We're looking for an enthusiastic Complex Needs Worker with considerable experience of working with people with substance misuse, mental health problems and knowledge of the problems that confront homeless people with complex needs. You will be joining the team in our Islington Progression Pathway, a  service that provides accommodation and support to individuals with complex needs in a recovery focused way. The team works collaboratively with clients in a safe and supportive environment in developing goals, plans and strategies to better manage their mental health, substance misuse and any associated risks.  

Your key responsibilities will include:

• Working as part of a multidisciplinary team to create an environment of person-centred support for clients with complex needs, maintaining a safe environment which clients who have experienced complex trauma are able to build their confidence and discover opportunities for positive change, in line with Psychologically Informed Environment principles and St Mungo’s Framework.
• Identifying and developing external relationships that increase accessibility to support within Islington for clients with complex needs.
• Providing training, support and case management to team members
• This role is Monday-Friday 9am-5pm

About you

We are looking for someone who has a good understanding of substance use and ideally one of the following; Mental health, Trauma Informed Care and/or Psychologically Informed Environments. To be successful in this role you will also require:

• The ability to maintain enthusiasm for a high level of contact with clients on a day to day basis.
• An understanding of a Person Centred approach and a non-judgemental way of working with people who have been homeless, and have perhaps had negative experiences of services.
• Good communication skills and the ability to work with others whilst managing your own time.
• The ability to empower individuals to make positive changes and a willingness to develop these valuable skills. 
• The ability to work flexibly which may include some weekends to ensure continuity of the service.

Rough sleeping data shows that migrants make up more than 50% of the people rough sleeping in London. The MAPS service provides a rapid intervention to support clients who are migrants and rough sleeping in London to find a route into accommodation that is suitable and sustainable in the UK or elsewhere in the world. We are looking for someone to join our team as an Assessment and Reconnection Worker where you will:

• Provide rapid holistic assessments with migrants who are rough sleeping across London, helping to clarify entitlements and identify routes off the streets.
• Work with clients to produce person-centred, rights-based actions plans that meet their needs and resolve their rough sleeping in the UK or elsewhere in the world.
• Work closely with external agencies to ensure all clients’ needs are met. Many of our clients have multiple support needs. This may include experience of navigating the immigration and asylum systems, complex trauma, substance misuse and mental and physical health support needs. 
• Advise and support other professionals working with migrants on options to resolve their clients’ homelessness.
• While your usual working hours will be from Monday to Friday, 9am-5pm, occasional work during early or late hours may occur in response to challenging situations and to prevent rough sleeping. 

About you

We are a small team so try to maximise our resources and find creative solutions to resolve people’s homelessness. Due to the legislative and political climate in which we work, accommodation options are often limited for our client group, and so we are looking for someone who can be flexible, proactive and is passionate about working migrants.

Ideally you would have experience of working with vulnerable people and an understanding of the needs and support requirements of this client group, particularly non-UK nationals.

We are particularly interested in hearing from you if you have lived experience of migration and/or speak additional languages to English.

About the Role

Do you enjoy working in a fast-paced environment where no day is the same This role requires using your initiative and proactive attitude than reactive.

You will also be working with women who are in trauma and can be in crisis requiring you to be empathetic but aspirational. You will provide advice and support to women who have experienced domestic abuse in a refuge setting, across the three sites.  You will carry out risk and needs assessments, develop support plans and carry out key work with women survivors. You will be expected to work in one of the three Refuges as part of your role

Job role tasks include:

• Support – Induction, Risk Assessments, Domestic Abuse, Mental Health, Benefits, Debt Advice, Move On, maintain accurate recording on Solace case management system and ensure regular reviews are carried out.
• Frontline delivery of support directly to service users.
• Repairs Identification/reporting/meeting contractors.
• Health & Safety – Basic observational H&S assessments and ensuring building compliance, flag risks in properties.
• Void inspections, basic void works – i.e. clean/clear, furniture, welcome packs.

About You

The ideal candidate will have in-depth knowledge of domestic abuse and its impact on women and their children, as well as experience of providing emotional and practical support to victims of domestic and sexual violence.  An in depth understanding of the needs of women and children from a diverse background is essential. 

You will have experience of managing risk and following case management procedures to meet the needs of the client group, with the ability to work with women to help them to understand their needs, risk assess, develop safety plan and support them in linking with other agencies.

You should have an understanding of the homelessness process, either professionally or personally. You will also have a working knowledge of the available legal remedies, housing and welfare benefits, as well as experience of providing workshops.

Outside of this, you will have excellent administrative and time management skills and the ability to effectively manage a caseload.

