Alongside managing a caseload supporting adults, children, young people and families, you will act as a Senior Community Wellbeing Coordinator within the team, supporting colleagues with complex cases, sharing best practice and helping to maintain high standards of service delivery. Working closely with the Social Prescribing Team Lead, you will contribute to quality assurance, service improvement, partnership working and the continued development of Community Connect.

This is an excellent opportunity for someone who enjoys supporting others, solving complex problems and helping shape high-quality services while retaining direct work with residents.

We're looking for someone who: 

• Has significant experience supporting residents in health, social care, education, community or voluntary sector settings. 

• Has experience managing complex cases and working across multiple agencies. 

• Can support and guide colleagues through coaching, mentoring or sharing expertise. 

• Has a strong understanding of safeguarding, health inequalities and wider determinants of health. 

• Is proactive, solution-focused and committed to continuous improvement
