About the Youth Crisis Service

Our Young Person’s Crisis Service offers a safe, welcoming, and non-judgmental environment outside of normal school hours for those aged 14 – 17 who feel in mental distress or crisis.

About the Role

The Youth Mental Health Worker will support young people aged 14-18 to deescalate crisis and improve their mental health and wellbeing, through focussed 1:1s and structured crisis interventions. The role will involve assessing needs and offering one-to-one support to service users to enable them to begin the process of improving their mental health and well-being and to provide clarity on services and pathways available in the area. The role will be part of a larger Young Person's Crisis team made up of Youth Mental Health Workers, Managers and volunteers.

The successful candidate will have experience of working with young people (under 18) with mental health issues, experience of assessing level of need and experience working in a team or small service.

Service Delivery

• Deliver crisis focussed 1:1s and ensure outcomes which supports young people experiencing a mental health crisis in line with Mind BLMK’s agreed crisis service and requirements as well as contract requirements.
• Assessing need and suitability for the service in line with the access to services process, and undertaking 1:1s.
• Contribute to maintaining an effective volunteer workforce for the service in line with Mind BLMK’s HR policies, procedures and guidance (workload planning and support).
• Carry out health and safety and cash handling in line with Mind BLMK’s policies, procedures and guidance.
• Carry out health and safety responsibilities as directed by the Team Leader in line with Mind BLMK’s H&S policies, procedures and guidance (risk assessments, reviews, reporting, safeguarding, incidents/accidents, premises).

Attraction of Volunteers

About the Service

The Youth Mental Health Navigator supports young people who have been referred to the CAMHS front door/single point of access. The service is suitable for people who have mental health / wellbeing needs but are not felt to be at immediate risk of suicide.

About the Role

This role will provide individualised, one to one support to young people, aged 14-17 who are currently experiencing difficulty with their mental health and wellbeing.

This post holder will be working closely with clinical colleagues in Child and Adolescent Mental Health Services (CAMHS) team offering hybrid in person and online support. The post holder will also be responsible for the development of pathways which enables young people to access services offered by the Voluntary and Community sector.

The post holder will ensure they have a good understanding of the services on offer by the Voluntary and Community sector, and other agencies, and will work with individuals to help them access services of their choice.

The Youth Mental Health Navigator will offer mental health and wellbeing support in a safe and enabling service, supporting young people to explore their challenges and identify positive mental health coping strategies, resilience skills, distress tolerance and a solution focused approach to recovery that will help to future proof against potential repeated incidences of poor mental health.

We have a vacancy for a Flourish (Refugee and Asylum Seekers) Comminity Outreach Worker to join our team, supporting refugees and asylum seekers in the community with multiple needs who have mental health as a primary need, through providing advice, information and holistic support.

Complex needs are defined by the National Complex Needs Alliance as a combination of health needs and social needs.

About the Service

‘Flourish’ is a highly flexible and responsive outreach service developed to meet the needs of refugees and asylum seekers in Hertfordshire. The aim of the service will be to support refugees and asylum seekers in the community with multiple needs who have mental health support as a primary need. 

Purpose of Post

• To provide advice, information, onward referral and holistic support to clients with mental ill health.
• Support to build social networks and reduce isolation whilst settling into the Hertfordshire community this includes building a sustainable and empowering peer support network and building confidence and self-esteem.
• To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
• To remain a source of independent support for all clients.
• Embed a person centred and solution focused approach in all aspects of the role.
• To work in a trauma informed way including awareness of the 6 principles of trauma informed practice, putting the clients emotional and practical needs at the forefront of this work.

Key Responsibilities:

• Working closely with all key delivery partners across the system to ensure the service is responsive, inclusive and easy to access.
• To take allocated referrals from any agencies including the Community Navigators, Borough and District Councils, Herts Help, Housing Providers, Herts Welcomes Refugees, the Refugee Council and self-referrals including walk in contacts at any of the Mind wellbeing centres across the county.
• Hold a caseload of clients, supporting for approx. 10 weeks
• To create a safe space for the individual to be able to explore their feelings and improve their understanding around their mental health.
• To complete a holistic needs assessment.
• Ensure all person centred risk assessments and support plans are completed.
• Proactively recognise the indicators of deteriorating mental health and facilitate appropriate action, whilst liaising with relevant agencies.
• Support clients to build their emotional resilience, promoting self-management through building an understanding of their own triggers/coping strategies.
• To provide safeguarding intervention support. This will include responding to problems, which pose an immediate risk.
• To work alongside translators when required to provide a robust package of support for clients.
• To accompany clients to initial visits and appointments as necessary in order to ensure the sustained use of existing support services in the Borough.
• To provide advocacy services to clients experiencing mental ill health.
• To assist clients to access appropriate mental health services, housing/tenancy services and other services according to assessed needs.
• To establish effective pathways across a variety of agencies to maximise and provide effective partnership working, working with other third sector and VCSE organisations to provide signposting and continuity of care.

We are looking for a Domestic Abuse Support Worker to join an exciting partnership project in Stockport.

You will support victims of domestic abuse within the Stockport Borough offering a person centred and trauma informed package of support and advocacy. 

The role 

In this role you will complete risk and needs assessments with victims of domestic abuse who live in the borough of Stockport. You will liaise with referring agencies, actively contribute to risk management procedures and team working. You will carry out a large and varied range of practical support which may include safety planning, advocacy, risk assessment and risk management. We also offer emotional support to those people we work with, encouraging and supporting them to rebuild lost confidence and self-esteem. We are a consent driven service and therefore the needs of the person we are supporting is at the core of all the work that you will do. 

