About the Role
- Located at a 24-hour supported housing service in Clapham, we provide high quality self-contained accommodation for 15 women who have experienced homelessness and multiple disadvantage.
- The aim of the service is to provide a therapeutic environment in which women feel safe and can flourish, enabling them to move on into independent, permanent accommodation. The accommodation also includes overnight safe spaces.
- In this role, you will provide support to all the women, including running co-designed activities and accompanying women to appointments. You will work closely with Specialist Support Workers who manage individual resident support and safety plans.
- You will model person-led, psychological, gender, and culturally informed support, will be skilled at building trust and you will be a great team player.
- The post holder will undertake 8-hour shifts on a 7-day rota, covering the hours 8am to 8pm. Please be aware that the building has several floors and no lifts.

Waking Night Support Workers are responsible for ensuring continuity of this support service at night and for keeping service-users and the building safe. More specifically, you will:
Understand the concept of recovery and apply those principles to develop an environment and service that enhances recovery.
Put the customer at the centre of their support and promote optimism and hope for their increased independence
Provide intensive support (both practical and emotional) to customers as required

Lambeth Step Forward Recovery Cluster is an accommodation based service, set across Lambeth including hub sites situated in Brixton and West Norwood. The service provides care and support to over 60 customers living with enduring long term mental health. We also provide a service for two short-term post hospital discharge schemes for customers, who need support following hospital admission.
The role of our Support Worker play's a crucial role in fulfilling our customers expectations, making a positive difference and where appropriate supporting customers to step down to a lower level of support. Our Support Worker's are guided and mentored by 2 team leaders alongside a Contract Manager and as part of the team you will be developing and sustaining a good quality service to all our customers. An essential part of this role involves developing good partnership working with a range of professional's and maintaining excellent record keeping in line with our compliance management. More importantly, you will be required to oversee support planning, updating Risk Management Plans, and Keyworking. East London Apartments provides 24 hour accommodation based support for up to 31 individuals with complex histories, multiple traumas, complex personality disorder(s) and a varied, disrupted housing history with experience of homelessness as well as individuals who have been diagnosed with severe and enduring mental health conditions who are under the Care Programme Approach (CPA).
When working with individuals with complex needs, who may display challenging, chaotic behaviours, regularly testing boundaries and have experienced multiple traumas as a result of abuse, neglect, years of street homelessness, or long stays in mental health hospitals, it is integral that the post holder is motivated to support individuals to progress. Working closely with the prison and their staff, people in prison, Shannon Trust volunteers and mentors, this post will lead the delivery and development of our literacy and numeracy programme at HMP Belmarsh, maximising opportunities for people in prison to learn to read.
Ideally you will have some experience of prison settings underpinned by the ability to build relationships and personal qualities that include resilience, determination and a problem-solving approach.

IBIS House MH Step Down & Crisis
Working in partnership with the London Borough of Newham and East London Foundation Trust, IBIS House supports people who are medically fit for discharge from mental health inpatient stays but have other barriers to discharge. These may include housing, living skills, accessing benefits or the community, or other interventions and will be on a person-centred basis. The main aim of the service is to provide short-term support to customers to link with longer-term services to reduce escalations in need, and reduce inpatient readmission and reduce pressure on inpatient beds.
Shifts will be on a rota pattern and include morning shifts (08:00-16:00) and afternoon shifts (14:00-22:00).

Inducting new customers, and helping orient them to the community and local area

Building a relationship with the local borough and any key agencies. For example: no recourse to public funds teams, homelessness teams, community mental health teams etc.

Teresa House and Hamlets Way are two supported housing accommodations for adults with enduring mental health issues. Both services provide a high level of support to 16 customers on each service. Teresa House is a fast stream service where customers are supported up to two years while Hamlets Way is a slow stream service where customers can expect support for a minimum of two to a maximum of 4 years.
The service is under Look Ahead management since April 2015 and the buildings are owed by Notting Hill Genesis. About the role:
The post holder will work within The Passage’s Community Services as a Complex Case Outreach Worker and will lead on case management for people experiencing homelessness who are presenting in the footprints in which the team operate and/or who are accessing the Resource Centre. The post-holder will support individuals with substance use support needs and/or mental health issues and will lead on work with clients who are experiencing alcohol and/or drug dependencies and who may also be experiencing mental health problems or other health or social care needs.
Main duties:
- To manage a caseload of clients presenting within the footprints in which the Engagement & Outreach service operate and individuals who are accessing The Passage Resource Centre. To support clients with complex needs, supporting them to identify and work towards recovery and away from homelessness.
- Carry out assessments, case management, support planning and the delivery of interventions for clients including those experiencing mental health and/or substance misuse issues who are multiply-disadvantaged in their homelessness.
- Provide assertive outreach and engagement with street-based service users presenting with complex needs and barriers to engagement. Carry out monthly street-based audits to inform reporting and analysis of service outcomes.
- Support clients who are rough sleeping or involved in day time activity in the footprints in which the team operate to end their homelessness by linking them in with appropriate internal and external support services relevant to their needs. To provide support to clients who are accommodated, but may require ongoing support in order to maintain this accommodation, linking them in to appropriate local services.

Previous experience of working with vulnerable, at risk and disengaged children and young people

A passion for building relationships with children and young people and helping them grow and develop.

Experience of building partnerships with other organisations and communicating with professionals as well as parents and carers

Knowledge of formal and informal education systems.

Ability to be flexible and adaptable, reliable and innovative.

Ability to work under pressure, be self-motivated and driven to succeed

Well-developed people skills and excellent organisational skills.

Excellent written and oral communication skills.

Good working knowledge of main office applications (word and excel)

An up to date DBS.
