The Mental Health Practitioner will play a key role in helping individuals improve and maintain their mental and emotional wellbeing.

What You'll Do: -

- Innovation and Development: Channel your passion delivering and promoting wellbeing support by actively contributing to the creation and improvement of innovative programs, resources, and approaches that enhance the mental health and resilience of the communities we serve.

- Collaboration and Efficiency: Work together with existing Mental Health Provision for people, as well as other local agencies/organisations to effectively support people who are experiencing mild to moderate metal health challenges.

The role will focus on promoting positive mental health using the Five Ways to Wellbeing framework: Connect, Be Active, Take Notice, Keep Learning, and Give. This role involves delivering individual and group interventions, offering wellbeing planning, and signposting clients to appropriate community resources.

Key Responsibilities

  • Deliver one-to-one wellbeing support sessions tailored to individual needs.
  • Co-produce personalised wellbeing plans using the Five Ways to Wellbeing.
  • Work with the person to identify their recovery pathway to move towards their goals, maintaining a person centred, flexible approach walking alongside the person, implementing interventions/safety plans.
  • Facilitate group wellbeing sessions or workshops on topics such as resilience, stress management, and lifestyle habits.

· Provide support to local health services with their SMI (Severe Mental Illness) clinics 

· Support the improvement in attendance and quality of annual health checks of Individuals who have existing mental health conditions in the community. 

  • Encourage people to participate in local activities aligned with the Five Ways to Wellbeing (e.g., walking groups, creative workshops, volunteering).
  • Support individuals to develop coping strategies, increase self-awareness, and set achievable wellbeing goals.
  • Promote self-management and long-term wellbeing beyond the scope of the service.
  • Ensure ICT systems and data recording systems are used appropriately in line with policies and procedures. Work with colleagues to ensure effective internal systems including but not limited to maintenance of records and GDPR compliance.
  • Carry out work in accordance with policies and service procedures, current legislation and quality standards. Be prepared for, attend, and engage in staff meetings, supervision, appraisal and training as agreed with your line manager.
