Job description 

Job Purpose 

The hospital to home service works collaboratively with hospitals to support people when they are discharged from hospital to return home. As a Hospital to Home Outreach Worker, you will provide temporary, practical and emotional support to service users within their home and make sure they understand the care and local community services available to them.

The service is time limited, usually up to 4 or 6 weeks, to adjust and settle back in their homes. 

Key Tasks 

• Assess the needs of service user’s either in hospital before discharge, or in their homes, and understand what barriers and enablers there might be to moving home. 

• Aid the discharge process and improve patient experience through tasks such as:

• Key cutting

• Co-ordinating with colleagues for the setup of the home environment such as checking fridge for in-date food 

• Checking heating, electric and gas is functional prior to discharge 

• Allowing access to the property for cleaning/repairs and receiving equipment

• Provide and/or arrange the practical and emotional support needed to enable service users to return home and/or remain safely at home. 

• Be vigilant and make referrals/signpost for additional needs that our wider team, or partners can support. E.g. for benefit checks, energy advice, carers support, befriending.  

• Help maximise service user independence by adopting an enabling approach. 

• Develop a listening, caring and professional relationship with the service user and their families/carers. 

• Provide domestic practical support within the home such as:

• Light cleaning,

• Washing (e.g. bedding),

• Changing bed sheets etc. 

• Prepare and service drinks and simple meals with or for the service user ensuring nutritional needs are met. 

• Accompany service user outside the home e.g. hospital/GP appointments etc. 

• Support service users with other tasks e.g. making telephone calls on their behalf, reading and responding to correspondence, completing simple forms (training will be provided) as directed by the service user’s etc. 

• Ensure services provided by AUKEL in the community are safe and person centred. 

• Report safeguarding concerns, in accordance with AUKEL safeguarding procedures. 

• Provide cover in the case of sickness and annual leave periods of colleagues. 

• Support service user reviews, ensuring all visiting information is uploaded on to Salesforce. 

• Support volunteers in the team and provide support and guidance to their development. 

• Work with a range of professionals including: 

• Supporting health and social care professionals with patient discharge 

• Liaising with the hospital health and social care professionals (e.g. Social Workers) to facilitate smooth supported discharge. 

• Handypeople & contractors.

Administration  

• Input all records, reviews, and visiting information to service user’s case notes in a timely manner to maintain up-to-date and accurate records on Salesforce.

• Report all “no access” failed visits, or changes in service user’s condition or circumstances in accordance with AUKEL’s policies and procedures. 

• Ensure service user comment sheets are completed.

• Ensure service user’s feedback questionnaires are sent at the end of each intervention and record returns. 

• Submit travel expenses regularly for sign off by manager. 

Quality  

• Be familiar with and to implement AUKEL policies and procedures in line with our Values. 

• Ensure the service is delivered in accordance line with Care Quality Commission principals. 

• Ensure integration with other AUKEL projects, and service users are referred and/or sign-posted to appropriate support services available to them.

• Respond to safeguarding or other concerns and make appropriate reports and management action.

Liaison  

• Work in collaboration with other agencies providing support within the service user’s home. 

• Work under the direction of the service manager and project officer/s. 

• Represent AUKEL and participate in appropriate external meetings and events to remain aware of local, regional, and national issues affecting quality and compliance issues affecting care and home support services. 

 General 

• Meet regularly with your line manager for support, supervision, and appraisal. 

• Attend team and staff meetings, and other meetings as required. 

• Undertake any other duties within the competence of the post holder as may be required from time to time for the continued smooth running of AUKEL. 

• Complete any training which is required to fulfil the role. 

• Carry out the duties of the post in accordance with AUKEL policies and procedures including Equal Opportunities, Mental Capacity, Deprivation of Liberty, Food Hygiene, Health & Safety, Confidentiality, Complaints, GDPR (General Data Protection Regulation), Safeguarding Vulnerable Adults etc.

• The post holder reports to the manager /or Project Officer.

