The Community Health and Wellbeing Worker will speak to households about health and wellbeing, alongside offering support to connect people to services and activities in their local community.
We are looking for the right people who can empower residents to take control of their lives. This will involve working with residents in their homes who may be experiencing a number of issues, so you will need outstanding listening and communication skills, empathy, problem solving and enthusiasm. This role offers an exciting opportunity to grow and develop a Dementia Service that was launched at Age UK Westminster in 2022. Offering a weekly community-based Maintenance Cognitive Stimulation Therapy group that provides a vital contribution to living well with dementia and a monthly Memory Cafe, we are looking for someone with the knowledge, skills, experience and passion to ensure that more residents can benefit from this Service.
Summary of role Age UK Westminster launched its Dementia Service in 2022 supporting older people (aged 50+) living in the City of Westminster who are experiencing memory and cognitive impairment and are experiencing confusion and dementia resulting in severe social isolation. The service also supports those who are caring for people with these issues.
In 2024 the ambition is to grow the reach of the successful monthly ‘Heart of Westminster' Cafe delivered in partnership with the Salvation Army’s Regent Hall and a weekly community-based MCST group and provide expertise to the Befriending team to manage some befriending matches. Demonstrating the value of these community-based activities and ensuring they are supported by a trained and supported volunteer team will contribute to securing continuation funding.
The role will involve close liaison with Westminster’s Memory Service and other community organisations that support older people living with memory loss and dementia.
Key responsibilities
● Co-ordinate the successful running of the monthly ‘Heart of Westminster’ Cafe in the Soho ward of the City of Westminster.
● Co-ordinate the successful running of up to two weekly MCST groups at Marylebone Church in Westminster including the co-ordination and triage of referrals for MCST groups
● Ensure the design and provision of activities and information that will enhance the lives of those attending and their carers. There is a significant resource bank of materials and activities
● Facilitate an environment where acceptance of dementia will bring about positive feelings for anyone affected by it
● Promote the service across Westminster to ensure that all those who would benefit are able to attend. This will include working with other professionals and ensuring that publicity materials are available
● Working with colleagues at Age UK Westminster and other professionals to identify and assess individuals who would benefit from being matched with a trained dementia volunteer. Home-visiting may be required.
● Deliver dementia awareness training to staff and volunteers

We have an exciting new opportunity for a Social Prescriber to join our team in Merton.
The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care GP practices in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. We expand the options available in Primary Care by creating a ‘managed’ way for referrals of patients with social, emotional and/or practical needs to a variety of local non-clinical services in the community. These include leisure, social activities, education, welfare, housing and employment advice and support.

Pershore Plus Volunteer Centre are working in partnership with Wychavon District Council to recruit a Communications and Community Builder to work in and around Pershore and the surrounding villages.
The contract is for an initial 18-month term.
The initial focus of the Community Builder role is to research and establish a deep understanding of the area, collaborating with local residents and existing organisations to uncover the key community assets and skills of local residents.
The role involves ‘Supporting and encouraging residents to be active in their local community and helping them to take the lead on topics that are important to them.’
Confident at all levels and ‘have an interest in community led projects and services.

You will have the skills to create graphics, infographics, videos, and posters and also be competent at event organisation.

We are based mainly on the site of Springfield University Hospital Mental Health NHS Trust. Springfield Law Centre is unique as a working model of a health justice partnership.
We are seeking a for a new Director & Head of Legal Practice to take forward this unique, niche Law Centre. There is an opportunity for expanding the work of the Law Centre and showcasing the worth of health justice partnerships.We are looking for an energetic and compassionate lawyer with a broad experience of either housing or community care casework and representation, who is capable of being a legal aid category supervisor in either area, who has the confidence to lead the team and to develop it so that we can continue to make a real difference within the community.
Qualifications and experience
- 5 years’ PQE qualified lawyer, with demonstrable experience of management at a senior level.
- Ability to meet the supervisor for the Legal Aid Agency contract in either housing or community care.
- A track record of implementing and managing change.
- A track record of leading and motivating others demonstrating strong people skills.
- A track record of innovation and problem solving.
- Appropriate skills and experience that demonstrate an ability and commitment to manage staff effectively. This should include support, supervision, and appraisal and performance management.
- Appropriate skills and experience to effectively handle and manage complaints.
- Experience of managing a Legal Aid Agency Contract and Legal Help matters, with billing targets.
- Ability to bill all types of legally-aided work and to report to the Legal Aid Agency.
- Ability to manage a varied caseload of sufficient, appropriate matters to meet the billing target in place.
- Ability to draft appropriate documentation in your specialist area of law.
- Experience of giving successful training or talks, for example giving presentations, or
- representing clients before Courts or Tribunals.

- Ability to take an active role in, leading the direction of the Law Centre, developing new strategies and raising profile.
- Ability to manage the Lexcel Quality Mark and ensure that systems and standards are maintained across the organisation so that audits are passed.
- Ability to understand the financial viability of the Law Centre.
- Ability to report effectively to funders and to monitor progress against grant funded projects.
- Commitment to equality and diversity as set out in the principles and policies of the Law Centre.
- Ability to be self-servicing: you will be expected to manage most of your own typing, filing and billing of Legal Help/Aid cases to comply with LAA requirements.
- Ability to prioritise your workload with minimum supervision.
- Excellent communication skills: you must be able to relate well to colleagues, clients and third parties, in person, in writing and on the telephone, despite the pressures of the role.
- You must be very well organised, with excellent ICT skills, in order to work well under pressure without compromising standards.
- Ability to work flexibly and outside regular office hours including attending and reporting to the trustees on the progress of the Law Centre.
Consideration will be given to requests for flexible or family friendly hours of work to include job share, part time working or specific proposals. The Management Committee consider the range and number of duties to be a full-time role and that any reduction from a full-time role will result in a division of duties to accommodate all aspects of the Law Centre’s needs and aspirations.

The purpose of this role is to provide administrative support for Global Black Thrive involvement in the Culture of Care Programme liaising with the Lived Experience Advisors, the Race Equity Lead, Neurodiverse Connections, the Royal College of Psychiatrists and NHS England.
This will include HR, Operations and Admin functions relating to the Culture of Care (CoC) Programme and will include managing systems to efficiently run the business and achieve strategic objectives agreed by Black Thrive Board of Directors.
The role will be the first point of contact for our CoC team, organising travel and accommodation, managing HR and office queries.
You may be involved in managing onboarding and offboarding, preparing employment contracts (Full-time and/or part-time, zero hours and/or fixed terms contracts), checking and following right-to-work in the UK, and administrating DBS checks.
You will be working with the HR and Admin Manager in Global Black Thrive and may be required to assist in wider team activities where required.
