group, in partnership with the People Team
- Ensuring under-18 voices are heard through creative, age-appropriate engagement approachesManaging administration such as agendas, minutes, service user payments, and progress reports for senior stakeholders
About you:
We’re looking for someone with experience in coordination or engagement roles, and a strong commitment to amplifying lived experience voices. You might have personal or professional experience of mental health challenges and be confident working with a wide range of people, including young people and those from marginalised backgrounds.
You’ll need excellent communication skills, a strong grasp of safeguarding and trauma-informed practice, and the ability to work independently while collaborating with teams across the organisation. Most of all, you’ll believe in the power of co-production and want to help shape a fairer, more inclusive mental health system.
