You’ll play a pivotal role in driving forward the charity’s service improvement priorities to improve health and care services for people with Parkinson’s.
Using your quality improvement expertise to support health & care professionals with projects and programmes, equipping them with the latest tools, resources, evidence and data to develop and improve services for people with Parkinson’s.
You’ll support the development of a vibrant regional network, working in collaboration with the Professional Engagement team and the regional clinical leads. You’ll also work with health and care professionals and Parkinson’s UK colleagues to respond to any threats to Parkinson's services and effect positive change.
This position is part time, working 28 hours per week and will cover the South West of England region.

What you’ll do:
- Facilitate health and care service redesign and improvement projects in collaboration with clinicians, Parkinson’s UK staff and people living with Parkinson’s and drawing on the principles of co-production
- Build relationships to influence service improvement across a designated NHS Region and support the development of a vibrant regional network
- Monitor and support pump primed posts
- Respond to threats to services in collaboration with the area teams, people with Parkinson’s and health and care professionals
What you’ll bring:
- Experience and expertise in service redesign and effecting change within health and/or social care in England and knowledge of health and care structures and commissioning across England
- Experience in the use of quality improvement and project management tools
- Ability to work collaboratively with people with Parkinson's, their care partners and health and social care professionals. Experience of co-production is desirable
- Ability to analyse data and evidence to support service improvement and the development of business cases
This role is home based with the requirement to travel around your assigned geography and attendance of team meetings in our London office. There may also be the requirement for occasional overnight stay.

The post holder will coordinate and provide information-based sexual health outreach services, testing and brief HIV prevention interventions in commercial venues and at events across London. Interventions will predominantly target people who may be at higher risk of HIV and STI acquisition, particularly amongst higher prevalence communities such as Gay, Bisexual and Men who have Sex with Men (GBMSM), Black African and Trans communities. This role will involve working within sex on premises venues frequented by GBMSM and would therefore be best suited to someone from that community. This position is only open to applicants who identify as male (Equality act 2010 Schedule 9 Part 1) as the role will involve working in male only venues.
Programme
The post holder will be a member of the GMI Partnership team, based at METRO, working closely with their counterparts at other GMI partner agencies (Positive East and Spectra). The partnership is the main provider of the London HIV Prevention Programme, a sexual health and HIV testing outreach programme for GBMSM, across the capital. The post holder will be responsible for liaising directly with other contract partners and being a main point of contact for various outreach venue providers. The worker will form part of the METRO HIV Domain and will support the overall HIV prevention services provided by the charity.

About the role
Location: Community based in Somerset, with some homeworking elements.
Contract: Fixed term for 8 months, starting on 1st July 2024.

You will have the rewarding experience of providing information, guidance and support to people affected by dementia in identifying their needs, to help maintain independence, improving sense of well-being, and putting them in more control of their lives.
You will;
- Assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improving their sense of well-being, and putting them in more control of their lives.
- Build relationships with a range of local contacts, networking with health and care professionals, and providing dementia support.
- Develop networks and pathways with partner organisations who work in the field of dementia, ensuring seamless and consistent support and empowering individuals affected by dementia to make informed choices.
- Provide support face-to-face, by telephone and online and keeping detailed and confidential records.
About you
We are looking for a highly motivated individual, with an understanding of dementia, and the needs of those affected with these conditions. We are looking for someone who is approachable and knowledgeable to join our knowledgeable and passionate team. Our Dementia Adviser’s support people in their homes. We interact face to face, over the telephone and virtually, to give people the tools and knowledge to make informed decisions about their future. As a community-based worker, you will be required to attend a range of meetings, appointments and venues as well as working from home.
You will;
- Be empathetic and non-judgmental in your approach with a commitment to equal opportunity.
- It is desirable that you have knowledge and experience of Adult and Children Safeguarding, Information Governance policies and procedures and statutory laws such as the Mental Capacity Act
- Have good communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies.
- Have a sound knowledge of IT systems to record data, write reports and communicate by email.
- Understand client confidentiality and how this is applied when representing client needs.
- Preferably had some experience of working with a wide variety of relevant agencies.
- Have the ability and means able to travel independently around Somerset.
