You will play a pivotal ambassadorial role, spearheading Centre’s 404 service expansion into the London Brough of Brent and bringing our expertise, vision and values to families in need.
This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.
The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.
Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.
This role consists of consists of day time working patterns including early shifts from 8am and late shifts from 2pm. Shifts are 8 hours long and 5 days a week.

What you'll do:
- Building supportive, trusting relationships with young people and creating a positive atmosphere
- Working proactively with other members of the team to handle the service caseload and administrative responsibilities
- Conducting key work sessions as required, that are innovative and engaging in order to achieve Support Plan goals
- Co-producing ongoing assessment and management of risks in collaboration with young people with an attitude of 'positive risk taking'
- Providing support with daily living activities, including practical assistance to support young peoples to independent living e.g. budgeting, cooking, establishing a night time routine
- Supporting young people to maintain a healthy environment and where appropriate assisting them to clean their accommodation and maintain good personal hygiene
- Assisting in the recording and reporting of customer incidents
- Managing referrals, completing assessments and obtaining and maintaining all relevant paperwork
- Developing productive relationships with partner organisations to improve service outcomes
- Adhering to lone working protocols and maintaining high levels of awareness in relation to the health and safety of yourself and colleagues

“For My Future” is a bespoke 6-week course designed in 2019 specifically for people in the justice system. Focusing on both personal skills and practical ones (CVs, applications, interviews), the course is delivered either in person or online to small, single-sex cohorts over a 6-week period. To succeed in this role, you will have experience of supporting people in group scenarios and be confident presenting both in person and online. An opportunity has arisen for a lead Support worker to work within our Sanctuary crisis intervention and prevention service, based in Fore Street in Enfield. We act as a short-term A&E and Emergency services alternative, providing support to individuals in the Enfield Borough aged 18+.
This role is to provide leadership and support to the Sanctuary team on shift. The role will:
- Lead shifts during the week, providing support to team members via case management support, escalation, Health and Safety guidance
- Support the Sanctuary Manager to embed processes within the service, and ensure that these are carried out
- Work alongside the Manager and team to ensure that the service is operating to high quality standard.
- Provide support in times of emotional distress for clients. The support worker will provide a calm, supportive presence.
The role will support clients through co-producing support plans with practical and therapeutic support offerings, as well as contributing to the development of a warm welcoming environment and ‘safe space’ to be.
This position would suit someone with a minimum of 2 years experience of working with people with mild, moderate and severe mental health issues.

Within Mental Health and Homelessness services they will enable vulnerable people living in a variety of different tenures to manage their tenancies/ licences and move towards greater independence within the community by providing a flexible and individually tailored support package. About the Role
The primary role of a Family Support Worker is to provide effective, efficient, and supportive service to children and their mothers residing in refuge with Solace Women’s Aid. The staff member will lead on the development of a child and family-centred approach to work in the refuge. In so doing, they will:
- Develop and deliver a range of trauma-informed services to children and young people (0-17 years) and their mothers within the refuge and when they are resettled into the community.
- Ensure ongoing development of age-appropriate children’s activities.
- Ensure the completion of comprehensive assessments of family needs and risk of families, ensuring an integrated approach with children and adult services (internal and external)
- Promote ‘Integrated Working’ through internal and external mechanisms in partnership with colleagues and community partnerships.

Assessment Centres are fast paced services where people are referred in, usually from the street, and rapidly referred on to appropriate longer term accommodation. The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

Summary of Role
