The Creighton Centre is a local charity with a long history of working in Hammersmith & Fulham, caring for older, vulnerable and disabled people. We tackle problems caused by social isolation and disadvantage by developing and providing programmes that offer people practical solutions and emotional support.

The Learning Disability Outreach (LDO) project needs a Literacy Tutor to teach people with learning disabilities, helping them to improve their reading and writing skills, to enhance their lives and achieve their aspirations. The teaching will be mostly in a 1-to-1 capacity, or with small groups. You will need some experience of teaching literacy skills and a compassionate attitude.

An interest in gardening and educating people in a small community garden would be a bonus, although not essential.

Our Carer Liaison Workers work within both local NHS Trusts (BRI and Southmead Hospitals), where they make a real difference, supporting adult carers – those who care for others who can’t manage without their help, when the person they care for goes into hospital.

You would provide information, advice and support, enabling carers to navigate their way through the intricacies of the hospital journey, whilst liaising with health and social care professionals to ensure the carers voice is heard and their needs are supported, including during discharge planning.

You would also work with hospital staff to improve their knowledge and experience of supporting carers, through development work and training.

You’ll be joining the team at a pivotal time for our charity. Having recently secured our core statutory income for the next 3 to 5 years we’re now looking to use that as a launchpad to grow and develop new and existing services.

Disability Advice Service Lambeth (dasl) is Lambeth’s peer-led Disabled People’s Organisation.

About the Role

Service Delivery

Could you be a Caseworker for vulnerable young people? Could you give them the support and guidance they need to get back on their feet and live independently?

The young people we work with have become homeless for a number of reasons – isolation or mental ill health, abusive relationships, substance misuse, involvement in gangs/county lines - their issues are complex so you will need to be able to build strong, trusting relationships so that our customers feel safe and supported. 

We are opening two new supported housing schemes in Lewisham.  Both schemes are part of our well-established Young Persons and Complex Needs service in London and will provide temporary housing for young people aged 16-17 who are experiencing homelessness.

A bit about the role:

As a Caseworker, you will be working with a team of highly motivated, compassionate, and dedicated colleagues supporting a caseload of young customers with complex needs. You’ll need to meet each customer weekly to help them with their goals and discuss their progress. And, when someone’s ready to move on, we'll rely on you to help them access the housing register or look for suitable rented accommodation.

What we’re looking for:

You may have worked with vulnerable young people in a paid or voluntary capacity, maybe as a Youth Worker or in an education setting providing pastoral care. You’re a great relationship builder, who can create trust so that our young people feel comfortable sharing their issues with you, you listen but in a non-judgemental way and you are able to spot signs for concern and act on them.

You’ll also need to be able to work on your own, sometimes dealing with challenging situations and diffusing them to avoid escalation. Is this you?

Victim Support is proud to have been commissioned in Cumbria to provide support for victims of all crime, whether or not they have reported to the police.

The service will be a valuable point of contact for anyone affected by crime at any time to contact and receive help, support and information. It will be busy and fast paced receiving referrals and calls of varying nature from a variety of people, and working to provide an offer of support by a victim's preferred method and time of contact.

We have an opportunity available and are looking for dedicated, passionate and empathetic person to join and enhance our team as an Initial Support Agent (known internally as a Triage & Early Interventions Officer), this role is working 37.5 hours per week and is home-based.

About the role:

The role of Triage and Early Interventions Officer (TEIO) is a fast paced one which requires the post holder to answer incoming calls and make outgoing calls to victims of crime and those accessing the service in Cumbria.

As a TEIO, you will;

• answer incoming calls, speaking to those affected by crime & manage a caseload
• make outgoing calls, assess needs and risks and action plan with callers
• work on the case management system to allocate cases, provide administration on cases and assess/ensure data quality
• liaise with other agencies where required to find out more information, give information or make referrals or give signposting information
• work with the wider team to ensure an effective, trauma informed service is given to those affected by crime

You will be a confident, compassionate and empathic person who can work well under pressure. You will be speaking to people who may be upset, angry or in emotional difficulties and will need to be composed, empathic and be able to quickly assess situations.

You will be confident in using your IT skills to work on a case management system, access phone and online systems, email and Microsoft Office programmes.

Salary: Circa £26,700 per annum for full time hours, pro rata for part time hours
