About this role…

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues.

We are looking for a Lead Worker  to join the Hillingdon Streat Outreach Team.

You will:

• Lead outreach shifts, providing a rapid response to referrals for people who are rough sleeping in the borough 
• Take a lead role within the team, working closely with support workers and assistant support workers to identify the quickest and most appropriate route off the street for people found rough sleeping, and devise plans to achieve this 
• Provide direct support to people who are found rough sleeping to enable them to move away from the streets and into accommodation 
• Work alongside partner agencies to ensure that clients are accessing appropriate support to access and sustain accommodation. 
• Represent the team at multi-agency meetings 
• Accurately record all work completed within given timeframes 

To succeed as the Lead Worker, you will have:

• A strong commitment to providing a high standard of service 
• Experience working with people who have rough slept or vulnerable adults and supporting them to access services 
• Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently. 
• Good communication skills and confidence in public speaking when representing Thames Reach externally 
• Experience in forming strong working relationships with partner agencies, being able to challenge them when required 
• Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone 
• A clean driving license (desirable)

The services are short stay, and staff provide wrap around support and liaison with several external partners, including physical health, mental health, substance use and housing. Staffed 24 hours to ensure this quick turnaround, they are a key service to ensure people are off the streets and in stable accommodation.

We are looking for a Chief Executive who can provide leadership, aligned with our valies and manage the organisation, working with the Trustee Board to provide vision and communicate a clear and inspiring strategic direction, implementing change as required.

The main purpose of this role is:

• To lead all staff and volunteers engaged by Motiv8 to successfully achieve objectives in line with Motiv8 valies in an inclusive way
• To develop the policy and strategy of Motiv8 in collaboration with the Board of Trustees
• To oversee and lead implementation of policies and strategy to deliver Motiv8 strategic aims now in the coming years
• To ensure that Motiv8 maintains and develops policies that reinforce inclusion and diversity in the workplace, that addrss all kinds of harrassment and protect minority and youth groups with whom Motiv8 works
• To represent Motiv8 on behalf of the Board of Trustees, to develop relations with our staff members, volunteers, partnership and stakeholder groups
• To further develop Motiv8 in line with the charity's objectives, to implement necessary change and ensure that Motiv8 exhibits ongoing relevance.

We are seeking to recruit a highly skilled Key Worker for our Key Worker Service in East Berkshire. The role will be diverse and service user centred and would suit someone with significant experience in supporting young people and young adults with Autism and Learning Disabilities and their families.

The day to day work will involve a flexible approach to coordination and direct support for the young person and their family, as well as providing responsive communication at times of increased need. This is an exciting role which allows for the Key Worker to support families to navigate the system whilst advocating for them and ensuring that their voice is heard.

As the service is new there will be opportunities for collaboration and co-production and our vision is to provide young person and family centered support, whilst influencing system change to achieve better outcomes for more children,  young people and adults under the age of 25.

The service will be provided across the entire East Berkshire footprint covering: Slough, Royal Borough of Windsor and Maidenhead, Bracknell Forest.Although the team will be “virtual”, there will be frequent travel within and across the East Berkshire area, therefore being a car driver with access to a car is essential.

The Key Working Function has been developed as a response to the NHS England & NHS Improvement Long Term Plan (LTP) commitment that by 2023/24, children and young people with a learning disability, autism, or both, with the most complex needs will have a designated Key Worker, applying the recommendations made by Dame Christine Lenehan in ‘These are our Children'.

Aquarius is a charity that helps people and communities overcome the harm caused by alcohol, drugs and gambling. Right now, we’re looking for a self-confident and energetic individual with a good understanding of gambling, substance misuse and health related issues, to provide a vital support service for individuals with gambling related needs.

Working flexibly across the community and within multi-agency partnership settings on an outreach basis, we’ll rely on you to engage people in a variety of interventions, using a person-centred approach. A core part of the role will involve addressing holistic support needs that could include employment related issues, family work interventions, issues of exploitation, physical and emotional health, domestic abuse and support to engage in rehabilitation. Whatever it takes, in fact, to empower them to reduce risk, increase resilience and achieve positive outcomes.

To succeed, you’ll need significant experience of engaging effectively with clients in an addictions or social care setting. You’ll also need knowledge of primary, secondary and other outreach settings and a good understanding of the functional model. Used to liaising with voluntary and statutory agencies and health professionals, you have a flexible approach and lots of initiative. You also have good IT skills (particularly Microsoft Office) and are willing to travel throughout the area in order to get the job done.

You will hold a caseload of complex cases requiring a case co-ordination approach. Requiring you to complete all recording and case management administration for your caseload accurately and promptly,

• Providing screening, co-produced assessment, psychosocial interventions, integrated and outcome focussed care planning, after-care support, and onward transition to other services as required
• You’ll also need a flexible approach, excellent record keeping and report writing skills and a willingness to work flexibly across the community and within our multi-agency partnership settings, on an outreach basis.

We are looking for a Lead Worker to join the Lambeth Temporary Accommodation Support Service. Our team are commissioned to work with, and support individuals placed in temporary accommodation by the London Borough of Lambeth.

You will:

• Assess the housing and other needs of people living in Temporary Accommodation in Lambeth
• Complete referrals for immediate and long-term housing options, and refer people to organisations that can help with other support needs

A fantastic opportunity has arisen for a Regional Casework Coordinator to join the Northwest Region at SSAFA, the Armed Forces charity.

You will be part of the regional office team who are the first point of contact to all beneficiaries into the Regional Office. You will be assisting with sometimes complex problems from individuals who may be distressed, identifying the presenting and potential underlining needs to determine the best way to support the beneficiary, either by beginning a casework referral, referring to another service within SSAFA or making an external referral to organisations better able to offer support. You will also have ownership of cases from beginning to end, coordinating the casework process in a timely manner and ensure that the beneficiary journey is at the centre of the Service. At all times you will adhere to the safeguarding, confidentiality, and data-protection policies, including escalation to emergency services for clients where there are immediate areas of concern.

To help you establish yourself in this new post you will receive excellent training and induction to SSAFA and will influence the processes and procedures within the office, working closely with the Regional Casework Manager and Regional Operations Manager.

The post is home-based, but you may have to travel occasionally around the Northwest Region of England.

About the team

In this new role you will be working closely with volunteers from the six SSAFA branches in the Northwest, supporting them to administer casework for SSAFA beneficiaries. The successful candidate will work as part of a larger support team which will include a Casework Manager and a Regional Operational Manager.

About you

To carry out this role successfully you will have a track record of providing welfare advice in key areas such as, housing benefits, debt, disability, mobility or immigration issues. You will have knowledge of safeguarding and GDPR, along with experience in dealing with sensitive information and people in vulnerable situations. You will be able to provide excellent customer service by telephone, e-mail and face to face. You will have used Microsoft Office 365 to a high standard. It is important that you have experience of planning and managing your own workload, with minimal supervision.

It would be advantageous if you have an understanding of the way of life for today’s Armed Forces, veterans and their families, along with supporting volunteers. An understanding of the voluntary sector and preferably the military charity sector landscape across the North West.
