Your role

This is a very special role to provide holistic health and wellbeing of those who visit our open drop-in at the Vineyard Community Centre in Richmond. To us, they are our guests, not 'clients', 'patients' or users'.

This will include personal health screening, and holistic health support; also to provide health education, and liaison with outside agencies and local surgery. Many of our guests have multiple and complex needs involving, mental health, drug/alcohol dependency and homelessness.

You’ll report to our Community Nursing Manager and work with our Case Worker and Caseworkers from the Glass Door charity, and external hosted partner organisations like SPEAR and C.A.B.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.

The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across london on a spot purchase basis.

Specialist Support workers will manage a caseload of customers to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.

Consists of working 5 days a week across all days, 8 hours shifts starting at either 8am or 2pm.

Suzy Lamplugh Trust is the UK’s pioneering personal safety charity and leading stalking authority. We were established in 1986 following the disappearance of 25-year-old Suzy Lamplugh, an estate agent and lone worker who went to meet a client and never returned. The Trust exists so that what happened to Suzy does not happen to anyone else. For the past 37 years, we have worked towards reducing the risk of harassment, stalking, aggression, and violence. Our Stalking Helpline offers support and advice to thousands of victims of stalking every year, while our bespoke advocacy teams work closely with partners across the Violence Against Women and Girls’ (VAWG) sector, the criminal justice system, and both statutory and non-statutory services to ensure our clients can live a live free from abuse and fear.

You will be working alongside a team of advocates who are dedicated to supporting and empowering victims and survivors of stalking. By working with a caseload of 20- 25 clients, you will have the opportunity to provide holistic and bespoke advocacy to your clients, whilst leveraging your knowledge of gender-based abuse to support the Trust’s specialist mission.

Purpose of Role:

· To provide expert advice and advocacy to victims of stalking, both within the context of domestic abuse, and non-domestic abuse related stalking.

· To provide guidance and support by telephone, email and face-to-face to those affected by stalking.

About the Youth Endowment Fund 

We’re here to prevent children and young people becoming involved in violence. We do this by finding out what works and building a movement to put this knowledge into practice. 

In recent years violent crime has risen significantly. Homicides, assaults, robberies and offences involving weapons have all seen sustained growth. We have also seen large increases in violent crime involving children and young people. This is a tragedy. Every child captured in these numbers is an important member of our community and society has a duty to protect them. 

The Youth Endowment Fund (YEF) is a charity with a £200m endowment and a mission that matters. We exist to prevent children and young people becoming involved in violence. We do this by funding great initiatives, finding what works and working for change – scaling up and spreading the practices that make a difference.

This role and why it’s important 

It's critical to our mission that we understand how and why young people become involved in violence. The YEF’s work combines conducting rigorous evaluations and research studies with hearing directly from young people. It is also important to us that young people are central to the messages we share externally.

Your role involves ensuring that young people’s perspectives are heard and integrated into our work. This includes supporting our Youth Advisory Board (YAB) and helping young people’s voices play a central part in YEFs external communications.

This role also supports the wider team managing grants, contracts and providing general team support.

About the role:

The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.

Main duties:

• To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
• Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
• Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
• Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

General responsibilities:

• In conjunction with your line manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of The Passage.

Desired experience:

• Significant experience of delivering support services to socially excluded clients in a street based/day centre or similar setting.
• Significant experience of working in a mental health and/or substance misuse setting, working collaboratively to achieve positive outcomes for clients.
• Experience of providing a fully sensitive and responsive service to clients in a psychologically informed manner.
• Experience of working cooperatively with external partner agencies, e.g. Police, Social Services, NHS, as well as housing providers and voluntary agencies.
• Experience of assessing client needs and supporting clients to address their needs through assertive outreach and case management.
• Experience of working in a client-centered recovery focused approach and the necessary professional social skills to initiate and maintain constructive and appropriate relations with clients and agencies.

Desired knowledge:

• Understanding the support needs of the street populations with complex health, and housing needs.
• Knowledge of services and legislative environment regarding housing, health, work and immigration as it relates to people experiencing homelessness.
• Ability to communicate, both verbally and in writing and collate and evidence work and outcomes using case management databases.
• Knowledge of Psychologically Informed Frameworks.
• An understanding of the causes of homelessness and, in particular, the needs of people who are multiply-disadvantaged in their homelessness.
• Knowledge of housing pathways available to dual diagnosis clients and how to access them.
• A working knowledge of how to support people with either substance or mental health needs.

Felstead street is a CQC registered care home that provide care and support for 24 adults with high level mental health needs but also with complex physical health needs.

As a Waking Night Mental Health Support Worker, you will play an integral role as a part of the team by offering the continuity of care and support in every aspect of their needs in line with individual support and risk management plans, such as medication administration, emotional support and regular welfare check, personal care and so on.

Shift: 10.15pm-8.15am. Must be able to work some weekends and bank holidays. 4 working days (10 hours x4) a week, according to the rolling rota.

What you'll do:

Buildng supportive, trusting relationships with customers and creating a positive atmosphere in the home.
Following lone working protocols and maintaining high levels of awareness to health and safety for self, colleagues and customers.
Supporting customers to maintain a safe, healthy and hygienic living / shared environment.
Providing emotional support and assistance to customers overnight who may be experiencing difficulties with their own mental health and seeking appropriate support.
Providing support with daily living activities including practical assistance to customers who may require this during the night.
Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
Develop and maintain links with all key agencies and service providers in the local community.
Managing the petty cash during the shift.
Continuing the infection control duties at night.
Maintaining fire safety and H&S at night, and managing the arising issues in line with the local protocol and Look Ahead policies and procedure.
Empower customers to ensure they receive the service and benefits they are entitled to
Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards
