KEY OVERVIEW

Reporting to the Housing First Team Leader, the Housing First Support Worker is responsible for coordinating and providing a personalised, trauma informed support service to people with complex needs facing multiple exclusion in line with Housing First Principles.  The Housing First Support Worker will be responsible for coordinating and providing a person centred, strength-based support service to create long term sustainable tenancies by empowering customers, increasing independence, and maximising connectivity with the local community.

ADDITIONAL INFORMATION: 

• Valid driver's license and access to a car business purposes
• Please note the working pattern for this role is Monday to Friday, with occasional weekend work required 
• This post will include a 6mth probation period.

KEY RESPONSIBILITIES:

1. Delivering wrap-around intensive, trauma informed support to people with multiple and complex issues within their own independent tenancies in the community.

2. Working with Housing First participants to help them articulate their goals and aspirations offering personalised support and promoting choice and wellbeing. Supporting Housing First participants to access community resources, to develop positive peer networks and meaningful leisure opportunities.

3. Maintaining accurate up to date records on the Inform case management system of progress against individual support plans, safety plans and outcomes information.

4. Regularly review the person-centred support plan to update, record and monitor progress of customer caseload.

5. Develop, maintain and co-ordinate effective working relationships with external partners and agencies, promoting effective communication with Housing First residents. Ensuring Connected Housing First is widely known and promoted in Mendip and Somerset.

6. Liaise with landlords to assist the person living in accommodation of their choice; including practical help with furniture; benefits advice and accompanied visits.

7. With the agreement of Housing First participants, to liaise with other agencies to facilitate support including substance misuse and recovery, offending and criminal justice, mental health, Adult social care, debt, domestic abuse, welfare and housing benefits, employment and training.

8. Promote, and actively encourage peer involvement within the service.

9. Complete dynamic risk assessments and maintain own safety by following lone working procedures.

10.  Adhere to good practice safeguarding procedures with a positive risk-taking approach to ensure the wellbeing of vulnerable people.

11.  To identify barriers of accommodation and managing tenancies, providing support including practical help with benefits, budgeting, furniture, neighbour relations, and liaising with landlords.

12.  To contribute to regular reports on the service evaluation project as required.

13.  To participate in reflective practice to support and enhance wellbeing in the workplace and personal development.

14.  Record all petty cash and other expenditure of project monies in line with Connect Centre policy.

15.  Work within the Connect Centre Lone Working Policy and other Connect Centre Health and Safety Policies as appropriate at all times

Role Overview

As a member of the Joint Leadership Team, the post-holder will contribute to the leadership, planning and delivery of cross-team projects, helping to influence and meet the charity’s strategic objectives. Specifically, they will lead projects and programmes to deliver improvements in quality and standards of health services for people affected by Crohn’s and Colitis. In doing so, they will work closely with not only the  Director of Services & Evidence and Heald of Health Services, but also external stakeholders including health and social care organisations, academics, researchers and other charities.  The post-holder will also be actively involved in leading, supporting and developing the Health Services team.

About You

Ideally educated to degree level or equivalent, you will have experience of delivering health service projects and programmes and have come from a health services or medical charity background. You will be adept at developing operational performance metrics, strategies and objectives and have a proven track record in developing strong working relationships with external stakeholders. Whilst not essential, experience of managing and leading a team would be beneficial, as would be an awareness and knowledge of the needs of people affected by Crohn’s and Colitis.

We are based in Hatfield, Hertfordshire, so we are easily accessible by road and rail. In this role, there is plenty of scope for working from home,  although there will be times when you will need to travel to events/conferences. You will be required to attend two Directorate days each year and occasional face-to-face meetings with your team or our partner agencies. In addition, the charity meets four times annually at its office in Hatfield (or a location in London) for its’ ‘All Staff Together’ days, at which attendance is mandatory.

Specialist Behavioural Support Workers are responsible for the customer's emotional, psychological wellbeing as well as providing support to help them develop the life skills they require to meet the assessed needs. At Nimrod House, this will include providing physical, emotional, domestic, and social care. They will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices.

What you'll do:
Behaviour Analysis:
- You are liable to observing and monitoring the customer's behaviours, the duration, the intensity, and the damage, as well as factors that may alleviate or trigger the behaviour.

- You require superb interpersonal and relationship skills and must have the capacity to handle psychologically behavioural changes.
- You understand and collaborate with the different support organizations accessible in the community.

Other Responsibilities:
- If a need arises, deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. In some services, this will include delivering personal and physical care as appropriate.
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
- Participate in the support planning and risk management, as facilitated by the lead Support Worker.
- Carry out security duties to ensure the safety of the customers and premises.
- Monitor the CCTV throughout the shift.

About the role: 

As Director of Services, you will be responsible for a wide range of services including Hospital services (A&E support, discharge and falls prevention), community services (activity groups, befriending, clubs), information and advice services (including benefits, housing, carers and statutory advocacy).

Your key responsibilities will include: 

• Provide strategic leadership, direction and development of all Age UK HHB Services, contributing to organisational strategy and the annual business planning process.

