About the role

We are seeking a Floating Support Worker to join our Accommodation Service based at Kensington and Chelsea Town Hall. This service provides support to adults who are experiencing homelessness or are at risk of becoming homeless.

In this role, you will manage a caseload of clients, supporting them to sustain their tenancies and successfully settle into their new homes. You will work closely with individuals to help them access employment and education opportunities, and connect with appropriate services to address mental health needs, financial challenges, and substance or alcohol-related issues.

Working in this role, your key responsibilities will be to:

• To use a range of specialist assessment and personalised support planning tools to engage clients in the assessment and support planning process.
• To work with clients in producing person-centred support plans and ensuring that case reviews are carried out, support plans are implemented and adequate case records are maintained.
• To assess client risk and develop risk management strategies with the client and other relevant services.
• To work with a range of agencies to ensure all clients have access to relevant services including both statutory and non-statutory support.
• To provide specialist advice to clients on housing, health, welfare benefit claims and other issues as necessary.

About you

This role will suit people with: 

• An understanding of the issues faced by homeless or vulnerably housed people – you may have had personal experience of homelessness yourself. 
• Experience of helping vulnerable people to identify personal goals and supporting them through a process of change. 
• Good interpersonal and listening skills, to establish effective working relationships with clients. 
• An enthusiasm to learn and work with clients with a holistic, person-centred approach.

About the role

We are currently looking for a Custody and Probation HAWk worker to be based in a Prison Establishment and Probation Delivery Unit (PDU) to support The Probation Services objective in reducing reoffending.

In this role you will be assessing clients’ housing needs and take action to prevent homelessness by;

• Supporting clients to retain existing accommodation or to relinquish tenancies to avoid accruing rent arrears and debt.
• To support clients who will be homeless on release to access appropriate accommodation.
• Working alongside The Probation Service to achieve set interventions to prevent homelessness.
• Working as part of a larger team based across 10 prisons and multiple probation offices.
• The post requires the successful candidate to be in prison establishments 4 days a week in prison and 1 day a week at the PDU.

About you

We are looking for someone who has:

• Some experience of working with ex-offenders and people who are homeless/at risk of becoming homeless.
• The ability to work as part of a multi-disciplinary, multi-agency team within the prison and probation environment and to maintain effective relationships with external agencies.
• The ability to work flexibly in a changing and often challenging environment.
• The ability to work to agreed deadlines and targets.
• The ability to cope with clients with multiple needs, who may display challenging behavior.

You will join a dynamic team of managers, day shift and night shift workers, taking part in the renewal of the service, helping us to provide a psychologically-informed environment and a supportive and secure place where our residents can make meaningful life changes.

What you’ll be doing as a Project Worker

• Oversee a caseload of clients and provide ongoing 1-1 support in an approachable, caring and person-centred way.
• Provide practical and personal support to clients with complex needs around areas such as housing and health. 
• Have a good understanding of a person centred approach and how to provide the right support to clients to meet individual needs, identifying personal aims and ambitions and making plans to achieve them.  
• Work with the team, external statutory agencies and voluntary organisations to ensure everyone has access to available support.  
• Continue to develop your skills further around working with clients in a holistic way - we will provide training and support to help you gain a deeper understanding of housing and benefits law, and other specialist areas through training, supervision and line management support. 

Working hours

You will work full time on-site on a 7‑day rolling rota, including early shifts from 7:30am, late shifts until around 10pm, bank holidays, and two extended weekend shifts in every six. Some roles also offer a 20% pay premium for hours worked between 8pm–8am Monday to Friday, and for all weekend hours.

We have an exciting opportunity for a Service Manager to lead and support a professional team to support people experiencing homelessness. Leading a team of support workers, you will provide 24-hour staffed, supported accommodation to some of the most vulnerable people in the borough, using psychological and trauma-informed methods to support them in their recovery.

As Service Manager you will:

• Take overall responsibility for managing and coordinating the support for residents.
• Ensure the services is a safe and welcoming place to work and live.

You will also be responsible for managing the H&F Progression Coach, who is linked to all 7 projects across the H&F pathway holding a caseload, providing tailored support to individual residents for ETE purposes, also providing tenancy sustainment support to groups of clients in preparation for move on whilst maintaining relationships with volunteers across the pathway and ensuring they are supported.

Secondment / Fixed Term opportunity for 12 months (Maternity cover) 

The Grange Road team is looking for someone to fill the Service Manager role for 12 months. The service opened in 2022 and is the first High Complex Needs Supported Housing project within the City of London’s pathway. The team works with clients who have been rough sleeping within the Square Mile of the City of London, working with them to build skills in managing support needs and tenancy sustainment. Many of the clients in the service have support needs around substance use, alcohol use or mental health. The service focuses on implementing the recovery approach and improving access to support through in house nurse and psychologist clinics. 

The Service Manager is supported by a Deputy Manager and Senior Project Worker. 

As the service manager you will: 

• Support the team in creating a safe and supportive environment for clients, using PIE techniques and practices. 

• Implementing routes to practical support for clients, through on site clinics and external partnerships 

• Ensuring best practice is implemented and ongoing in the service 

• Maintaining and building relationships with stakeholders and the community 

• Leading and motivating the team in day-to-day work, personal development 

• Taking on line management responsibilities for multiple staff members 

• Liaising with commissioners, providing contract monitoring reports and maintaining effective joint working 

Working hours will be 9am – 5pm, Monday-Friday. 

About you 

We are looking for someone who can motivate and support the team to work with clients creatively, focusing on their strengths and overcoming barriers. You will be able to work flexibly balancing multiple priorities with strong interpersonal skills to be able to build positive relationships with clients, staff, and stakeholders. 

You will be recovery focused and have a clear idea of best practice and how to promote this within the team and service. You will have experience, or the skills needed, to report to commissioners and lead the service in contract delivery. We work with those affected by homelessness and those vulnerably housed in Tower Hamlets, offering an integrated service of housing advice & support, access to substance use, physical & mental health services, as well as, training & progression programmes. • Holding a caseload of people who are sleeping rough where their substance and/or alcohol use
• Ability to conduct street outreach at sleepsites and known hotspots within the Borough
• To actively promote and work in a way which encourages harm reduction
• To work collaboratively with partner agencies to ensure positive outcomes are achieved, safeguarding is shared and to increase the persons access to support services.
• Increase the total number of people accessing structured treatment & recovery through a bespoke pathway for rough sleepers as well as sustain treatment for these individuals, reduce harm, and support bespoke planned treatment exits or transitions into community-based services (continuity of care).
•  Reducing barriers of accessing Primary Healthcare services, such as GP registration, attending hospital appointments and specialist health services by advocating for and supporting clients.
• Attending and coordinating client related MDT’s and attendance at meetings relating to the rough sleeping community
• You will promote and deliver Naloxone training & distribution to those on your caseload 
• You have excellent communication skills and can create positive working relationships with a wide range of people, including external services.
• You will support the development of the service and ensure that you are delivering a solution-focussed and strength-based approach.
