Having identified a gap in the identification of young carers and adult carers looking after someone with substance misuse issues, over the past year we have successfully piloted a project to identify and provide long-term support to these carers. We are now entering the second year of the project, and are looking for a new Family Support Worker to bring their expertise, enthuasism and ideas to the next stage of the project. 

The Family Support Worker will work across our Young Carers and Adult Carers Service to provide support to families affected by substance misuse, through whole-family assessments and reviews. The successful candidate will ensure that adult and young carers, and their families, have access to a range of emotional and practical support appropriate to their needs.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. We are seeking candidates with a professional qualification in health & social care, youth or community work, and/or direct experience of delivering family focused interventions.

Carers’ Hub plays a vital role in Lambeth, supporting the many unpaid carers across the borough. Our work stems from the fact that carers often go about their roles despite the toll it might take on their own wellbeing, often having to make sacrifices that hamper their own ability to lead a normal life. 2. influencing local policy through community engagement activities,

Support Worker

We're looking for a kind, compassionate and resilient Support Worker to join our Hope House service in Slough.

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.

The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Aside from support workers delivering support there is a strong element of housing management associated wit this role, for example managing rents, raising/monitoring repairs and health and safety.

132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.

The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.

The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.

Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.

Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.

Overall Purpose

Duties & Responsibilities

• To be a Support Worker for newly arrived young people who are in the care of Baca in all areas of the young person’s life, supporting them from first arrival through to their move to semi-independence.
• To welcome new arrivals into the accommodation and communicate basic information about the house, making them feel safe and able to rest; with compassion without discrimination of age, race, colour or behavioural issues.

• To enable young people to live with positive values and good boundaries.
• To be a proactive role model for the young people, enabling them to learn day to day living skills and helping them integrate positively with the local community.
• Training young people in living skills such as washing, cleaning, cooking and safety, food shopping and budgeting; proactively and intentionally in line with the Baca independent living preparation plan.
• Helping young people settle into a routine which will include education and social activities.
• Work in close partnership with social workers and other professionals and volunteers to provide a holistic development package for each young person enabling them to become independent.
• Maintaining very good communication with relevant members of the Rebuild Team and relevant social workers through regular appropriate reporting.
• To work collaboratively with the rest of the Rebuild Team to ensure all young people are safeguarded and supported to rebuild their lives.
• Complete and update all admin work as part of the role such as month end reports and support plans on time and in a consistent manner.

Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. As a support worker you will play an integral role in developing effective support and risk management plans. As the Deputy Team Leader (Court Services), working alongside Team Leaders you will be responsible for the delivery of the Witness Service within your court/s. 

You will share responsibility for the effective coordination and development of volunteers who support witnesses in their journey through the criminal justice system.

The Role

Reporting to the Service Delivery Coach in the East of England, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.

Key responsibilities will include:

· Supporting new stroke survivors and their carers to adjust to life after stroke following hospital discharge into the community.