Job Title: Duty Independent Gender Violence Advocate (Duty IGVA)

Location: Lambeth, London
Salary: £29,701.36 per annum (Inclusive of London Weighting, which may not be applicable depending on your home location and any agreed permanent homeworking arrangement)

Contract type: Full Time, Fixed Term Contract (Until March 2027)

Hours: 37.5 hours per week. As part of this role, you will be required to participate in an out-of-hours on call rota. Operating hours of the service 8am- 6pm Monday- Friday with the expectation to work on some days 8- 4pm or 10- 6pm to cover duty shifts.

We are recruiting for a Duty Independent Gender Violence Advocate (IGVA) who will be working closely with survivors of domestic, and other gender-based abuse to provide high quality independent support, guidance and advice to survivors across all risk levels and their children.

In this role the Duty IGVA is a part of a busy team. The Duty IGVA will be the first point of contact for survivors who have been referred to the Gaia Centre by other professionals. The Duty IGVA also will be responsible for processing any self-referrals of individuals contacting the service and wishing to access support. The Duty IGVA will be responsible for responding to daily enquiries from partnership agencies, public and impacted individuals over the phone and email.

In some circumstances the Duty IGVA will be contacting survivors directly to assess their level of risk and needs to ensure eligibility. The Duty IGVA will be responsible for carrying out and implementing safety plans and needs assessments. The Duty IGVA will ensure an effective handover of the case to the relevant team for ongoing support.

The Duty IGVA will empower survivors by providing them with emotional, practical and personal welfare support. The Duty IGVA will ensure that survivors are provided with a safe, supportive and welcoming environment, enabling them to access their rights, make decisions and increase their life options. The job involves working within a multi-agency framework consisting of the MARAC and local partnership protocols and procedures that prioritise the safety of survivors.

The job involves informing survivors of the full range of civil, criminal and practical options that might increase their safety. The jobalso  involves working in a fast-paced environment.

As part of this role, you will be required to participate in an out-of-hours on call rota.

 Your Role in Growing Our Forest Story

As the New National Forests Project Manager, you’ll lead on effective project management of centralised activities for the three new national forests.

Specifically, you’ll manage the centralised reporting of progress, targets and budgets, liaising closely with the programme managers for each new national forest.

Health and Safety:

In the 1980s, high unemployment and steep inflation was contributing to a shocking rise in youth homelessness across London. Thousands of young people were sleeping rough every night, with many areas notoriously dubbed “cardboard cities” due to the visible rise in street homelessness. We’re Hertfordshire Mind Network

We deliver essential mental health support in Hertfordshire, providing a diverse range of services from our Wellbeing Centres and other locations across the county. We help individuals experiencing mental ill health to make choices, find their solutions, build resilience and manage their wellbeing. We offer opportunities for individuals to get support to allow them to recover from or live with mental ill health.

About the role

As People Officer, you will provide high-quality, accurate support across the full employee/volunteer lifecycle, while also contributing to employee and volunteering relations, and continuous improvement of our People & Culture services. You will be a role model for our values and a key contributor to a positive team culture

As People Officer you will be part of a team who support a headcount of approximately 250 employees, 200 volunteers and 150 self-employed individuals across Hertfordshire. You will be instrumental in ensuring that all individuals employed or volunteering within Hertfordshire Mind receive advice, guidance in a responsive, knowledgeable, and customer-focused way.

Key Responsibilities include:

Employee Lifecycle

• Deliver a high-quality, confidential and customer-focused People service from recruitment through to leaving.
• Advise line managers on recruitment; selection, interview and safer recruitment
• Provide People advice for queries from the People & Culture team in line with policies, managing and or escalating complex issues as appropriate.

Systems, Data & Reporting

• Produce timely and accurate People management information to assist organisational decision-making. (ihasco, Breathe, Livepay, Charity log, etc)
• Maintain accurate and up-to-date records within the HR information system (Breathe HR), including new profiles, personal data, annual leave and absence reporting.
• Oversee monthly payroll data collation, including hours, absences, and any contractual changes, ensuring timely submission to Payroll.
• Ensure Breathe annual holiday renewal and other system updates are effectively completed and communicated

Employee Relations & Case Management

• Manage employee relations cases, such as sickness absence, conduct, capability, disciplinary, grievance, family focussed leave, flexible working requests.
• Undertake casework, guiding and advising on relevant procedure including gathering evidence and drafting investigation reports, tracking case progress and ensuring communications and actions are completed.
• Provide consistent and fair advice to managers in accordance with policies, escalating complex issues as appropriate.

