The Junction is an out of hours “calming café” service which is open 365 days of the year to support people experiencing pre and/or mental health crisis. The aim of the project is to alleviate the need for people within the Swindon locality to access A&E and/or acute mental health services.

As a Wellbeing Practitioner, you will be providing a preventative, person centred approach to people accessing the service who may be experiencing a mental health crisis.

The service is open from 4PM-11PM and there are positions available for part time hours and full time hours and  working patterns will include some weekends.

About the Role

The Reconnect Worker works closely with the Deputy Prevention Services Manager and the housing team at Gravesham Council, you will be responsible for setting up and running our new Reconnect service.  Through offering a structured support package including family mediation, you will help young people to return, or stay in, the family home, and where not appropriate we will enable young people to rebuild support networks vital to successful independent living.

You will work collaboratively with all Depaul colleagues and services across the region, as well as with external agencies, to build and maintain a caseload of clients experiencing family difficulties and at risk of homelessness. You will work with clients facing a variety of situations, including young people who are experiencing a breakdown in their family relationships and are experiencing homelessness for the first time, as well as those who have left their homes and are temporarily staying with our volunteer Nightstop hosts.

Key responsibilities include:

 

· Work directly with young people aged 11-25, and their families, who are experiencing family difficulties and at risk of homelessness, providing guidance, personalised family support and one-to-one and joint party mediation if appropriate.

· Carry a caseload of 15 – 20 clients per FTE for whom you will be the named keyworker.

· Process referrals from relevant agencies to build and maintain a full caseload of clients.

· Work in partnership with young people and their families to complete risk assessments, needs assessments and change plans and outcome questionnaires to work with them towards measurable outcomes.

· Build strong working relationships with all Depaul colleagues and services across the region, as well as other relevant agencies and local authority teams, to promote the service, build effective referral routes and processes and provide holistic support to young people.

· Maintain clear, accurate and up-to-date records of all aspects of your work, in line with monitoring and evaluation processes and standard operational procedures.

· Work closely with statutory bodies and other agencies to support any relevant safeguarding procedures, advocating on behalf of young people and parents/carers when necessary.

About You

You will need to have the following skills and experience:

• Significant experience working with young people and families in crisis; reflective, creative and solution-focused in your approach and committed to working in an assets-based way.
• To hold or complete a Level 4 Interpersonal Mediation Practitioner’s Certificate (IMPC). Training will be provided if the candidate doesn’t have the qualification, but training will have to be completed before the end of the probation period.
• Experience working independently and managing own caseload; self-motivated and able to prioritize tasks and carry out efficient organisation and administration.
• Experience carrying out risk and needs assessments and support planning for clients.

• Ability to develop strong, collaborative and productive relationships with colleagues and key external agencies, promoting the value of our work and its impact on families.

• High-level understanding of professional boundaries and ability to maintain impartiality.

About this key role
Our successful candidate will assess referred clients, with reference to St Giles Trust assessment practices, and produce support and risk management plans based on these assessments, promoting inter-agency collaboration in the assessment and planning process.  You will be expected to deliver a holistic support service, working independently or with colleagues as the situation dictates, providing practical help such as social and housing support, education, training and employment options, benefits work and debt advice.

Purpose of the role:

Assisting in the resettlement of vulnerable refugees and Afghan evacuees through the provision of advice, guidance, and support.

Context of the role.

Upbeat Communities delivers several services to welcome refugees and support them to rebuild their lives. We are looking to recruit an Integration Worker who will offer support working on the refugee resettlement scheme.

Refugee resettlement takes place across Derbyshire and Lincolnshire. Over the past 7 years, Upbeat Communities has welcomed and played a key part in the resettlement of Syrian refugees in partnership with local authorities in both counties. This role has included supporting families to apply for universal credit and set up bank accounts, registering with GPs and local schools in addition to integrating into their local community. Since the summer of 2021, we have taken on responsibility for the resettlement of Afghan families in both regions.

This role requires working within a fast-paced and dynamic environment to manage a caseload of participants' families, with a lot of travel across the county of Lincolnshire. Areas covered could including Sleaford, Bourne, Spalding, Skegness, Boston.

Role responsibilities 

As the Initial Support Lead, you will: 

• work innovatively to shape the concept of an Initial Support Pathway, with a view to assisting agencies in identifying, safeguarding, and supporting potential adult victims. This may require the delivery of awareness raising sessions, whilst also working closely with First Responders to tactically advise in a manner that secures the best outcome for victims. 

• lead on the provision of direct support to potential victims of trafficking, ensuring they receive independent advocacy, helping them navigate services, benefits, and the advice they need.

The Initial Support Pathway will provide short-term support to adults who have experienced exploitation. The pathway initially focuses on the individual’s basic needs such as safety, food, clothing, accommodation, subsistence, and access to medical services. The project then considers each individual’s rights and entitlement to support as a potential victim of modern slavery. The key principle of the project is informed choice for the individual. Although the initial support is short and temporary, the long-term impacts for victims can be significant. 

Essential role requirements 

• Experience in health, social work, or working with vulnerable adults. 

• Knowledge of the impact of modern slavery and human trafficking on an individual. 

• A good understanding and awareness of the rights and entitlements (including immigration, housing, and benefits legislation) of migrants, trafficked persons, asylum seekers and refugees. 

• Able to work autonomously and take initiative working under pressure. 

• To provide trauma informed care.

• Experience of developing service delivery including partnership engagement. 

