Background to the role 

Age UK East London are recruiting a Community Engagement Officer to join our growing Community Outreach team. We support hundreds of people a year to maintain their health and wellbeing, combat social isolation and digital inclusion through the varied activities and by signposting to relative local support services. 

We support some of the most deprived communities in the UK, tackling poverty and health inequality while promoting independence and well-being.

Job description 

Job Purpose 

• To provide a high-quality outreach support service that enables older adults with low level support needs to live independently within the community. 

• To a promote network services across the Boroughs to maximise their take-up. 

• To work closely with statutory and voluntary sector agencies to ensure appropriate services are accessed. 

• To ensure that service users have maximum choice and control over the planning and management of the service they receive, including initiating individual support plans and connecting them with culturally appropriate services. 

Key Tasks 

• To signpost people to appropriate services as required through outreach sessions in the Hub, Satellite centres, GP practices and pharmacies. 

• To develop shared working practices across the service network. 

• To work closely with providers of similar services 

• To work to agreed individual, team, and organisational objectives. 

• To work with existing activity groups at the hub and satellite centres and support the setting up of activities where appropriate. 

• To ensure all contacts with service users are recorded appropriately and to maintain an adequate filing system. 

• To use word-processing software to produce own correspondence and reports in accordance with relevant policy on confidentiality and data protection. 

• To collect monitoring information to enable the service to be evaluated. 

• To provide support to services outside of normal working hours and occasionally attend sessions when required. 

Administration  

• To ensure all relevant administration is organised and efficiently completed. 

• To assist co-ordinating service user client surveys/questionnaires as appropriate, collate feedback and evaluate the service provided. 

• To assist with the collection of data necessary to provide specific information needed for KPI’s and external requirements and outcomes. 

• To ensure all data is collected through AUKEL organisational customer relationship management data base systems. 

Quality 

• To be familiar with and to implement Age UK East London’s policies and procedures. 

• Ensure that confidential data is managed within AGE UK’s policies and procedures.

 General 

• To meet regularly with line manager for support, supervision, and appraisal. 

• To attend team and staff meetings, (and other meetings) as required. 

• To undertake any other duties within the competence of the post holder as may be required from time to time for the continued smooth running of Age UK East London.

The Healthy Ageing Project will address specific challenges within the community, while actively promoting healthy ageing in practical and effective ways across our network.

We will focus on the strengths and expertise of our member groups to promote engagement with Ireland's unique music, dance, sport, language and culture.

We will work together with our members throughout this three-year journey. Support Worker

We're looking for a kind, compassionate and resilient Support Worker to join our Hope House service in Slough.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.

Job description

This is an exciting opportunity to join an established, dedicated and enthusiastic team supporting people with mental health conditions such as stress, anxiety and depression to gain and retain employment.

Hillside Clubhouse operates an established Employment Advice (EA) service integrated into NHS Talking Therapies (iCope).  The programme aims to support Islington residents who are assessed within iCope and who require employment advice.

The EAs in iCope initiative was designed as an integrated employment and mental health service to improve both mental health and employment outcomes. EAs are part of the iCope team and work alongside therapists to provide employment support to help iCope clients remain in work, return to work, or find employment.  This takes the form of personalised support for the client backed up by an action plan detailing the client’s employment goals and the actions that the client, the therapist, and the EA agree to take to meet these goals.  

Overall aim 

iCope (Talking Therapies) is an NHS Programme that provides evidence based psychological therapies to treat people with mild to moderate and moderate to severe depression and anxiety disorders.  

The new Employment Advisor (EA) will work within the Islington NHS Talking Therapies provision. They will work one-to-one with a caseload of between 25-35 clients referred by NHS Talking Therapies therapists to find employment, remain in existing work if they are struggling, improve their working environment, return to work following sickness or positively leave or retire from work. 

You will primarily fulfil these duties by providing information, advice and guidance to clients who choose to receive employment support. The support aims to empower clients to make better decisions about their working lives, as well as communicate better with colleagues and managers in existing roles. You will also signpost clients to other relevant support agencies, as appropriate. 

This role would suit someone with a background in employment support, psychology, counselling, human resources, recruitment, or a desire to transfer their skills into the mental health support sector.  

Method of working 

EMPLOYMENT ADVISOR RESPONSIBILITIES AND DUTIES 

1. Engage a mixed caseload of clients at any given time serving 100-125 per year, with common mental health problems, to establish trusting, collaborative relationships to support them to find new employment or support them in, or to leave, existing employment. Support will be provided face-to-face in Talking Therapies settings, via secure online meeting platforms or over the telephone. 

2. Along with their NHS clinician, assess clients’ support needs with a view to creating, implementing, and adjusting a personalised action plan to help improve clients’ mental health and achieve their stated employment goals.

3. Develop and deliver a range of practical services to support clients to find work, including career guidance, job searching skills, CV preparation, application form completion, interview skills, advice on local labour market opportunities and advice on education and training to further their career. 

4. Provide guidance to support clients to stay in existing work, including advice on mental health disclosure, negotiating reasonable adjustments to existing workplaces, creating wellness action plans, and improving productivity, support to return to work after sickness absence or to leave an existing role with dignity, and signpost clients to organisations that can represent them (EAs should only look to represent clients in exceptional circumstances). 