Projects & Continuous Improvement

• Contribute to projects such as employee engagement initiatives, policy updates, wellbeing programmes, EDIE (VIBE, Voice, Inclusion, Belonging, Engagement) and system improvements.
• Support quality assurance processes in line with National Mind policies and ISO9001 standards.
• Identify opportunities to improve people processes, recommending and implementing enhancements under the guidance of the People & Culture Director

General Responsibilities

• To support any member of the People and Culture team and to cover team absence as required.
• Note taking for meetings as required
• Promote Hertfordshire Mind services by attending events, campaigns, and open days to raise awareness of mental health.
• Uphold Hertfordshire Mind policies and procedures, including the Equalities Statement.
• Maintain strict confidentiality at all times.
• Undertake any other reasonable duties as required by the Senior Leadership Team.

The Opportunity

This is an exciting opportunity for an organised and proactive individual to join a small, friendly, and dynamic team in a varied role. If you are looking to build your fundraising experience, develop new skills across multiple fundraising areas, and have your work make a tangible impact on people’s lives, this could be the perfect role for you.

Within a small team of executive and assistant roles, you will work closely with the two Fundraising Managers (one focused on Trusts and Foundations and the other Corporate Partnerships) and the Development Director to support corporate, trust, foundation, and individual fundraising efforts. The role offers the opportunity to contribute across a broad range of fundraising activities and, depending on your interests and experience, take ownership of key tasks and projects.

Joining during a time of growth and strategic development, you will have the chance to be part of the implementation of a new Fundraising Strategy. This is a fantastic opportunity to gain valuable experience in donor stewardship, event coordination, individual giving campaigns, and data management, all while contributing to the success of the Warm Welcome Campaign.

In the role you will:

Assist the team across the fundraising function supporting corporate, trust, foundation, community and individual fundraising activities.

Develop and maintain positive relationships internally with colleagues and externally with stakeholders, donors, and funders.

Demonstrate strong organisational skills and reliability, maintaining systems for data management and accurate records of fundraising activities and donor interactions.

Liaise professionally with Warm Welcome Spaces, funders and supporters, acting as a warm and approachable communicator.

Confidently use and manage our CRM system (Beacon) to track donor data and fundraising activities.

Support basic marketing efforts, including creating quality written content such as emails and social media updates, as needed.

Securely store and organise documents on our G-Drive, ensuring accessibility for the team and compliance with GDPR and good fundraising practice.

Support the logistical planning for fundraising and wider team events.

Ensure deadlines are met, prioritising a varied workload and maintaining attention to detail in all tasks.

Take initiative to explore new areas of fundraising and contribute to innovation.

Own and manage delegated tasks or parts of projects, ensuring successful and timely delivery of work.

Essential Skills and Experience

1. Strong organisational and administrative skills with a track record of managing multiple tasks and meeting deadlines.

2. Experience in using CRM systems (e.g. Beacon) and proficiency in Microsoft Office and Google Suite.

3. Good written and verbal communication skills, with the ability to liaise professionally with diverse stakeholders.

4. A reliable team player who can prioritise work effectively and manage tasks under pressure.

5. Good numeracy skills and the ability to work on simple budgets.

KEY RESPONSIBILITIES:
•Managing the continued development of the YAA’s CRM system, working with the Fundraising, Marketing, Admin and Finance teams to ensure the system serves their requirements.
•Managing all aspects of the database and associated data systems and processes as part of YAA’s fundraising activities and the Charity’s web systems.
•Responsible for ensuring the Charity can demonstrate compliance with new and existing Fundraising and Data Protection Regulations including the General Data Protection Regulations. Keeping up to date with any changes in policy that may affect the management of supporter data.
•Liaison with the YAA Fundraising, Marketing, Administration and Finance Team on data/data selection and reporting requirements.
•Promote and support data audit, cleansing and effectiveness within YAA, deploying and providing day to day management of the CRM system and providing reports and analysis of the data as required.