• Take a lead approach in multi-disciplinary settings ensuring best outcomes for individuals. 

• Proven experience of carrying out needs and/or risk assessments and the support planning process. 

• An empathetic and compassionate outlook, whilst being able to maintain professional boundaries with those you work with.

• Strong communication skills with the ability to communicate with a range of audiences. 

• Travel across Derby and Derbyshire is key to this role. The candidate will need to have a vehicle and valid driving licence. Mileage costs to be reimbursed.   

• Good people skills especially with those whom English is not the first language. 

• Ability to communicate with individuals via interpreters.

To provide safety planning, support, advice and assistance to children who have experienced domestic violence and abuse and are staying in the refuge.

The work will focus on preventing domestic abuse by providing support and facilitating safe space for children and young people and peer support networks in the refuge and supporting children and young people to keep safe in/maintain their refuge accommodation

The post-holder will work to empower and support children to make positive choices.

To establish resettlement services to support women and children to move on from refuge accommodation.

The post-holder work as part of the refuge team, with clients who live in the refuge after being referred via the Front Door Service.

To participate in the delivery of the on-call out of hours service on a rota basis.

Job Purpose

Money Minded is a new service designed to support adults (18 years and over) in Ealing, Hammersmith & Fulham, Hounslow. This role offers an exciting opportunity to collaborate closely with the West London NHS Mental Health Teams, operating within the tri-borough area.

The service provides support with welfare benefits, accessing debt support, housing and well-being. The role of the Mental Health Information and Advice Officer will be to provide advice, guidance and customer representation to tri-borough residents living with Mental Health difficulties. Advice issues may include housing, welfare benefits, council tax, accessing debt support, signposting etc.

The successful candidate will deliver support to enquirers and manage a caseload of individuals, as agreed with the Service Manager. In addition to advice casework, the Mental Health Information and Advice Officer will also be expected to assist onboarding referrals to the service, as and when needed.

The successful candidate will work closely with the West London NHS Mental Health teams and project volunteers, ensuring adherence to the Advice Quality Standard framework (AQS) and Mind's policies and procedures.

Due to the nature of the work, if successful, you will need to have an up-to-date DBS check completed, prior to undertaking unsupervised client work.

Place of Work

The Mental Health Information and Advice Officer will be required to work within the West London NHS Mental Health team offices.

Key Responsibilities

• Deliver support to enquirers and manage a caseload of individuals as agreed with the service manager

The Person
We are looking for confident individuals to join us in this critical work in preventing ongoing sexual harm. You will need to have knowledge and understanding of sexual violence and its impact, strong crisis management skills and skills in providing non-judgemental trauma informed support. The Role
This exciting new role will be required to provide a range of early interventions for children impacted by sexual harm or sexual exploitation.

The post holder will conduct assessments, provide recommendations, provide time limited non-criminal justice advocacy and psychosocial education to support the needs of children and young people who have either experienced sexual harm or may have caused sexual harm to others.

Tasks include:

- To undertake risk and needs assessments (including specialist AIM assessment where required for children who have caused sexual harm) and undertake associated report writing and recommendations.
- To provide time limited non- criminal justice advocacy, psychosocial education and support to children and young people and their families who have experienced sexual violence/have caused sexual harm to others.
- To develop and deliver interventions based on individual needs of clients.
- Collaborate and support a range of multiagency partners to ensure the most appropriate pathway of support for the individual survivor.
- Advocate on behalf of those impacted by sexual harm to ensure they can have access to a range of services based on need.
- Carry out institutional advocacy and stakeholder engagement and communication to enhance to wellbeing of all survivors of sexual violence.
- Ensure all risk and safeguarding processes and procedures are followed.
- To undertake evaluation and outcome meetings and data reporting to ensure continuous development of services.
- To support the group work facilitation of the delivery of psychosocial education in schools, colleges and community settings.
- Represent and promote WMRSASC in meetings and events.

We have roles focused in Herefordshire and Shropshire, all role holders will be expected to travel across West Mercia as and when there is a need.

The Supported Accommodation Kickstart Futures service. currently supports young people across the London Boroughs of Sutton and Croydon, who are living either in specified semi-independent properties or elsewhere in the community.

Location of the post:

 You will be community based, so you would be expected to support and travel where the young people live covering Croydon and Sutton.

Hours

Full time – 37 hours.

The Ideal candidate will need:

• Experience of working with looked after children / care leavers and understanding of the accommodation needs of care leavers.
• Knowledge and understanding of the specific needs of young people leaving the care system.
• Ability to listen to and communicate effectively with children and young people.
• Knowledge/understanding of the needs of Unaccompanied Asylum-Seeking children and young people.
• Understanding benefits processes, including Universal Credit and Housing Benefit.
• You will need to be a confident writer with analytical skills and experience in completing reports.

Some of the key responsibilities include but are not limited to:

• The delivery of 1:1 keywork support to care leavers that meets their individual needs. Frequency of key work will be dependent on the support package of each young person.

History of Service:

The Kickstart Futures service has been established to provide support to Unaccompanied Asylum Seekers and young people who have been trafficked, aged 18-25. We also support care leavers who are not from a forced migration background. The service launched in June 2020, initially supporting young people living in 1 semi-independent property in Sutton. We are currently providing support in 6 semi-independent properties in Sutton and Croydon, as well as within different community settings. We expect this service to continue to grow, and to provide support across an increasing number of properties.
