• As a Children’s Residential Support Worker, you will provide a caring, safe, secure and accepting environment that meets the physical, health, social needs and rights of the young people.
• You will maximise educational opportunities and achievements for the young people by assisting in identifying, establishing and supporting educational provision and contributing to educational reviews.
• You will also undertake direct work that is designed to meet the young person’s needs as identified within the care plan, enabling them to reach full their potential.  Supporting the young person’s significant relationships will be a part of this task.
• Under close supervision you will take responsibility for key working at least one young person.
• You will maintain accurate and up to date records of the work undertaken with the young people and complete written reports for planning meetings and statutory reviews as necessary. 
• Support young people with their individual personal care needs.  This can include full reliance upon you to change continence pads, assist with full body and hair wash, as well as dressing.   Or, encouraging them to be as independent as possible and washing/dressing themselves to progress with their skills development. 
• Undertake a range of domestic duties, including cooking and some cleaning, to a good standard, to ensure compliance with health and safety policies and the guidelines for young people and staff.

• Experience of working in a residential setting, undertaking the full range of duties in a well-planned way that respects & values the young people as individuals.
• Flexibility to respond to the needs of the unit and young people.

The successful candidate will support individuals on a one-to-one basis and work with them to put together a personalised action plan to address their immediate and longer term needs in line with their mental health priorities. Over a number sessions you will work collaboratively with individuals to explore the situation and factors affecting their mental health, support them to develop skills and tools to help restore and maintain their emotional wellbeing and find solutions to address the wider issues affecting their mental wellbeing including debt, housing, unemployment, and loneliness.

Service Delivery

• Deliver focussed support to individuals experiencing mental health crisis in MK in line with Mind BLMK’s agreed Crisis Recovery Service as well as contract requirements.
• Manage and prioritise a caseload of service users including processing referrals and assessment of their needs and suitability in line with the access to services process.
• Work with individuals to explore the situation affecting their mental health and wellbeing and co-produce a personalised, bespoke support plan to address their immediate and longer term needs in accordance with the priorities and urgent support required by individuals.
• Take a holistic approach, based on the individual’s priorities and the wider determinants impacting on their health and wellbeing such as debt, poor housing, unemployment, loneliness and caring responsibilities.
• Offer focused support to individuals on a 1-2-1 basis with the aim of building their knowledge, skills, confidence and resilience.
• Signpost individuals to wider services and encourage and support people to access local activities and community services.

About the role

We’re looking for a Social Worker to support children and young people diagnosed with cancer in our Leeds Team.

As the first point of contact, you will be responsible for delivering a warm and welcoming front-of-house experience for visitors, including children, young people, staff, tenants, and contractors. About the role:

We are looking to appoint a Kentown Family Support Worker to deliver a high-quality family support service as part of the pilot Kentown Children’s Palliative Care Programme, in collaboration with Kentown Wizard and Together for Short Lives.

Reporting to the Kentown Project Lead out of our Lancaster team base and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness.

What we’re looking for:

· An experienced child health, education or social care professional - applications will be particularly welcome from those who have worked in a community environment and those with a recognised qualification in education, health or social care.

· A warm, inclusive approach to achieving goals quickly and correctly.

· Practiced in child protection, information sharing and the rules around data protection - you lead by example, drawing on your own professional experience and working within established guidelines.

· Practical and people-oriented - you will thrive working at a fast pace whilst maintaining accuracy and be a confident user of IT (including MSOffice)

· A persuasive and open communicator, you will work collaboratively with your team and volunteers to ensure delivery of a high-quality service and support fundraising colleagues by writing case studies and family updates

· A practical knowledge of diversity issues affecting children, young people, and their families – aware that being responsive to others needs and concerns, is essential.