KEY DUTIES:
Provide the expertise and insight to support an integrated data strategy across fundraising and marketing, advising on and sharing best practice:
1. Work with CRM users to ensure best practice is adhered to across the department and maximise fundraising performance.
2. Identify opportunities and apply insight and data analysis within the database to improve our income generation performance and opportunity.
3. Communicate and build relationships with external suppliers to inform the data strategy and database development and secure the smooth delivery of database services through the database.
4. Keep up to date with current database and legislative developments and consider how they should be applied at YAA.
5. Work collaboratively across Fundraising, Marketing, Admin and Finance to bring systems and data expertise to the launch of new fundraising or communications products, projects, systems or new ways of working.

Work with fundraising teams to develop reporting, analysis and data selections to support improved retention, income generation and ROI and put the supporter at the heart of data planning:
1. Be responsible for data profiling, segmentation and selections for fundraising and marketing activity to support increased income generation and supporter retention, ensuring data selections are planned and organised effectively across fundraising. Advise teams on selecting data, exclusions and undertake data selections.
2. Work with fundraising teams and external agencies to support the development of long term sustainable supporter journey planning.
3. Lead on fundraising data analysis and data quality in developing and implementing reporting tools, producing regular management information for performance management and campaign evaluation.
4. Create a library of reports and dashboards colleagues can self-serve in real time.
5. Provide relevant data to SMT and Trustee Board.

Maintenance of the database and ensuring data integrity:
1. Be responsible for technical maintenance of the database and liaising with suppliers to ensure integration with digital fundraising platforms as required and the automation, where possible, of importing high quality data.
2. Ensure data and income are reconciled between database and finance functions.
3. Ensuring data is secure, there are adequate back-ups and that data can be retrieved in the event of a system failure.
4. Oversee the import and export of data to and from external suppliers (in conjunction with Digital Marketing Manager as appropriate). Investigate and resolve anomalies, liaising with suppliers, third-party agencies, or colleagues
5. Manage the set-up, documentation and maintenance of coding structures on the database to ensure effective data selection, analysis and reporting across all fundraising.

Ensuring compliant use of data and the database:
1. Be an expert on relevant Data Protection legislation and Fundraising regulations and lead on this for Fundraising. Ensure the compliance with legislation and best practice. Raise the profile of data protection and data quality issues.
2. Audit the database regularly to ensure data is clean, accurate, archived in line with data retention policies and that users are following procedures for data entry and management.

Develop and implement a programme of training to support all database users to maximise the benefits the CRM brings:
1. Develop and deliver comprehensive induction and ongoing training and associated ‘How to’ documentation for users.
2. Develop, implement, communicate and regularly review database protocols and procedures ensuring they are up to date and being followed by staff.
3. Work closely with users to identify opportunities to improve processes and ways of working across YAA, including updating and maintaining internal user guides and process maps.
4. Provide technical support for users.

Staffing and Budget Management
1. Train staff and volunteers in the use of the data management systems.
2. To work closely with the finance team in ensuring fundraising income data is robust and meets all relevant financial management and reporting requirements.
3. To propose and monitor the CRM budget and manage the relationships with key suppliers to ensure value for money for YAA.

Knowledge & Skills Required;

Essential:
❖ Previous or current experience in a Database Management role
❖ Demonstrable record of success in working with CRM’s (YAA’s CRM is Beacon)
❖ Experience of data import protocols – extracting data and transforming it ready for import
❖ Experience of relationship building and management with various stakeholders internally and externally
❖ Proficient in MS Office, particularly Word, Excel, PowerQuery and Outlook
❖ Highly organised, meticulous with a thorough approach to work
❖ Ability to see projects through from start to finish, managing time and tasks effectively with a varied workload, including following up with colleagues and stakeholders

About The Music Works

The Music Works is a Gloucestershire charity that transforms young lives through music. We’re specialists in working with young people in challenging circumstances to help them reach their full potential in music, in learning and in life. We work with over 3,000 young people a year in schools and our four community studios in Gloucester, and the Forest of Dean. Our approach is youth-led, with young people involved at every stage of planning and delivery.

Main Duties & Responsibilities

The function of the Maintenance Operative is to support the safe, compliant and effective maintenance of the Hospice estate, including grounds and retail properties, by undertaking a wide range of reactive and planned maintenance tasks.

The postholder will ensure that facilities remain safe, functional and well-presented, working under the direction of the Estates & Facilities Lead to deliver maintenance activities, support statutory compliance, and assist with the coordination of contractors and improvement works across all sites.

