About this role…
Thames Reach’s Supported Housing is comprised of Accommodation, Housing First, Navigator and Move on services supporting people to make real and lasting change in their lives. People may present with complex mental health, offending histories, drug and alcohol needs.
Housing First services: Using the established principles of Housing First we work with people to access accommodation first, explore their aspirations, building opportunities for them to spend their time meaningfully and overcome barriers around unmet mental or physical health needs.
We are looking for a Support Worker to join the Greenwich Housing First team.

You will:
- Build and maintain positive, trusting relationships with individuals that have a history of rough sleeping or insecure housing.
- Support individuals to move into their own, independent accommodation, and to make it a home. You will work closely with landlords and housing providers to help individuals to understand and sustain their tenancies
- Work in partnership with key agencies, supporting individuals to access support services, community resources, and to feel a part of their local community
- Empowering individuals to take an active role in developing their own personalized support plans, and motivating them to achieve their goals and aspirations

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. Applicants will need to be pro active and support our customers to enhance their lives.

- Undertake key-working responsibilities for a caseload assigned by Management staff
- Meet customers regularly for support sessions, providing emotional and practical support
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
- Develop support plans with customers to identify their goals, ensuring a person centred approach, enabling the customer to establish positive change
- Support customers to take care of their living environment, providing emotional and practical support to ensure customers enjoy a high quality accommodation
- Develop and maintain links with all key agencies and service providers in the local community
- Support customers to apply for benefits and manage their bills. Empower customers to ensure they receive the service and benefits they are entitled to
- Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards
- Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

Streets of Growth is seeking to employ an experienced, motivated, and personable individual to work as part of the frontline team to deliver targeted street work to engage and build relationships of trust with high-risk vulnerable young people (15-25 years), in hotspot areas known for Anti-Social Behaviour (ASB), exploitation, serious youth violence and young people that are furthest away from engaging in services.

The role:
Delivering targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Working with and engaging young people in strategic interventions across Lifestyle, Enterprise/Employability, Education and Environment that constructively challenge negative patterns of behaviour, create an experience for positive thinking, confidence building, entrepreneurial work skills, and tapping into the young person’s ambition, potential and talent.
You will be working alongside Intervention Coaches to progress street-engaged young people into Streets of Growth’s staged-based Appropriate Intervention Bridging Programme delivered from our flagship Centre based in Aldgate East and delivering appropriate targeted intervention work at the street/estate level during critical incident situations in the community.
Who we are looking for:
Along with a relevant qualification up to at least Level 2, you will have robust frontline experience working with young people, including experience of detached outreach work with young people. You will also have good knowledge of safeguarding risk assessment and contextual safeguarding. A key component of the job role is the ability to engage and build transitional relationships with young people, most particularly hard-to-reach individuals.

This is an exciting opportunity to join our new service supporting Unpaid Carers across Lewisham to improve their health and wellbeing, and access support that meets their needs.
The team works with unpaid Adult Carers, Young Adult Carers and Young Carers, completing statutory and holistic assessments to understand people’s individual circumstances and level of need. You will carry out home visits and community meetings, co-produce support plans, and develop effective relationships with Unpaid Carers, families, and professionals.
You’ll build on your local knowledge to signpost and facilitate access for Unpaid Carers to support services, including financial and benefit advice, housing, health and safety home systems, and leisure and social activities. You will also support the development of escalation plans so that Unpaid Carers are prepared for unforeseen circumstances that would impact negatively on ability to care.
Applicants should have relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a social care, health, education or voluntary and community sector. You’ll need a practical understanding of barriers faced by Unpaid Carers, and an awareness of cultural differences and access to services. The role:
Taking on a lead role to deliver targeted street work and outreach across the borough of Tower Hamlets. This involves engaging and building relationships of trust with young people at street level and in identified hotspot areas of risk where there is low to high anti-social behaviour, criminality and violence. Working with and engaging young people in strategic interventions across Lifestyle, Enterprise/Employability, Education and Environment that constructively challenge negative patterns of behaviour, create an experience for positive thinking, confidence building, entrepreneurial work skills, and tapping into the young person’s ambition, potential and talent.
You will have senior responsibility with the Frontline Director to plan and execute street work and outreach delivery in identified hotspot areas or risk based on street/community and partner intelligence. Job Description
LMK is looking for passionate and organised educators (LMK Leaders) to run community workshops, aimed at young people aged between 11 and 24. Through engaging, relatable, workshops you will help young people identify signs of healthy and unhealthy relationship behaviours, provide strategies for them to recognise the early warning signs of abuse and give them tools to keep themselves and their friends safe.
The minimum hours required per workshop is 3 hrs. This includes arriving 30 minutes before delivery, 1.5 to 2 hrs delivery and then remaining back 10 minutes and being accessible to young people who would like further information or a one-to-one conversation, surrounding the subject of relationships.
There is also 30 minutes admin, which is usually completed off-site, such as inputting student surveys and a Leader survey.

Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. About the role:
The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.
Main duties:
- To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
- Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
- Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
- Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. * Maintain records as required at the project under the direction of the Support Workers/ management
* Participate in team meetings/reviews and the general development of the service
* Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned
