Job Purpose: The Adult Carers Service supports carers aged 19+. The service provides practical, emotional and planning support so that carers are supported to maintain their caring roles, increase their resilience, and support them plan for the
future to avoid crisis. The Carers Support Coordinator will work in close collaboration with the Services Manager and SMT to support the implementation and development of the Bromley Well Adult Carers Service. The post holder will coordinate the provision of emotional and practical support to Adult Carers in the London Borough of Bromley, aligned and working closely with the integrated care networks and other Bromley Well Services, both within Age UK Bromley & Greenwich and Bromley Well partner organisations.

We have an opportunity to work for our Adult Carers Team as an Adult Carers Support Advisor. You will provide emotional and practical support to Adult Carers in the London Borough of Bromley, aligned and working closely with the integrated care networks.
- To offer emotional and practical support to build resilience for carers on a one-to one basis.
- To provide support face to face, over the phone and in group settings.
- To develop and run local support groups.
- To actively target those carers facing multiple issues, most likely to fall into crisis
- To support carers to access and where appropriate complete Carer’s Assessments
- To build and maintain a network of contacts with local service providers
- To organise and participate in training events for carers and professionals.
- To organise and participate in events for carers and or to raise awareness of carers needs, such as but not limited to Carers Week and Carers Rights Day
- To run an active caseload within the KPI requirements of the Bromley Well contract.
- To input cases on to the Charity log system accurately
- To ensure that services are accessible to carers.
- To participate in the provision of news and the production of the Carers newsletter
- To collaborate with other Bromley Well partners, including Mencap/MIND etc. to ensure effective appropriate referral pathways for quality service provision.
- To abide by and take part in the development and review of policy and procedure for the Carers pathway.
- At all times to maintain the professional integrity and reputation of the Charity and represent their main interests in any dealings with other bodies, groups and individuals.
- Attend staff meetings, supervision and organisational events as required.
- Collect case studies to help demonstrate AGE UK Bromley and Greenwich’s Adult Carers Support service’s impact.
- To undertake any other duties commensurate with the purpose and remit of the post.
- Undertake out of hours and weekend work as the role requires for which TOIL will be granted

Healthier Lifestyles Project Officer x 2
Responsible to: Healthier Lifestyles Service Manager
Responsible for: Volunteers, as required
Salary: £27,330.60 per annum FTE (Pro Rata if part time)
Hours: Full time and Part time role available Monday to Friday.
(A full working week is 35 hours)
Contract: 1-year fixed term contract with extension subject to funding

Location: Home, office (Thornton Heath) with regular travel in Croydon
We are looking to recruit two additional project officers mainly to help deliver our Live Love Later Life and Healthsmart services whilst providing organisational and administrative support to help these run smoothly.
An exercise qualification and experience of delivering health checks are highly desirable. However, if you do not hold these qualifications you will need to undergo a chair-based exercise qualification and blood pressure training. Applicants need to be willing to undergo this training. They also need to have the confidence and enthusiasm to deliver exercise to groups of older people whilst motivating participants that may have a variety of physical and cognitive challenges. These attributes will be assessed through a task during the interview process.
