Experienced Social Worker – NHS Pathfinder Partnership
 

GMRC is a registered charity working with adult women who are victims and survivors of sexual violence and child sexual abuse, providing independent, specialist support and promoting and representing their rights and needs.

The post-holder will be based at GMRC but work alongside TRC and MASH women’s services and Greater Manchester Pathfinder partnership, with service users who have experienced sexual trauma but who present with additional complex mental health needs.

Key responsibilities:

- They will work with service users in partner organisation settings and in the community, working collaboratively with individuals, carers and partners from local multi agency providers and services, to assess and identify social care needs that may be potential barriers to the client being able to address their sexual trauma.

- The post-holder will provide guidance, leadership and consultation on social care legislation and policy to the team and to other areas of mental health services, including joint working where appropriate.

- They will contribute to developing and maintaining high standards of decision making and professional practice in the field of social work.

- The post-holder will be responsible for a caseload of service users under the care of the Pathfinder Partnership, supporting them in their recovery journey.​

- They will provide specialist social work interventions to service users and their carers / families, and involve them in all planning, helping to develop social resilience, motivation and daily living skills.

Key Responsibilities / WHAT?
Service Delivery
• Act as the main point of contact for all financial wellbeing and grants-related enquiries.
• Conduct research to provide up-to-date information and practical guidance to help unpaid carers maximise

• Deliver benefits checks using Money Helper tools and support carers in navigating the welfare system.
• Provide tailored financial guidance, signposting to appropriate services, and assisting carers with benefits

Job Purpose:

Women in Prison’s Project Workers deliver high-quality, trauma-informed, independent advocacy for women in communities and in prisons, which focuses on early intervention, and holistic provision as part of a ‘whole system’ multi-agency response that looks to address the root causes of women’s offending. About The Organisation

In the 1980s, high unemployment and steep inflation was contributing to a shocking rise in youth homelessness across London. JOB PURPOSE

This role sits within The Passage Housing Needs Service (HNS) team. It will provide brief interventions to support individuals who have been placed into accommodation by the HNS team to settle into and sustain their tenancies. The post holder will work holistically to assist individuals to address issues that are impacting on their ability to sustain their accommodation on a time limited basis and will link them into services in their local areas for ongoing support in order for them to gain independence and integrate into their new communities.

MAIN TASKS

• To provide support to individuals who are moving into their new private rented sector accommodation where appropriate and needed. This can include setting up utilities, council tax among other things.
• To support individuals accommodated by the HNS team, in a brief solution focused manner, to find solutions to their housing, benefits, health and other issues when they arise, referring clients to support services, liaising with these, and following up in a timely manner.
• To establish and develop positive and constructive working relationships with a range of services and agencies across London boroughs that can provide support to our clients ensuring the best possible outcomes for them.
• To draw up psychologically informed action plans for clients ensuring they are person centered and taking into consideration the clients’ views.
• Support HNS Workers to complete exit questionnaires to gather feedback and promote co production with the individuals the team supports.
• To ensure that any health and safety and safeguarding concerns are reported appropriately using the correct systems.
• To assess any risk to clients, the environment and staff, record and communicate any risks identified using appropriate channels in the Passage.
• To ensure accurate and timely record keeping using the Inform database as well as client files and ensure all record keeping and information sharing is kept securely in line with Data protection.

GENERAL RESPONSIBILITIES

We’re an award-winning charity running local learning centres in the heart of the communities where the young people we support live. Our centres provide a high-impact education programme which includes practical learning support, pastoral care, and motivational and confidence-building activities for young people aged 7-18. We are looking someone who will thrive in a varied and innovative role as a fixed-term Project Worker in the Adult and Family Learning team. You will be based at our Maryhill (Glasgow) centre with frequent travel to our centres in Govan and Craigmillar (Edinburgh). This is a role focused on how we can offer additional impactful support in the local communities in which our Scotland centres are based. The Adult and Family Learning team will work in close collaboration with colleagues at the Universities of Edinburgh and Glasgow.

The pilot will focus on supporting the parents, carers and families of young people accessing the centres, with the potential for this to extend to wider adults in the community. As a Project Worker you will deliver the programme and support the Adult and Family Learning Manager to plan and develop activities tailored according to and with an understanding of the different communities in which it will be offered.

Working with colleagues based at the three centres, you will provide a range of activities which will support parents and carers to support their children in their learning, develop their own skills and knowledge around employment and education, and provide a range of family learning activities to engage the whole family together.

Centrepoint, the UK’s leading youth homelessness charity, is looking for two passionate and proactive Upstream Practitioner’s to join our Team based in Bournemouth.

About us

Centrepoint is the UK’s leading youth homelessness charity. We help vulnerable young people by giving them the practical and emotional support they need to find a job and live independently. We provide homeless young people with accommodation, mental health support and life skills in order to get them back into education, training and employment. We want to end youth homelessness by 2037. Together with our partners, we support over 16,000 young people each year.

The roles

We have two permanent Upstream Practitioner roles available, which are both school based and are part of our policy and prevention directorate. They will report to the Senior Upstream Practitioner and as a team, you will play a key role in our pre-16 prevention pilot, helping to identify and address the root causes of youth homelessness.

