You will give advice and information to carers and work closely with them to develop person centred support plans. You will work within a Community Support Team to deliver peer support groups, wellbeing activities, advice surgeries, outreach and events across the borough.

Support Coordinator

This is an exciting opportunity to work with stroke survivors and their families to provide Key Worker support following a stroke.

Position: S11121 Stroke Association Support Coordinator (two posts)
Location: Home-based, Cornwall however, extensive travel will be required as part of this role (May include team meetings or other work-related meetings)
Hours: Full and part-time (1 x 35 hours per week post and 1 x 32 hours per week post)
Salary: Circa £26,700 per annum for full time hours, pro rata for part time hours
Contract: This is a fixed term contract until 31 March 2026. Our services are contracted, we currently have funding for this contract until 31 March 2026.

About the role

The Salvation Army are partnering with the University of Stirling via the Salvation Army Centre for Addiction Services and Research to deliver a trial for the National Institute for Health and Care Research (NIHR). This unique and innovative social care trial will examine the effectiveness and cost effectiveness of a Peer Navigator/peer support intervention for people experiencing homelessness and problems with substance use. We are now looking for a Peer Navigator (lived experience practitioner) within our Homelessness service to deliver a relationship-based intervention to those experiencing drug and alcohol problems and homelessness, providing both practical and emotional support.

Key Responsibilities: The role of Peer Navigator (Lived Experience Practitioner) is fundamental to The Salvation Army’s desire to help vulnerable clients achieve positive outcomes and as such you will be passionate, target driven, dynamic and demonstrate an in-depth knowledge of services for vulnerable clients in a competitive climate. The Senior Multiple Needs Independent Domestic Violence Advisor (IDVA) will work within a dynamic, crisis intervention, advocacy and support service based at the CGL Ealing Women’s Wellness Zone. The IDVA will provide support in the local community and at the Ealing Women’s Wellness Zone. The successful candidate will be co-located up to 5 days a week at the Women’s Wellness Zone.
The IDVA will be closely working alongside CGL colleagues/team to deliver support to women with a range of complex support needs including substance misuse, mental health, domestic abuse, sex working, trafficking, and offending behaviour. The IDVA will work in collaboration with their team members to holistically support women in the borough of Ealing to achieve sustained recovery, improved health and wellbeing, helping women to manage/mitigate risks, and empowering them to reach their fullest potential in society as well as ensuring the voice of survivors informs every stage of the process.
The Senior Multiple Needs IDVA will assess the needs and risks of survivors, carry out short and longer-term risk management, safety planning and support. As well as identify and refer to services appropriate to their needs. They will also proactively advocate and advice women of their rights and options for seeking help and support from other agencies, encouraging them to engage with other agencies, and help co-ordinate the provision of multi-agency support where necessary.
About You:
To be successful as the Senior Multiple Needs IDVA you will need to have:
Excellent understanding of domestic abuse and its effects on women and children, of best practice in supporting survivors of domestic abuse, and of the MARAC. As well as understanding of drug and alcohol issues and experience of working within a related field.
Bringing your experience of risk and needs assessment, safety and support planning, and crisis planning and successfully managing high numbers of referrals and experience of working in a co-located setting with another agency and providing consultations/briefings to professionals to support in upskilling them around Domestic Abuse. The candidate will have experience of partnership working and of maintaining excellent working relationships with a range of stakeholders.
This is an exciting opportunity for you to work as part of an innovative service, designed to offer multi-disciplinary wrap-around support to women with multiple needs, with the aim of facilitating long-term recovery.

The young people we work with have become homeless for a number of reasons – isolation or mental health, abusive relationships, substance misuse, involvement in gangs/county lines - their issues are complex so you will need to be able to build strong, trusting relationships so that our customers feel safe and supported.
We are opening two new supported housing schemes in Lambeth, which will be Ofsted registered. Both schemes are part of our well-established Young Persons and Complex Needs service in London and will provide temporary housing for young people aged 16-17 who are experiencing homelessness.
A bit about the role:
As a Caseworker, you will be working with a team of highly motivated, compassionate, and dedicated colleagues supporting a caseload of young customers with complex needs. You’ll need to meet each customer weekly to help them with their goals and discuss their progress. And, when someone’s ready to move on, we'll rely on you to help them access the housing register or look for suitable rented accommodation.
What we’re looking for:
You may have worked with vulnerable young people in a paid or voluntary capacity, maybe as a Youth Worker or in an education setting providing pastoral care. You’re a great relationship builder, who can create trust so that our young people feel comfortable sharing their issues with you, you listen but in a non-judgemental way and you are able to spot signs for concern and act on them. The Role
Reporting to the Service Delivery Coach in the East of England, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.
Key responsibilities will include:
· Supporting new stroke survivors and their carers to adjust to life after stroke following hospital discharge into the community.
· Providing personalised information, advice and support.
· Enabling stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.
· Working with other health and social care professionals across the stroke pathway to ensure high quality support.
About You
You will have experience in:
· Experience/ background of providing person centred support to people with a disability or long-term health condition and their carers.
· Excellent IT skills and an ability to maintain accurate records.

· A flexible approach and an ability to effectively manage a caseload.

We are an ambitious, dynamic and impactful charity. Following their sons’ diagnosis of Duchenne muscular dystrophy (DMD), Emily Reuben and Alex Johnson founded Duchenne UK in 2012.
DMD is a devastating progressive condition diagnosed in early childhood which causes all the muscles in the body to gradually weaken. There are currently only very limited treatment options for DMD, so Emily and Alex set up Duchenne UK to change this.

We are currently seeking dynamic Practitioners to join our Children & Young People’s service (CYPS), working directly with young people affected by domestic abuse.
Domestic Violence Intervention Project (DVIP), a division of Richmond Fellowship since 2018, has been a leading provider of Respect accredited community perpetrator programmes working to stop domestic violence, increase safety for women and children, and reduce harm to families for over 29 years.
This role requires a good understanding of adolescent development, domestic abuse, child abuse and trauma, and the impacts of these on young people’s development, as well as a good understanding of the nature of domestic violence and its effects on women and children, and on the parent-child relationship. You will have experience of direct work with young people and their parents/carers, working flexibly but robustly and supportively, responding to challenging or harmful behaviour to facilitate change. You will have experience of providing structured one-to-one and/or group work counselling or behavioural change interventions, and of working with both children and adults from diverse backgrounds. You will have an excellent understanding of safeguarding procedures and be confident working both independently and as part of a team, working closely and collaboratively alongside partner agencies such as Children’s Services, Schools and Youth Offending Services.
You will have the opportunity to play a key role in developing DVIP’s young people’s services, working in partnerships with a range of statutory and voluntary agencies to deliver effective community responses to domestic violence, in line with RESPECT practice standards. Richmond Fellowship is part of Recovery Focus, a national group of charities highly experienced in providing specialist support services to individuals and families living with the effects of mental ill health, drug and alcohol use, gambling and domestic violence. From 1st June 2024 Richmond Fellowship will be merging with the mental health charity Humankind to form a single organisation that provides the joined-up mental health, housing and drug and alcohol support we’ve all known has been needed for decades. In October 2024, Humankind will then be renamed to reflect the new, bigger and better organisation.
