Working with the South West Peninsula ODN, The Hepatitis C Trust are now expanding this commitment through the introduction of the HCC (Hepatocellular Carcinoma) surveillance and early liver disease detection programme.

About the Service
The Youth Mental Health Navigator supports young people who have been referred to the CAMHS front door/single point of access. These young people may have accessed medical, clinical or emergency services prior to being referred to Mind BLMK. The service is suitable for people who have mental health / wellbeing needs but are not felt to be at immediate risk of suicide.
About the Role
This role will provide individualised, one to one support to young people, aged 14-17 who are currently experiencing difficulty with their mental health and wellbeing.
This post holder will be working closely with clinical colleagues in Child and Adolescent Mental Health Services (CAMHS) team offering hybrid in person and online support. The post holder will also be responsible for the development of pathways which enables young people to access services offered by the Voluntary and Community sector.
The post holder will ensure they have a good understanding of the services on offer by the Voluntary and Community sector, and other agencies, and will work with individuals to help them access services of their choice.
The Youth Mental Health Navigator will offer mental health and wellbeing support in a safe and enabling service, supporting young people to explore their challenges and identify positive mental health coping strategies, resilience skills, distress tolerance and a solution focused approach to recovery that will help to future proof against potential repeated incidences of poor mental health.

About the role
The Employment Advisor’s role is to deliver the core employment support provision in NHS TT to provide employment advice, skills-based interventions, information and practical support to help people who are looking to return to work or retain their current employment.

Felstead Street is a CQC registered care home, which has a heavy focus on supporting customers with enduring mental health care and elderly needs. We provide 24hr high support to 24 customers both female and male. As a Support Worker at Felstead Street you will play an integral role in supporting our customers to making their lives interesting and enabling to manage their social, physical and emotional well being. You will support our customers with a range of activities and health support throughout the day and to live in a safe living environment. You will be part of a team which is very supportive of one another.
The successful candidate must be able to work some weekends and bank holidays as allocated. This role involves working 5 days a week. Each shift is 8 hours, 8am-4pm or 2.30pm-10.30pm, as allocated on the rota. You must be able to work 9am-5pm for 2 weeks every several weeks as required by the rota.

Health Inequalities Project Manager

Salary: £23,450 per annum (£39,084 FTE)
St Christopher’s Hospice has a commitment to tackling health inequalities and inequities. This important role will help us do so. In this role you’ll work across the organisation and in community settings to progress and monitor our work to maximise health for all, and optimise end of life experiences.
In this role you’ll support colleagues to operationalise strategy and initiatives or lead them yourself. You’ll be proactive and responsive to ensure that at all times we are responding well to the needs and wants of the varied community and colleagues we work with. You’ll also respond to and work with the organisational Inequalities Working Group established to oversee this work.
To succeed in this role:
- You’ll be interested in and knowledgeable about health inequalities and of working with people to address them.
- You’ll be an excellent project manager, capable of planning delivery and reporting on progress of projects, or developing new processes.
- You’ll be confident with data and able to present it, and you’ll be committed to finding new voices, forms of insight and information about inequalities.
- You’ll be friendly, warm and approachable, and able to work with a variety of people.
- You’ll be confident, well organised and experienced in managing a variety of people and projects.
- You’ll be a clear, diplomatic and persuasive communicator.

We have an exciting 18-month fixed term opportunity to join British Heart Foundation’s (BHF) Health Partnerships & Community Resuscitation Team to deliver partnerships which make a lifesaving, life improving impact.
Through our work we want to reach more people, in new ways, with greater impact, involving more people in their heart and circulatory health and care, and in the work of BHF. This will improve health outcomes and help ensure that BHF strengthens its position as the nation’s heart charity.
About the role
In this role you’ll contribute to the operational delivery of innovative health partnerships for BHF. These partnerships help us to reach, on a mass scale, people at risk or living with heart and circulatory diseases; working with large corporate organisations, other charities, and community groups to provide information, support and skills in innovative ways to improve health awareness, support self-management and reduce health inequalities.
As part of the Health Partnerships team, you’ll support several health partners, delivering agreed activity and maximising the opportunity to reach their workforce and customers with our free health initiatives.
You’ll work collaboratively with colleagues across BHF particularly in our Medical and Marketing, Fundraising and Engagement teams, as well as external partners to meet agreed aims and objectives. You’ll also work closely with other delivery teams (e.g. Clinical Support, Health Content, Community Resuscitation and Customer Service Teams) to create an integrated and seamless customer experience for your partners.
Obtaining a comprehensive knowledge of our health offerings during your induction, you’ll develop a warm and productive relationship with your key accounts and ensure the delivery of activity is fulfilled effectively and efficiently and that your partners needs and opportunities for growth are appropriately identified so that they receive the best possible experience from the BHF.
Working arrangements
Please note this is an 18 month fixed term contract.
This is a blended role, where your work will be dual located between your home and our London office.
At BHF we believe in the power of being together, so our colleagues on blended contracts can expect to spend some time in their office, at least one day each week, on average. The use of our office spaces is driven in part by your role and the activities you need to do. This may vary from time to time, so you will need to work in a flexible way to unlock your best work for our cause.

About you
With knowledge and an understanding of BHF and organisational objectives, you’ll have previous experience delivering projects in large, complex organisations, as well as the knack of developing relationships with internal and external key stakeholders quickly, and with ease.
You’ll have previous experience supporting the delivery of high impact partnerships in either a charity or health-related environment and will have experience managing own workload and key areas of responsibility.
To be successful in this role you’ll have the following skills and experience:
• Good relationship management skills, showing empathy and resilience in handling challenging situations.
• Excellent planning and prioritisation skills, with the flexibility to deal with reactive demands
• Ability to think creatively and work collaboratively to approach partnerships in exciting new ways
• Passionate about improving outcomes for people at risk or living with heart and circulatory diseases
• Willingness to learn new skills and build knowledge of different areas of the organisation
• Adept in Microsoft Office and customer relationship management systems