• To work with the leadership team to identify and resource development opportunities, and to ensure our activity is providing optimum impact to improve the lives of older people in the borough

• Line Manage and develop Head of Services, Service Managers and other staff as appropriate, including setting targets and monitoring performance by ensuring that regular supervisions and appraisals are carried out, and tackling any performance issues in line with policies and procedures.

• Ensure Age UK HHB staff and volunteers are provided with up-to-date information sources, including any changes in national and local policy. To ensure services achieve external and internal quality frameworks and best practice guidance and to drive efficient working practices.

• Develop and oversee systems for feedback from service users to ensure the work, ethos and culture of Age UK HHB are informed by older people.

• To ensure quality data is captured for all service delivery allowing us to measure reach and impact for effective performance management, overseeing the provision of monitoring reports and dashboards for funders and board

• To work with the CEO, Managers and Trustees to develop annual budgets and to ensure services and projects are delivered to budget.

• Work closely with external partners and commissioning/funding agencies to ensure that Age UK HHB delivers the range of services that are required and/or contracted, on time and on budget.

To be successful in the role you will need to be able to operate effectively in challenging times and a constantly changing working environment. Experience in successful leadership, managing people, organisational change and improving business processes to support the charity’s strategy will be essential to this role. 

Essential skills and experience include: 

• Educated to Degree level or equivalent, or substantial successful senior management experience.

The Heaney Cluster an intensively supported housing scheme for the most challenging customers in the borough. · We support ill or injured men and women to have a safe place to call home, a home that meets their specific needs.

We are seeking an individual to raise awareness of the Fund’s welfare offer amongst social welfare agencies across South and Central Scotland but also, crucially, to help address the issue of social isolation and feelings of loneliness experienced by some members of the RAF Family.

The post holder will be working with socially isolated individuals to identify meaningfully enriching opportunities to reduce feelings of loneliness, some of whom may have complex and challenging welfare needs, to develop actions plans and coordinate support to overcome any barriers preventing successful connection into these enriching opportunities, assisting them by identifying appropriate support from within the RAF Benevolent Fund and the wider statutory and military charity sector.

The successful candidate will have experience of working in a social welfare role supporting individuals, have strong people and relationship-building skills, as well as empathy with or understanding of the issues affecting the Armed Forces community.  Individuals with a social prescribing or working aged veteran support background are encouraged to apply.

This is a community-based position working from home but covering South and Central Scotland, delivering both remote and face to face support.  The candidate must have their own vehicle to use to travel around these regions.  Occasional travel to Edinburgh and London will be required.  The successful candidate for this role will need to be Enhanced DBS checked and prove they have the right to work in the UK.

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people.

Purpose of post

SUN was launched in 2021 and is a service delivered by MFT and SABP for people with difficulties associated with the diagnosis of personality disorder. The innovative SUN service is based on therapeutic community principles and members are expected to both give and receive support as part of their membership. 

The Role

The SUN facilitator with lived experience will be working closely with a SUN specialist clinician to deliver regular Service User Network (SUN) support groups in different community-based locations across Surrey and North East Hampshire.

SUN provides community-based, open access facilitated peer support groups and aims to help people develop effective ways of coping and improve their relationship with themselves and others.

We are looking for individuals who are highly resilient and are confident in sustaining an emotionally demanding paid role.

Key responsibilities include:

• Co-deliver regular SUN support groups in the community, both face to face and online, with a clinical facilitator.

• Work in collaboration with the co-facilitator and the SUN members.

• Use own lived experience of mental health problems associated with a diagnosis of personality disorder to support the development and knowledge of the service and wider services.

• Contribute to ensuring that any potential risk is being constantly assessed within a culture of positive risk taking and shared responsibility.

• Co-lead on different areas of the service on a rota basis, i.e., reporting, outreach, and member involvement.

• Undertake all the required training in line with the development of the post and as agreed with both your line manager within MFT and the service manager of SUN.

• Attend and contribute to additional service development meetings as required.

• Assist in the promotion of the service through active liaison, meetings, and presentations with people who access services, staff and other organisations.

About You

You will be committed to undertake training on SUN protocols and methods and obtain knowledge regarding mental health with specific focus on personality disorder and the recovery approach.

You will have:

• Personal lived experience of mental health issues associated with personality disorder.

• Previous experience of working within a professional environment.

• Knowledge of personality disorder, mental health issues and recovery approach.

• Ability to reflect on and appropriately share your own life experiences and personal experiences of mental ill health as part of the facilitating role of the groups.

• Ability to guide others in taking charge of their own recovery.

• Ability to understand and create healthy boundaries.

• Excellent interpersonal skills.

• Excellent communication and listening skills.

• Ability to be non-judgmental, demonstrate empathy, compassion and patience.

• Practical experience in facilitating / co-facilitating groups or of being a member of a support or therapy group.

• Ability to remain calm and respond in a professional manner to distress, disturbance and unpredictability.

• Sensitive understanding of diversity issues and ability to promote anti-discriminatory practice and equal-opportunities.

• Proficient in Microsoft Word, use of email, Outlook, and basic excel skills.

The charity is a mental health and emotional wellbeing charity, supporting Surrey residents since 1994 in Banstead, Elmbridge, Epsom & Ewell and Mole Valley.
