Role purpose
The Client Support Worker (Crisis Navigator) will work with clients to provide non-judgemental, person-centred support. The work undertaken will be led by the different needs of each client, but will include: liaising with statutory services and advocating for clients’ needs to be met, for example with housing teams, social care, mental health services, etc.; support with form filling; support with grants, welfare benefits and income maximisation; emergency applications; supporting with emotional needs and psychoeducation.

The successful candidate will manage a caseload of individuals, taking referrals from their legal colleagues. This will involve a combination of immediate intervention (for example, access to a medication prescription upon release from custody), and longer-term work, planned in partnership with clients. Preventative work will be actively encouraged, supporting clients to address needs before crisis point wherever possible – both through direct work and through referrals and signposting.

Due to the nature of the work, if successful, you will need to have an up-to-date DBS check completed prior to undertaking unsupervised client work.
 

Key Responsibilities

• Triage, onboard and support a caseload of individuals autonomously
• Maintain an up to date understanding of what needs clients might have (e.g. with welfare benefits, support from statutory services, housing)
• Liaise with core agencies (social services, mental health teams, GPs, VCSE organisations)  
• Provide advice and information relating to all welfare benefits, including assisting clients with form filling and contacting relevant agencies on the individuals’ behalf
• Complete emergency applications and grant applications
• Attend training, forums and meetings to ensure knowledge remains relevant and up to date
• Keep up to date about current best practice and legislation within mental health, as well as within the field of information & advice more generally
• Provide updates and feedback to the relevant lawyers / legal teams
• Engage in multi-disciplinary meetings, both internally and externally
• Use our database to record details of all client referrals and contacts to ensure client information is kept up to date
• Use the correct templates, conduct outcomes and satisfaction assessments with clients on closure
• Record the results of outcome assessments and satisfaction surveys

Person Specification

Knowledge and Experience

• Substantial experience of working with people experiencing challenging situations with a non-judgemental, person-centred approach
• At least one year experience working in a crisis setting or with people experiencing crises (for example, inpatient mental health settings, homelessness relief services, drug and alcohol services, working with people going through the asylum process)
• Knowledge of statutory entitlements and experience advocating for clients
• Knowledge and understanding of mental health needs and neurodiversity
• Experience of managing complex cases and autonomously managing a caseload
• Experience of multi-agency and/or multi-disciplinary working, especially alongside safeguarding processes
• Experience of maintaining client records, in line with monitoring and evaluation requirements, both for funders and developing best practice

Job Purpose 

To support the manager and deliver on the home and settle services across two hospitals, supporting the wider team, and provide leadership when necessary. You will work in collaboration with AUKEL, the hospitals, and other community health and social care colleagues.

The service supports people returning home after a stay in hospital, helping them to settle back at home, regain confidence and support with some practical tasks, e.g. shopping/cleaning.  You will manage referrals, carry out assessments, and with the Manager, allocate service users to Discharge and Settle Worker caseloads. You will provide direct frontline support and co-ordinate visits to the service users’ home in preparation for their return home, as necessary. You will work collaboratively to lead and manage your team to support the smooth transition from hospital to home for service users.  The service is time limited, usually up to 6 weeks, to adjust and settle back in their homes.

Key Tasks 

• Act as the first point of contact for all referrals across the hospital they are attached to. 

• Meet with patients in the community and/or hospital and carrying out assessments and risk assessments

• Enter referrals on to AUKEL’s case management system (currently Charity Log) in accordance with GDPR guidelines. 

• Allocate referrals to Discharge and Settle workers using tech available (currently the Call Round App), training will be available. 

• Collecting feedback from service users and uploading to case management system. 

• Prepare and keep stock of AUKEL Welcome Packs and service leaflets. 

• Deputise for the Manger at discharge planning and other meetings as directed. 

• Support the hospital to improve the patient’s experience of the discharge process 

• Monitor the case loads of each of the Discharge and Settle workers (DSW) to ensure they are evenly and fairly balanced. 

• Work collaboratively with other agencies providing support services.

Administration 

• Keep AUKEL case management system up-to-date, input all data, and ensure all referrals, interventions, time spent with service users are recorded. 

• Allocate referrals to DSW using tech available (currently - Charity Log ‘Call Round’ App). 

• Ensure wards have good supply of service leaflets and referral forms. 

• Ensure data collected meets the requirements of the service’s contract monitoring and key performance indicators. 

• Ensure customer feedback questionnaires are sent at the end of each intervention and record returns. 

• Ensure admin tasks relating to service users, Discharge and Settle workers or general admin is carried out in a timely and efficient manner.

• Provide weekend cover as needed, i.e. annual leave and sickness, for the services you manage and other hospital settle services provided by AUKEL 

• Carry out monitoring of the service ensuring it meets the contractual requirements of commissioners.

About the opportunity

As Treasury and Accounts Receivable Officer within our Finance & Assurance directorate, you'll be at the heart of our financial operations. Ensuring we have the cash flow, systems, and processes that keep the Society running smoothly. Role Overview 

Support Through Sport CIO is seeking a highly motivated and organised individual to deliver our innovative School and Community Navigator programme throughout Nottingham and Nottinghamshire. The Senior School and Community Navigator will lead a team of three Navigators across four schools in Nottingham and Nottinghamshire. The role combines direct support to children and young people aged 10–17 with leadership and coordination responsibilities, ensuring delivery is of the highest standard and in line with Support Through Sport policies and project specifications.

Working closely with schools, families, and local agencies, the Senior Navigator will provide supervision and guidance to staff, build and maintain strong stakeholder relationships, and ensure effective safeguarding, monitoring, and reporting systems are in place. You will work with young people aged 10–17 in community settings, building trust-based relationships and providing early intervention to reduce risk factors linked to crime, violence, and exploitation.

The role combines youth work, mentoring, and community engagement, supporting young people to tackle challenges such as disengagement in education, substance use, poverty, discrimination, and school exclusion. You will act as an advocate for young people, bridging gaps with the wider community while promoting respect, empowerment, and positive life choices whilst working utilising a child-centred and collaborative approach.

At Support Through Sport, we use a range of creative approaches, including sport as a diversionary tool, to build brighter futures and create safer communities.
