About you

About this key role
As our Accommodation Adviser, you will complete an Initial Needs Assessment (INA) with each service user using a strength-based approached within agreed contractual timeframes, before assessing need and risk, alert to changes that could indicate change in risk or serious harm, including personal safety and safeguarding.
You will contribute to the design and delivery of an effective and solution-focused accommodation plan and advise and support service users to achieve successful outcomes utilising your specialist housing knowledge and a comprehensive understanding of accommodation and local service provision. You will also be expected to maintain and build new relationships with social and private housing providers and within the criminal justice sector, voluntary and statutory agencies. Monitoring progress and recording on digital systems is also a key duty.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.
We are looking for an Assistant Support Workers to join the Street Outreach Rough Sleepers Team

You will:
- Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough
- Work alongside senior colleagues to identify the fastest and most appropriate route away from the streets for the people that we are working with

- Maintain and support relationships with partner agencies
- Accurately record all work completed within given timeframes

Liverpool SHS is a mental health, 12 hour supported housing and community based service, working to a recovery model. We provide a range of support services for people with mental ill health to access in their local community, including outreach support, tenancy support, peer support and supported housing. We have been operating in the area for a number of years and offer a staffed service to our residents aged 18 and upwards.
We know that recovery can look and feel very different to each and every one of us. But we also know that if we provide the right support, at the right time, we can inspire recovery nationwide and that recovery is possible for everyone. That’s where you come in. Responsible for helping and inspiring the people who use our services to maintain their tenancy and independence in the community, we’ll rely on you to work with them to come up with a personal support plan that will see them achieve their goals and aspirations. No relevant experience is required as full training will be provided, although candidates with an understanding of mental health issues and/or lived experience would be welcomed. Whatever your background, you’ll need to be happy to work both independently and within a team and willing to be part of a weekly rota system and available for on call duties.

About the role:
We are looking to appoint a Family Support Worker to deliver high-quality family support services as part of our South West Care Team.
Reporting to the Family Support Manager and working in partnership with health, education, and social care professionals, you will take responsibility for providing needs-led emotional, social, and practical support to families where a child/young person has a life threatening or terminal illness.
Having worked in a stressful and emotional environment you will have a genuine interest in building supportive relationships and helping people; and having provided bereavement support to families, you understand processes of grief, loss, and change - and how best to help others deal with its impact.

• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation.
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation.
• Accurately record all work completed within given timeframes.
• Have a UK Driving Licence.