5. Facilitate access to other advice in areas such as financial benefits, debt management, food banks and community food outlets and social housing providers, as necessary. 

6. Work directly with partners like Jobcentre Plus, other employment providers, employers, trade unions and employment agencies to support clients to stay in employment and secure employment opportunities. 

7. Build a constant flow of referrals from the clinical team to ensure a dynamic and mixed caseload.

8. Meet referral and performance targets in line with the specific KPI / SLA requirements of the service. 

9. Regularly attend team and other meetings as reasonably expected. 

10. Maintain accurate and up to date records of activity and outcomes in line with service requirements, ensuring both the NHS and Hillside Clubhouse databases are kept up to date and paperwork compliant.

11. Receive regular supervision and training to meet individual, team and organisation’s needs. 

12. Contribute to the development of a service that is locally responsive and supports minority/disadvantaged communities.

13. Comply with and actively promote all Hillside policies and procedures including Equality and Diversity, safeguarding and data protection. 

14. Perform other tasks as required by your manager. 

The role holder must also: 

1. Pass a Disclosure and Barring Service (DBS) check at an Enhanced level. 

2. Possess excellent IT skills, including familiarity with Microsoft Office software, as well as experience updating and maintaining databases. 

3. Maintain accurate and up to date records of activity and outcomes in line with service performance requirements, ensuring the IT database is up to date and paperwork compliant.

4. Receive regular supervision and training to meet individual, team and organisation’s needs.

What you'll do:

* Technical condition surveys and reports on properties and their presentation to internal and external colleagues as required
* Designing, specifying and providing cost advice on any required maintenance, refurbishment, adaptation or development works
* Appointing and liaising with external contractors and consultants making sure work carried out is cost controlled and up to standard
* Pre, post and regular inspections and ensuring projects are delivered with quality and value for money
* Inspecting aspects of compliance making sure each project is aligned with company procedures and legislation

Healthier Lifestyles Project Officer x 2

We are looking to recruit two additional project officers mainly to help deliver our Live Love Later Life and Healthsmart services whilst providing organisational and administrative support to help these run smoothly.

An exercise qualification and experience of delivering health checks are highly desirable. However, if you do not hold these qualifications you will need to undergo a chair-based exercise qualification and blood pressure training. Applicants need to be willing to undergo this training. They also need to have the confidence and enthusiasm to deliver exercise to groups of older people whilst motivating participants that may have a variety of physical and cognitive challenges. These attributes will be assessed through a task during the interview process.

Night Support Workers enable people with particular needs to manage their tenancies and live independent lives of their choosing in the community. Night Support Workers are responsible for ensuring continuity of a support service at night and for keeping customers and the building safe.

What you'll do:

Building supportive, trusting relationships with customers and creating a positive atmosphere
Working proactively with other members of the team to handle the service caseload and administrative responsibilities
Conducting key work sessions as required, that are innovative and engaging in order to achieve Support Plan goals
Ensuring ongoing assessment and management of risks associated with customers within an attitude of 'positive risk taking'
Providing support with daily living activities, including practical assistance to support customers to independent living e.g. budgeting, cooking, establishing a night time routine
Supporting customers to maintain a healthy environment and where appropriate assisting them to clean their accommodation and maintain good personal hygiene
Assisting in the recording and reporting of customer incidents
Managing customer referrals, completing assessments and obtaining and maintaining all relevant paperwork
Developing productive relationships with partner organisations to improve service outcomes

The person

You will be part of the Young People’s Services Team who provide a range of services and activities for young wheelchair users and their families. You will have a friendly and professional approach to working with families and professionals and be highly organised to manage planning, delivery and relevant paperwork for all wheelchair skills services. Coproduction & Activities Coordinator

As lead for coproduction across our Care & Supported Housing Department, the post holder will have a firm understanding of  the breadth of strengths and skills of the people using our services, while also having an awareness of the operational requirements of the department. Maximising opportunities to work together, improving communication and fully embracing the experiences of all the people we support so that we continue to improve services, enhance support and offer solutions and services that are truly coproduced.

The role, will lead Hightown's coproduction group 'VoiceBox', overseeing all coproduction activities, seeking new coproduction opportunities, recording successes, outcomes and monitoring activities all in accordance with Hightown's coproduction strategy. Performance will be reported to Hightown's Board.  

Additionally the post holder will also  encourage, promote, and celebrate the wide range of activities carried out by our diverse services. This role will look to identify, highlight and promote the activities already being undertaken as well as seek new opportunities, funding, access and awareness of an increased range of activities for all the people we support to further enhance their lives and better their outcomes. They will encourage services to broaden their range of activities and oversee the introduction of an activities database.

• With experience of promoting and implementing coproduction and coproduced activities with proven success of building relationships and effective partnerships
• With experience of working with vulnerable young people and/or adults from diverse backgrounds and/or complex needs using a variety of methods to engage groups in different ways.With the skills to coordinate Expert by Experience activities including delegating responsibilities.Who is strategic and creative; they will also be pragmatic and able to put vision into clear practice through competent project management approaches.
