This is an exciting opportunity to be part of and help shape a new DHSC funded project - Early Support Hubs - providing innovative ways to support Young People’s (11 – 25 years) Mental Health and Wellbeing across the borough of Hackney.
This Early Intervention approach will enable young people to access support in a timely way within their own community and we will work collaboratively with the Hackney Super Youth Hub enabling a one stop shop experience for local young people.
We are looking for passionate and motivated Therapeutic Youth Workers who are committed to supporting positive change for local young people to join our team.
Main Responsibilities:
· Undertake 1:1, group, detached and outreach work with young people
· Provide one-to-one key-working up to 12 sessions, combining youth work values and principles with a therapeutic, trauma-informed approach to help address a wide range of issues that may impact on young people’s mental health and wellbeing e.g. housing, employment, physical health, education, finances and relationships.
· Use a supportive, young person-centred approach. e.g. explaining what services are and what they do, ensuring information is accurate and up-to-date, filling out forms together, making personal introductions, accompanying young people to initial meetings/sessions, being with them during phone calls etc.
· To work with the Hackney Super Youth Hub, to support young people and liaise with appropriate professionals in the network.
· To maintain confidential, up to date and accurate records of all sessions and plans.
· Be an advocate for young people, supporting colleagues to understand adolescence, and how to engage with young people, particularly with concerns around:
o mental health and wellbeing of children and young people.
o Supporting vulnerable groups who would not otherwise receive support (levelling-up).
o Improving children and young people’s engagement with EET.
Main Requirements (for details check the job description and person specification):
· QCF Level 3 Diploma in Youth work or equivalent and have experience of youth work in a youth work setting
· Experience of working with young people, delivering therapeutic groups and/or 1 to 1 interventions in a youth work or similar setting.
· To have an active interest in working with those affected by mental health issues and recognise the impact this has on their life.
· Experience of building positive, impactful relationships with young people responding to their needs
· Excellent, up to date knowledge and understanding of safeguarding needs and risks young people may face.
· Experienced in working as part of a safeguarding team around a child, i.e. Childrens services, education, CAMHS, etc
· Experience of working in with an outreach role, in the community and school settings.
· Confident working well independently as well as in a team setting.
· Ability to use own initiative and be able to respond to change as part of a pilot initiative.
· Computer literacy including Microsoft Office suite (incl. Outlook and Word), with ability to accurately input sensitive data electronically

Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. This could be the perfect role for you! The Family Support Worker is an exciting role within a growing and dynamic team, where you can help to make a real difference to the lives of unpaid carers in Lambeth.
Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project.
The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.
The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

An opportunity has arisen for a lead Support worker to work within our Sanctuary crisis intervention and prevention service, based in Fore Street in Enfield. We act as a short-term A&E and Emergency services alternative, providing support to individuals in the Enfield Borough aged 18+.
This role is to provide leadership and support to the Sanctuary team on shift. The role will:
- Lead shifts during the week, providing support to team members via case management support, escalation, Health and Safety guidance
- Support the Sanctuary Manager to embed processes within the service, and ensure that these are carried out
- Work alongside the Manager and team to ensure that the service is operating to high quality standard.
- Provide support in times of emotional distress for clients. The support worker will provide a calm, supportive presence.
The role will support clients through co-producing support plans with practical and therapeutic support offerings, as well as contributing to the development of a warm welcoming environment and ‘safe space’ to be.
This position would suit someone with a minimum of 2 years experience of working with people with mild, moderate and severe mental health issues.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Look ahead provides the support function and Riverside provides the housing management and night concierge. The Service provides on average 5 hours of weekly support to customers who reside in the hostel with the aim to provide them with the tools to move on within the community and live independently.

To provide support to peers and management, being flexible and responsive and help bespoke support for each customer's abilities and aspirations
To implement the principles of personalisation, as part of a local service plan.

To support up to 14 customers to sustain their tenancies
To promote customer involvement in all aspects of your work
To take on a project that will help assist the service move forward
To work closely with peers/customers and lead on initial referral assessments, aiding their journey for betterment & lead on the resettlement of customers

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. 132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.

Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.

Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management.
