Gateway Mental Health is a 24 hour male only high and medium support accommodation service. Gateway is home for 28 customers from Lambeth and Southwark.
Support workers are vital members of the team as they provide support to customers helping them to develop the life-skills they require to move on to less supported accommodations or even independent living.
Support workers will work with customers to promote social inclusion and alongside other members of the team to promote independent living, encouraging customers to maximise their skills and choices, and achieve their goals
Support Workers will have a caseload of customers, for which they are responsible, and are expected to support all customers while on shift, with any immediate support needs and appointments.
Shifts are - 8-4pm 2pm-10pm Monday to Friday and double or 8am-3pm/3pm-10pm Saturday and Sunday. This includes bank holidays
What you'll do:
* Building supportive, trusting relationships with customers and creating a positive atmosphere.
* Conducting regular key work sessions that are innovative and engaging in order to achieve Support Plan goals.
* Create support plans for providing appropriate services based on on-going assessment and reflecting the services and resources available to enable customer to progress in their recovery
* Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'.
* Provide support and access to relevant services to enable customers to continue their recovery and empower customers to self-manage their medication regimes through individual re-enablement programmes and informed decision making;
* Undertake support work in partnership with external stakeholders to compliment their interventions;
* Developing productive relationships with partner organisations to improve service outcomes.
* Demonstrate strong commitment to the functioning of the staff team and to support colleagues in providing a high level of service and safety to customers;
* Involving customers in the design, development and delivery of the service.
* Adhering to lone working protocols and maintaining high levels of awareness in relation to the health and safety of yourself and colleagues.
* Ensuring Look Ahead Health and Safety policies and local protocols are adhered to at all times e.g. fire risk assessments, fire alarm checks etc.
* Being responsible for the handover of key information between shifts.
* Adhering to all other Look Ahead's policies and procedures.
* Engaging in learning and development activity to increase knowledge and skills.
* Undertaking any other duties consistent with the grade and nature of the post as assigned by the Team Leader/ Manager.

A unique opportunity to join the team at Basis Yorkshire as a Navigation and Support Worker for women who sex work and who are sexually exploited who are vulnerably housed.
You will provide appropriate emotional and practical 1-to-1 support to women involved in sex work and those who are sexually exploited, who often experience stigma and face barriers to accessing services. You will be enabling people to access support and navigate services, particularly hose directly related to housing directly and indirectly. What you will do:
- Work as part of a team providing suicide prevention support to a range of clients via multichannel communication platforms.
- Work on a 7-day shift system including evening and weekends
- Use professional judgement to assess the needs of the service users who present with risk to life and manage and report any matters related to safeguarding.

- Participate in clinical supervision and reflective practise.
- Provide training, mentoring and coaching to new recruits.
To be successful in this role you will have:
- a degree or professional qualification in Health or Social Care, Community Work or a related discipline
- previous experience of working in an advisory capacity in suicide prevention or mental health
- a proven record of working directly with vulnerable young people
- experience of providing advice and guidance via multiple communication channels
- the ability to empathise, support and build rapport with suicidal people and those who care about them, remaining non-judgemental and adhering to the remit of the service
Salary: NALC Scale SCP 24-28 (Starting salary of £29,510 per annum based on working 30 hours per week progressing incrementally to £32,626 per annum)

About the role
As one of the Lead Worker you will play a role in supporting people who are homeless or at risk of homelessness by providing assessment, coaching advice and advocacy, whilst linking individuals into services that provide support.
The key to success will be your ability to make collaborative, consistent and psychologically informed relationships with services, colleagues, and partner organisations. You will have the ability to empower and upskill service users to be proactive in their accommodation search. You will provide support with practical aspects of their route out of homeless, including access to housing, benefits, and other relevant needs. You will Lead Work a caseload and provide effective case management in line with the Crisis Case Management Framework.
Skills, knowledge, and experience vital to succeeding in this role:
- Passionate about and dedicated to ending homelessness.
- Comprehensive understanding and experience of accessing housing via private rented sector, local authorities, housing associations, supported providers and others.
- Good knowledge of issues affecting homeless people, including housing, welfare issues, trauma, substance use, involvement in criminal justice services, and social isolation
- Proven track record of successfully using coaching, IAG or psychologically informed approaches with people who have experience homelessness.
- Skills and experience in partnership working and networking.
- Flexible and adaptable to change.
- Strong team player, able to use own initiative and reflect on your own practice.
- Previous experience in the voluntary or statutory sector with a focus on working alongside marginalised people or groups.

The Role

Campbell Rd Young Peoples service provides support and housing for 37 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of around 7-10 young people. You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

LSFRC is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker plays a crucial role in fulfilling our customers' expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Workers are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers.
An essential part of this role involves developing good partnership working with a range of professionals and maintaining excellent record keeping in line with our compliance management. You will also be required to oversee support planning, Risk Management Plans, and Key-working.

Job Purpose
• The hospital to home service works collaboratively with hospitals to support people
when they are discharged from hospital to return home. As a Hospital to Home
Outreach Worker you will provide temporary practical and emotional support to service
users within their home and make sure they understand the care and treatment choices
available to them. The service is time limited, usually up to 4 or 6 weeks, to adjust and
settle back in their homes.
Key Tasks
• Assess the needs of service users either in hospital before discharge, or in their homes,
and understand what barriers and enablers there might be to moving home.
• Aid the discharge process and improve patient experience through tasks such as:
• Key cutting
• Co-ordinating with colleagues for the setup of the home environment
• Checking heating, electric and gas is functional prior to discharge
• Allowing access to the property for cleaning/repairs
• Provide (and/or arrange) the practical and emotional support needed to enable
service users to move home and/or remain safely at home.
• Be vigilant and make referrals/signpost for additional needs that our wider team, or
partners can support. E.g. for benefit checks, energy advice, carers support,
befriending.
• Help maximise service user independence by adopting an enabling approach.
• Develop a listening and caring relationship with the service user and their
families/carers.
• Provide domestic practical support within the home such as:
• Light cleaning,
• Washing (e.g. bedding),
• Changing bed sheets etc.
• Prepare and service drinks and simple meals with or for the service user customer
ensuring nutritional needs are met.
• Accompany service user outside the home e.g. hospital/GP appointments etc.
• Support customers with other tasks e.g. making telephone calls on their behalf,
reading and responding to correspondence, completing simple forms (training will be
provided) as directed by the customer etc.
• Ensure services provided by AUKEL in the community are safe and person centred.

circumstances in accordance with AUKEL’s policies and procedures.
• Ensure service user comment sheets are completed.
• Support the return of customer comment sheets and general feedback in respect of
your own service users.
Quality
• Ensure services provided in the community are delivered in line with the Care Quality
Commission (CQC) principles.
• Deliver all work in line with AUKEL quality mark standards.
• Commit to undertaking the Care Certificate, if not already held or in possession of NVQ
level 2.
• Complete essential training as required by AUKEL to provide safe services.
Liaison
• Work in collaboration with other agencies providing support within the customer's

As an Enhanced Support Worker you will support vulnerable people living in a variety of different tenures to manage their tenancies/ licences and move towards greater independence within the community, by providing flexible and individually tailored support.

What will you do?

- Assist the events manager with delivery of events, particularly during evenings and weekends.
- Assist with the cleaning of the site as and when needed.
- Assist with room setup and moving of furniture safely. This includes preparing spaces to host events and functions, packing down and cleaning afterwards. And to service ongoing regular lettings.
