We require a Social Worker to provide high quality social work in our schools across the Diocese of Salford according to each school’s formal agreement.
You will work directly with children and young people, their parents, carers and schools in order to further the child’s welfare and provide the interventions necessary to enable them to use their learning opportunities to the full. You must be able to provide a non-judgemental, caring and confidential environment throughout this work to ensure the issues are addressed in an appropriate manner for all those involved.
To apply you will have experience of working with children and families and experience of direct work with children which includes supporting school staff with safeguarding. You must have competent IT skills which includes providing regular reports to the individual schools and Caritas and feedback to school staff as required. You must be able to maintain appropriate resources, databases and case files as well as maintain efficient and comprehensive referral and recording systems.
Registered social worker status with Social Work England (SWE) is essential and the salary includes accrued annual leave.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

What you'll do:
Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.
Liaise with partner agencies regarding making referrals where an unmet need has been identified via the multi-agency assessment.
Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible.
Undertake key-working responsibilities for a caseload assigned by the Team Leader/Manager
Support customers to undertake all domestic tasks as past of the assessment of daily living skills, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
Develop and maintain links with all key agencies and service providers in the local community.
Empower customers to ensure they receive the service and benefits they are entitled to, including assessment and support of budgeting and debt management to garner their tenancy readiness.
Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
Adhere to Look Ahead's Policies and Procedures
Engage in learning and development activity to increase knowledge and skills.
Provide day-to-day instruction of domestic staff/Personal Support Assistants where appropriate.
Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned.

About the role:
The role of Triage and Early Interventions Officer (TEIO) is a fast paced one which requires the post holder to answer incoming calls and make outgoing calls to victims of crime and those accessing the service in Cumbria.
As a TEIO, you will;
- answer incoming calls, speaking to those affected by crime & manage a caseload
- make outgoing calls, assess needs and risks and action plan with callers
- work on the case management system to allocate cases, provide administration on cases and assess/ensure data quality
- liaise with other agencies where required to find out more information, give information or make referrals or give signposting information
- work with the wider team to ensure an effective, trauma informed service is given to those affected by crime
You will be a confident, compassionate and empathic person who can work well under pressure. You will be speaking to people who may be upset, angry or in emotional difficulties and will need to be composed, empathic and be able to quickly assess situations.
You will be confident in using your IT skills to work on a case management system, access phone and online systems, email and Microsoft Office programmes.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Windsor Road service offers 11 beds to people experiencing mental ill health. We are a CQC registered residential care home. We offer individual one to one interventions and sessions such as social anxiety, anxiety, finance and budgeting, positive self-image, building self-esteem, support with diet and nutrition, healthy eating, sleep hygiene, DBT skills, building resilience, life skills, solution-based therapy, stress management etc. We offer a range of activities in our service planner including a daily coffee morning, a ‘here and now’ group, community gardening, cooking, arts and crafts, social inclusion and family and friends’ days. We support our residents with daily living skills where this is needed, such as support with shopping and cleaning tasks, this could include prompting or supporting alongside them. We also work closely with other agencies and our residents wider care teams, so good communication skills are required. And of course, all of this is recorded on our information management systems, so good record keeping and documentation is also essential. Our service consists of 8 24/7 Recovery Workers, a Service Manager and Registered Manager.

Responsible for helping and inspiring the people who use our services to achieve independence in the community, we’ll rely on you to work with them to come up with a person-centred support plan that will see them achieve their goals and aspirations, as part of your key worker role. Whether it’s developing their domestic and finance management skills or accessing work or volunteering or educational opportunities, one thing’s for sure ‐ you will have every chance to shine.
Care home, mental health support or social care experience is preferred. You will be required to administer medication and complete a medication qualification and the care certificate. Candidates with an understanding of mental health issues and/or lived experience would be welcomed. More important is your caring and compassionate nature and empathy and enthusiasm for helping others. Whatever your background, you’ll need to be happy to work both independently and within a team and willing to be part of a weekly rota system and available for on call duties as well as completing sleep in shifts.

This is a permanent part-time requiring the post holder to work 30 hours per week.
The post holder will be required to work a minimum of one sleep-in shift per week.