About the role

The Hammersmith and Fulham Supported Pathway are recruiting for a Support Worker at The Old Theatre; a 12-bed female-only very high support needs hostel based in Hammersmith.

In this role, you will be supporting our clients on their road to recovery in a psychologically informed environment; aiding clients to gain necessary skills to move on from our services and into suitable accommodation. You will work with a caseload of clients, aiding them to gain necessary skills to maintain independence in the community. The role is focused on supporting residents around their strengths and aspirations for the future.

About you

These roles will suit people who can;

• Work with clients to identify their personal aims and ambitions and work towards achieving them.
• Provide practical and personal support to clients in an approachable, caring and person centred way.
• Work as part of a dedicated team to provide safe, high quality, positive and inclusive service to clients.
• Complete administrative records relating to client work, housing management and health and safety.
• Coordinate with external services relating to substance use, mental health, physical health, domestic violence and more, to provide a multiagency approach to clients’ support.
• Work on a weekly shift rota including early and late shifts, weekends and bank holidays.

Waking Night Behavioural Specialist Support Workers are responsible for ensuring continuity of a support service at night and for keeping customers and the building safe.

The shift pattern for this role: 21:30-07:30 10 hour night shift two nights a week

What you'll do:
* Building a supportive, trusting relationship with the customer and creating a positive atmosphere.
* Supporting the customer in their preferred night-time routine, including personal care, and promoting good sleep hygiene.
* Supporting the customer to undertake activities in line with their preferences.
* Ensuring the customer's safety and wellbeing at night, providing emotional and practical support and reassurance as needed.
* Using positive behavioural support and Autism-specific approaches to manage any distress during night-time hours.
* Working proactively with other members of the team to meet the needs of the customer and handle administrative responsibilities.
* Conducting key work sessions as required, that are innovative and engaging to achieve Support Plan goals.
* Ensuring ongoing assessment and management of risks associated with the customer within an attitude of 'positive risk taking';

1. Job Purpose

The Senior Mental Health Support Worker will provide time-limited, one-to-one emotional and practical support to individuals experiencing low to moderate mental health challenges, including those with complex needs. This role involves conducting initial referral needs and risk assessments, developing personalised support plans, and delivering face-to-face, remote, and, in rare cases, home-based support.

The postholder will also take a lead role in supporting and supervising 1-1 Support Buddy volunteers, ensuring they are well-trained, supported, and delivering high-quality interventions. Additionally, the role requires managing complex cases, offering tailored interventions, and collaborating with multi-disciplinary teams to ensure appropriate care pathways are in place.

2. Main Duties and Responsibilities

Client Assessment and Support Planning:

• Conduct initial referral needs and risk assessments to determine service user requirements and level of need.
• Develop and implement personalised support plans, setting clear and achievable objectives.

One-to-One Support (Including Complex Cases):

Oaklands & St Kilda's is made up of two newly refurbished six-bedroom homes, located close to each other in Ealing, West London. We work with up to 12 young people at a time, providing a safe, supportive, and welcoming environment. Many of the young people joining us are moving on from hospital, stepping down from more intensive placements, or transitioning from Child and Adolescent Mental Health Services to Adult services.

Our team delivers 24-hour trauma-informed and psychologically informed support. You'll play a key role in helping each young person shape their own support and service experience. Everyone receives a bespoke welcome pack and is encouraged to personalise their living space, helping them feel at home from day one.

We place a strong emphasis on participation and co-production. Young people are encouraged to make decisions about their care, engage in partnership meetings, and take part in activities that build skills and confidence - whether that's cooking, budgeting, managing medication, or accessing education and work opportunities.

Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

This is a Waking Night role working to support young people throughout the night at the service.

The Rapid Intervention team, will be providing vital outreach support to individuals who are currently rough sleeping across Westminster. The team will work with the objective of supporting those rough sleeping to find a suitable and sustainable route away from the street.

Our belief is that rough sleeping is an intrinsically harmful and dangerous activity both for the individual and for the wider community. Therefore, we take a proactive and assertive approach to supporting people away from this activity. Our aim is to support people to recover from the effects of rough sleeping and move away from a street lifestyle permanently.

• Working as an Outreach Worker you will work with people to assess their needs and deliver a person centred support, as well as working in partnership with multi-disciplinary teams and agencies across the city to meet client’s individual needs.

• It’s not always an easy job, but the opportunity to work with different people every day, helping individuals experiencing homelessness and the local community end the harms of rough sleeping can be highly rewarding.

You will work on a weekly shift rota to ensure we are available to support people experiencing homelessness when they need us the most.
