The post-holder will be responsible for deliver a range of support interventions for people newly diagnosed with HIV, and those re-engaging in their HIV care, who may be identified via Emergency Department Opt-Out testing in Liverpool.
Collaboratively work with Liverpool University Hospitals NHS Foundation Trust (LUFHT) to ensure people living with HIV receive practical, social and emotional support. The delivery of peer support in a clinic setting and, in the community, and onward referrals to relevant services.
Ensure that people living with HIV in vulnerable circumstances are supported to minimise the risk of them returning to ‘need to find’ status and remain engaged in services.
We seek someone with experience working with diverse communities. Knowledge and understanding of the common experiences and need of people living with HIV is essential.
The ideal candidate will possess a relevant qualification (such as an NVQ Level 3 or above in social work, health, education) or equivalent experience, and demonstrate a commitment to continuous learning and professional development.

Age UK Bromley & Greenwich is seeking a dedicated and compassionate Care Navigator to join our expanding team in partnership with Oxleas NHS Trust. Based at Memorial Hospital in Woolwich, you will be embedded within a multi-disciplinary team working on the Proactive and Frailty pathways. Your focus will be on supporting older people with complex needs, helping them navigate health and social care systems, and connecting them to relevant services across the community.
This is a key role that sits at the heart of person-centred care. You will attend multi-disciplinary team meetings, work alongside professionals including GPs, case managers and mental health staff, and spend time speaking directly with patients and their families. Your role will be to truly listen, identify what matters most to the individual, and help them take practical steps towards better health, independence and wellbeing.
Key responsibilities include:

Attending Multi-Disciplinary Team (MDT) meetings and contributing to holistic care planning

Working with patients to understand their needs, provide emotional support and signpost to local services

Liaising with a wide range of professionals across the NHS, social care and voluntary sectors

Inputting information into systems including RIO and Framework I, with training provided

Keeping accurate records and contributing to service evaluation and improvement

The role of Health and Safety Advisor will be to review, monitor and advise on Health and Safety (H&S) processes, policies, risk assessments and safe working practices across the charity.
The successful candidate will report to the charity’s Risk Manager, but be expected to work closely with other teams, in particular the Estates and Facilities, and Operations teams.
A recognised Health and Safety Qualification (e.g. NEBOSH General Certificate, or equivalent) is essential for this role.
Overall objectives:
- To be the subject matter expert for staff, volunteers, visitors and contractors dealing with health and safety issues across all three sites.
- Write high quality practical guidance and policies for H&S at Battersea.
- Embed H&S policies and processes across Battersea.
- Help train and educate managers and team leaders in their H&S responsibilities.
- Develop and sign-off H&S risk assessments where appropriate.
- Work closely with the H&S Officer in the Estates and Facilities Team.