As part of the Stockport Domestic Abuse Support team, you will also be supporting the partners of those who are undertaking behaviour change work on the Striving to Change Programme. 

You will attend regular risk management meetings to ensure that both parties are fully supported and that the risk is appropriately managed.

You will be motivated to support people who have been impacted by domestic abuse and using person centred and trauma informed practice you will work to improve the experiences of all those impacted by domestic abuse.

As a Support Worker you will be expected to enable and support vulnerable individuals with complex needs to address this while maintaining their tenancy with the long term goal of living in an independent setting by providing flexible and individually tailored support package. Working with excluded and hard to reach customers can be challenging, however, with intensive support this challenge can be broken. You will need to be able to engage our customers in a non-judgmental way. You will also be responsible for dealing with emergencies that may arise during your shift whilst remaining calm and using initiative then writing up incident reports amongst other administrative tasks.

As a support worker, you will be working as part of a team as well as with external agencies such as London Probation Service, local substance misuse services, Mental health teams, and police amongst others. Sometimes customers may not be linked in with agencies when they entre the service, as a support worker it is part of your role to ensure they are receiving the correct support from agencies required. Ideally, you will have experience of working with vulnerable people or an understanding of the needs of homelessness. You will be required to support customers with managing their arrears; ensuring housing benefit is applied for as well as monitoring payments and acting appropriately if payments are suspended in order to ensure customer does not fall into any arrears.

You will be responsible for supporting the working shift pattern of the service which is a five week rolling rota which consists of working some weekends as well as long days.

There will be a rota for shifts, with working patterns of 7:30-15:30 and 12:30-20:30. There will be some weekend work.

This role requires a valid Right to Work for the UK as Look Ahead are not a sponsoring organisation.

What you'll do:

Work proactively with Slough Hostel team to handle the service caseload and support an effective team approach to meeting each customer's identified needs.
Motivate the team and champion a positive local culture within the service.
Contribute to a positive service environment, ensuring the service is supportive and a place of safety and stability for customers. This includes working flexibly and being proactively in delivering, reviewing and arranging effective support for emerging needs and risks in order to prevent escalating concerns.
Deliver all aspects of support to enable a customer to develop independent living skills as appropriate to the individual needs of the customer. This may include practical tasks, such a facilitating group actives
Support customers to undertake domestic tasks, including practical assistance to ensure a high quality living environment that promotes customers' dignity, respect and allows customers to feel safe and welcome in their own home.
Participate in support planning, risk management and assessment of needs in line with support planning.
Work with customers to promote social inclusion to promote independent living, encouraging customers to maximise their skills and choices.
Involve customers in the design, development and delivery of the service through co-production.
Report and react to any observations relating to customers welfare.
Proactive development of links with local statutory and voluntary organizations to provide a range of engagement opportunities for customers in the community.
Competent use of relevant systems, company policies and frameworks.
Adhere to all the policies and procedures of Look Ahead Care & Support plus those specific to the project including any statutory requirements.
Carry out other duties commensurate with the designated role level, as may be deemed reasonable by Look Ahead including working at an alternative patch, if such a need arises.

Support Coordinator

We’re looking for an innovative, passionate, and professional individual with excellent communication and organisational skills to join our Stroke Recovery Services based in the Dorset East area.

There will be opportunities to hot desk within Bournemouth and Christchurch, as well as Poole NHS sites.

This is an exciting opportunity to work with stroke survivors and their families to provide Key Worker support following a stroke.

Salary: Circa £21,391 per annum

Contract: This is a fixed-term contract until 30 June 2026. We have funding for this role until 30 June 2026.

The Role

The service aims to identify and address the needs of stroke survivors and carers across the stroke pathway, by providing a range of innovative support solutions, supporting them to meet their desired outcomes.

The Stroke Association Support Coordinator will:

· Support new stroke survivors and their carers from hospital discharge into the community.

· Provide personalised information, advice and support.

· Support stroke survivors to make informed lifestyle changes which will help them to prevent further strokes.

· Work collaboratively with NHS colleagues and other areas of the community to make a difference in the lives of people affected by stroke.

About You

The post holder will have experience/background in:

· A caring profession ideally with experience of supporting people with disabilities.

· Setting up and running virtual groups.

This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role.

About this exciting opportunity

As Mentor/Facilitator, your focus will be to oversee the delivery of a vital Mentor Programme to targeted at risk students and young people up the age of 25 years and to establish and maintain positive relationships with each young person to provide emotional support, guidance and mentorship.  You will be expected to address each individual’s needs and challenges, utilising personal experiences, local knowledge, professional skills and St Giles’ materials to offer support and intervention, while also assisting each young person with setting and achieving academic, personal, and career goals.

We will count on you to collaborate with professionals to support and develop strategies to minimise the risk to young people and to maintain a trauma informed approach, that recognises and responds to trauma and amplifies the voices and experiences of individuals.  Developing positive, pro-social attitudes, behaviour and social skills among those you mentor and supporting and/or co-delivering whole school awareness and professional sessions are also key aspects of the role.

• An understanding and demonstration of professional standards and accountability
• Experience of working collaboratively with multi-agency partners
• Excellent record keeping and good technical literacy of Microsoft Applications
• A commitment to excellent safeguarding practice
• Excellent interpersonal, relationship-building and communication skills, verbal and written
• A flexible, collaborative and professional approach to your work
• Full Driving Licence and your own transport.