1. Daily walk-through main site & grounds to identify problems/issues including checks of lighting, wells, sewage system, CCTV/security & BMS login
2. Report & take action on any breakdown or defects in premises, copiers, plant, equipment & grounds taking action promptly, notifying senior managers & prioritising safety
3. Undertake a wide range of general maintenance and minor works, including basic plumbing, carpentry, decorating and repairs, ensuring work is completed safely and to an appropriate standard.
4.  Organize moving & storage of items delivered to the main hospice site
5.  Oversee contractors on site as agreed with the Estates & Facilities Lead
6. Ensure weekly check of generator fuel, roadworthiness of all hospice vehicles, fire alarm test, flushing & logging results on a web portal & agreeing a remedial plan with Affinity water if necessary
7. Management of copiers and any issues with them to include lease renewal
8. Support the Estates & Facilities Lead with weekly maintenance support & response to emergency facilities requests for all retail premises, liaising with shop staff as necessary & ordering & collecting supplies
9. Manage and prioritise jobs as required using the PPM
10. Use the Estates PPM system to log, update and close jobs, ensuring accurate and timely record keeping of all maintenance activities
11. Participating in the departmental meetings reporting on: Quantity of outstanding works &/or repairs with subcontractors, health and safety and any regulatory compliance risks
12. Support the coordination of servicing and maintenance of clinical equipment as required, recognising that responsibility for clinical assets and compliance remains with the relevant clinical teams.
13. Progress chasing outstanding work with the in-house team and contractors
14. Ensure all weekly checks and planned tasks are carried out
15. Ensure drinking water stations are replenished whenever necessary
16. Monthly check of emergency lighting
17. Check products against the COSHH register and dispose of any that are not required or should not be held.  Carry out COSHH Risk Assessments as necessary
18. Check all Health and Safety Notices are current and update/replace/remove those that aren’t
19. Co-ordinate quarterly fire drills, support the Fire Marshalls meeting and ensure feedback at Link Up
20. Support the Estates & Facilities Lead in water management (including septic tank and well water filtration) in compliance with statutory processes & procedures to ensure HSG274 compliance
21. Ensure all quarterly checks and planned tasks are carried out, including in-house PAT test scheduling
22. Support the Estates & Facilities Lead to ensure compliance with biannual services of all patient equipment subject to LOLER regulations, the biannual water quality test, the dismantling and clean extractor fans, ventilation shaft and solar panel checks
23. Check Hospice vehicles ensuring records of services, inspections, road tax, MOT; and that the competencies and licences of minibus drivers comply with appropriate regulations
24. Assist with the preparation of the annual maintenance budget
25. Assisting with energy monitoring and reduction strategies
26. Provide direction and support to maintenance staff
27. Promote a proactive, solution-focused team culture
28. Ensure safe working practices are followed at all times

Qualifications, Skills, Experience, Knowledge & Approach

• A good standard of general education.
• Proven experience in estates, facilities or maintenance environment
• Strong practical, hands-on maintenance skills
• Experience coordinating maintenance activity
• Experience working with contractors
• Ability to work independently and make sound decisions
• Knowledge of health and safety compliance requirements, including correct use of PPE, COSHH, Manual Handling, Legionella, Risk Assessments etc
• Ability to work with a diverse team of volunteers effectively i.e. with varying motivation, age, gender, and physical and mental capacities
• Maintaining a large and varied Estate to a high standard, including grounds, structures and buildings
• Proficiency in operating various tools and powered equipment
• Training others on maintenance techniques and safe use of equipment
• Monitor and address hazards such as severe weather conditions like heatwaves, snow, ice, and remove litter to ensure safety of visitors, volunteers and staff
• Proficiency in operating various garden tools and powered equipment

· The post holder will be required to visit other hospice building (shops) or event and will need to have the availability to travel there (car/ public transport)

Health & Safety

· Complete all mandatory training and ensure compliance and safe working practices for all staff, direct reports, volunteers and contractors

· This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

· Available to respond to out of normal hours emergency situations on a rota basis

· Work in accordance with all Health & Safety procedures, always using appropriate PPE and safe working practices, and report any accidents, incidents or hazards promptly.
