About the role:
We are looking to appoint a Family Support Worker to deliver a high-quality family support service as part of our Reading Team.
Reporting to the Family Support Manager and working in partnership with health, education and social care professionals, you will take responsibility for providing needs-led emotional, social and practical support to families where a child/young person has a life threatening or terminal illness.
Having worked in a stressful and emotional environment you have a genuine interest in building supportive relationships and helping people; and having provided bereavement support to families, you understand processes of grief, loss and change - and how best to help others deal with its impact.
Location:
This role covers Reading and surrounding areas, including West Berkshire, Slough & Maidenhead, Oxford, Windsor and Basingstoke. The role does require travel, however you have the freedom to plan your own diary around the demands of both the families you are working with, your team, and also any personal appointments you may need to attend.

You will contribute to the smooth and safe running of the services, along with supporting young people to develop living skills such as cooking, cleaning, budgeting, managing behaviour, seeking and securing employment and making wise decisions. When required you will record any incidents, accidents and safeguarding issues and share information with relevant colleagues. You will also undertake housing management and health and safety-related tasks as required.
This role has a flexible range of shifts; the hours can include evenings, weekends and bank holidays.

An opportunity has arisen for a lead Support worker to work within our Sanctuary crisis intervention and prevention service, based in Fore Street in Enfield. We act as a short-term A&E and Emergency services alternative, providing support to individuals in the Enfield Borough aged 18+.
This role is to provide leadership and support to the Sanctuary team on shift. The role will:
- Lead shifts during the week, providing support to team members via case management support, escalation, Health and Safety guidance

As a Support Worker you will have the opportunity of developing authentic relationships with residents, you will also have responsibility for making sure our residents are safe, providing support, liaison with other professionals and completion of support documents to record information.

West Berkshire Supported Housing & Floating Support is a support service, working to a recovery model and offers a support service to our clients aged 18 and upwards. Supported housing offers 13 self-contained flats for those looking to live a more independent life and our floating support offers support to those in their own home. About the role
Recovery Workers ensure our clients are at the heart of everything we do. They are responsible for inspiring the people who use our services to lead their own recovery, while maintaining their tenancy and independence in the community. Together they co-produce a personalised support plan designed to address their specific needs and achieve their goals and aspirations. Recovery Workers also develop your leadership skills by taking on lead areas and extra responsibilities. The support work at our service varies a lot so not every day or support session is the same. The Rough Sleeping Team in Oxford provide a rapid response service to support people at immediate risk of rough sleeping. We are currently looking for inspirational people to join the Somewhere Safe to Stay Service (SSTS) team as Assessment and Reconnection Workers where no day is the same. You will work as part of a busy, dedicated team while developing the necessary skills to:
- Provide direct support to people new to, or at risk of rough sleeping; working closely with individuals on assessment action plans which fully involves them in their recovery. To do this you will build positive working relationships with a range of services and individuals across Oxford.
- Work on the weekly shift rota including shifts from Mon – Fri 9am – 5pm, and some weekends and bank holidays.

Job Purpose
This role is within our crisis alternative service Safe Space - an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm (midday) - 11:30pm across all of our boroughs. The role will consist of two shift patterns to ensure the service is covered: 11:30am-6:30pm and 4:30-11:30pm. This will be on a rota basis across support workers.
Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.
The objectives of the service include:
- To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
- To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To treat service users with respect, dignity and personalised support
- To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
- To increase self-management skills of those accessing the service
- To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
- To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.
The Role
The role of the support workers will be to support the rest of the team in delivering interventions on a one-to-one and group basis. The role will also involve triaging and assessing all clients accessing the service, signposting and delivering collaborative self-management plans with clients to improve mental wellbeing.