We have one full-time role and one term-time role available. Both roles are 37.5 hours per week and will be based in The Bourne Academy. The full-time role is working over 52 weeks and includes annual leave entitlement in line with Centrepoint’s policy. The term-time role is working over 41 weeks per year (39 weeks during term time plus 2 additional weeks). This role does not include annual leave. Instead, the annual leave is paid in addition to the salary.

The successful candidates will need to be available week commencing 23rd March Monday to Friday in Manchester, to undertake Level 4 mediation training with UK Mediation. All expenses will be paid for by Centrepoint.

Job Description

As Beacon Team Leader, you will be a member of the local management team within the victim service, managing a team of Beacon Support Workers as part of a multi-disciplinary team ensuring the effective delivery of end to end support for victims in line with contractual expectations. In this role, as a Night Support Mentor, you will work within a 24 hour service which provided person centred support to our residents, working to ensure the safety and wellbeing. Our residents have experienced multiple forms of exclusion, such as historical/ongoing abuse, involvement in the criminal justice system, mental health challenges, drug and/or alcohol dependency and enduring high levels of violence and coercion. You will build trusting professional relationships with residents, helping them to achieve their goals and aspirations which could be through tailored one to one sessions, group sessions, in and out of service activities, and general wellbeing check ins. Your role will focus on empowering our residents to take control over their lives and meet their personal goals

 Responsibilities include:

• Holding a caseload, conducting key work sessions which are personable to resident needs
• Carry out regular welfare checks, safeguarding checks, and general living checks
• Encourage residents to take part in activities and events both within the service and the community
• Keep support plans up to date on a regular basis
• Work closely with the rest of the team to provide overall excellent service delivery, this may mean also supporting with their caseloads, and other tasks as necessary

We are looking to appoint an Operations Officer to help build and scale our social enterprise. You will report to the Head of Operations. Operations Officers are a crucial part of our Services Team. You will be the main point of contact for our partner charities, receiving referrals for personalised budgets and processing these on a weekly basis. You’ll be responsible for managing incoming cases, liaising with support workers, providing advice and guidance about each case, light touch financial support, and releasing personalised budgets to clients across our contracts. You’ll work closely with our partners around the UK to ensure our personalised budgets reach as many people as possible and our contracts are a success. 

The ideal candidate is someone who is highly organised, possesses excellent attention to detail, confident in communicating to partners and at ease delivering presentations. We’re looking for people who are proactive, restless for change and want to be part of an innovative solution to ending homelessness for good. 

About Greater Change

Founded in 2018 by Alex McCallion and Jonathan Tan, Greater Change is an innovative social enterprise focused on helping people overcome the financial barriers on their pathway out of homelessness using personalised budgets. We partner with charities and support workers who refer people to us who would benefit from our financial support. The personalised budgets (supported cash transfers) we provide are typically for rent deposits, ID documents, training courses etc.

On average, Greater Change spends £1400 per individual and last year 85% of the people we supported sustained their move into stable housing, saving the public purse over £41,000 per person per annum.

Our goal is ultimately to use personalised budgets as a dignified and effective tool to end homelessness.

Main Responsibilities

1. Manage referrals end to end, including receiving, triaging, and processing cases to ensure timely release of funds to partner organisations.

2. Maintain accurate and up-to-date records on our CRM to track referrals, case progress, and outcomes.

3. Provide financial case planning support for each referral, using a strengths-based approach to support individuals to move away from homelessness.

4. Process weekly payments and maintain accurate financial records, ensuring compliance with internal controls.

5. Liaise closely with support workers in partner organisations to progress referrals and resolve issues.

6. Onboard, train, and support partner charities, including scoping calls, delivery of training sessions, workshops, and bespoke support and resources.

7. Ensure high-quality partnership management across all contracts, including the development of tailored resources as required.

8. Prepare operational and monitoring reports for partner organisations, lead partners, local authorities, and other stakeholders.

Essential Skills, Knowledge and Experience

• Strong organisational skills with the ability to manage a varied caseload, prioritise workload, and meet deadlines.

• A good understanding of the housing system, homelessness, benefits processes and services which support people who are precariously housed. 
• Strong IT skills in particular G-Suite, Canva and Microsoft Office.

Fostering Form F Assessing Social Worker

Key Duties for Form F Assessors include:

• To undertake and complete Form F assessments in the timescale set out in our policy and guidance.
• To visit personal referees as identified on application form for prospective foster carers.
• To ensure that all checks and references are undertaken and completed.
• To provide to the supervisor, regular written updates of the draft assessment.
• To liaise with our administrative team concerning the progress of administrative checks and completion of the Form F.
• To be available for supervision, consultation or liaison as required.
• To be available to present completed Form Fs to the virtual fostering panel and provide support to the applicants through this process.
• To be aware of our child protection procedures and immediately report any child protection matter or concerns to a TACT manager and any relevant placing authority and the area authority.
• To carry out the duties in line with our Foster Carer and Supervising Social Worker Standards and Practice Guidance.

Rates of Pay for Form F Assessors

• We will pay £2250 (inclusive of VAT) + up to £500 bonus payments (if criteria are met) upon submission of a successfully completed Form F Assessment.
• £37.50 per hour for assessments which are not completed, and any other piece of work.
