Job Title: Part-time Co-production Coordinator - Adults
Salary: £30,000 FTE
Hours: 18 hours per week. To be worked 3 days a week (6 hours per day)
Contract: 1-year, fixed term
Location: Mainly home based with regular meetings in the London Borough Hammersmith & Fulham
Parent/Carer Co-production Coordinator to empower and support parents of young adults with SEND.

Parentsactive CIC is the parent/carer forum for Hammersmith and Fulham and works to empower parent/carers of children and young people with special educational needs and disabilities (SEND) from birth through to adults.
We are delighted to be recruiting a Co-production Coordinator to work alongside our parents and carers of young disabled adults to ensure they have meaningful opportunities to inform and influence strategic and operational developments across Hammersmith and Fulham that will lead to a better future for them and their young person.
The Co-production Coordinator will support the coordination of a range of activities designed to increase coproduction across the borough’s health, education and social care systems.
The successful candidate will be passionate about coproduction and ensuring the voice of people with lived experience is heard. They will have strong skills in working collaboratively with a wide range of people including families, statutory and third sector partners. The person will be a good listener, compassionate and understanding of the issues related to supporting a disabled adult to live their best life and have the confidence to challenge when necessary.

About the role
A Health Care Assistant works within the Inpatient Unit alongside other Health Care Professionals, providing an excellent standard of care to patients with palliative and end of life care needs whilst also offering vital emotional support to their families.
Our Health Care Assistants provide a patient-centred and holistic approach to the care they deliver, this may include assisting to meet hygiene needs, monitoring symptoms, basic wound care, developing clinical skills whilst working closely with the registered nurses.
Our staff to patient ratio allows Health Care Assistants to spend time with patients and their families to ensure great care is given and we are able to make a difference.
What you’ll be doing:
· Working as part of a wider MDT to provide individualized, holistic care to inpatients in Crimple House.
· Providing excellent standards of personal care to patients.
· Helping patients and their loved ones with emotional and practical support.
What we’re looking for:
Caring, compassionate and enthusiastic people.
Experience isn’t as essential as being motivated and passionate about developing your specialist palliative care knowledge and skills.
Great communication skills.
Someone who enjoys working within a team.

Transforming Lives for Good (TLG) are a national Christian charity working in partnership with local churches to bring struggling children hope and a future. Building on the huge success of TLG Early Intervention Coaching, TLG Therapeutic Support will be a new way for TLG, with partner churches, to provide support in school for the increasing number of pupils struggling to cope due to their emotional, relational, and mental health experiences. This programme addresses the underlying issues children are facing, whilst also providing the option of direct support to their parents/carers and the school staff team.
In this role we are looking for individuals with an accredited coaching qualification that thrive working with children and young people in a school context and are passionate about supporting pupils and their families struggling with emotional, relational, and mental health challenges. The ideal candidate will have an aptitude for creativity whether this is music and drama, sport and outdoor recreation or other fields of kinaesthetic development. The TLG Therapeutic Coach will be present each week in the school, providing specialist trauma aware care for these children, whilst supporting staff and parents/carers too, so we're looking for an individual who can build and foster strong relationships.
