About the role:
The SHP Somerset Cluster is based in the heart of the London’s theatre and shopping districts – the main hub is just off Drury Lane, with the other sites near Oxford Street and Tottenham Court Road. The aim of the service is to support people over the age of 18 to navigate various support needs and develop the skills required to maintain independent accommodation long term.
As a multiple disadvantage (MD) referral and move on coordinator, you will work as part of a larger support team to ensure that consideration is given to the individual experiences of multiple disadvantage when it comes to accessing and moving on from our service.
You will work with people from their first interaction with the service: at the point of referral, ensuring that all aspects of someone’s experience is considered by undertaking a trauma informed, culturally sensitive assessment and supporting your colleagues to develop their awareness of these experiences.
From here you will then support their journey through the service by working alongside other support staff to foster a culture of move on, supporting the development of holistic and realistic move on goals.
This work will be underpinned and supported through the development of a move on programme. This programme, developed and overseen by the MD Referral and Move on Coordinators in partnership with the Recovery and Opportunity Coordinators, will be not only look to develop confidence and ability to navigate social systems, but also be accredited through SHP’s AQA Registered Learning Centre.
In addition, you will carry your own caseload for people living in the step-down services to ensure that they are prepared, equipped, and confident about moving on. This may include identifying current barriers within the system that can prevent someone from moving on successfully or within their set timeframe.
About you:
- Experience of coordinating projects across multiple teams or people with varying roles to achieve successful outcomes for vulnerable people
- Knowledge and awareness of the intersection of needs in people facing multiple disadvantage & strong understanding of how the intersection & co-occurrence of these issues can make it difficult for people to engage with support or to move on from services.
- The ability to judge the appropriate response to situations that you find challenging or emotionally difficult
- The ability to work as part of a team to ensure that everyone is clear in their roles and provide peer support and guidance when needed, working through any arising conflict professionally and successfully
- Strong time management skills, ability to work on own initiative, manage competing priorities and maintain high standards.
- Excellent administrative and IT skills including Microsoft Word, Excel, database usage and e-mail, and the ability to interrogate and extrapolate data from various sources

Who are you?
The successful candidate will have previous experience as a CQC Registered / Regional Manager or Quality professional, with experience of auditing within Health and Social Care, and a proven track record of leading services to “Outstanding.”
You will have an in-depth knowledge of safety, health and social care regulatory compliance and supporting legal frameworks, to be able to coach, advise and mentor care and support colleagues within your designated areas. You will be skilled in embedding processes to drive quality standards, and have a solid understanding of assurance and “what excellent looks like.”. A self-starter, you will have a continuous improvement mind-set and enjoy building strong relationships with your stakeholder, and will have excellent communication skills.
A level 5 Diploma in Health and Social Care or equivalent, and a recognised Auditor qualification (or willing to work towards one) is essential. You will have experience in quality improvement methodology, risk management and root cause analysis, with a willingness to work towards any required qualifications.

The successful candidate will have an understanding of working within the substance misuse field and an understanding of associated health and social issues. They will preferably have experience of working with both adults, children and young people. Having been paused to new referrals, Action for M.E.’s free, independent, Adults Advocacy service will soon be re-opening. It’s one of our range of free Support Services that work with adults, young people, and/or families living with M.E. We support colleagues across the team to take part in and share reflective practice, self-care, peer-support and training.
Our Adults Advocacy service offers non-statutory, single-issue, instructed advocacy for adults with a confirmed diagnosis of M.E. We encourage self-advocacy where possible, though many of our clients are too severely affected by M.E. to be able to take undertake this.
As an Adults Advocate, you will manage a caseload of clients, as well as inputting and contributing to the triage process along with your Advocacy colleagues. You will meet with clients by phone, email, Zoom and/or SMS; we can also facilitate communication by post if this is required. We are not able to offer home visits or face to face meetings.

Summary of role: Our Green Doctors deliver home visits and telephone consultations across Kent & Medway. We aim to reduce our client’s energy bills and save them money in a variety of ways including providing a bill switching service.
You will be a practical, people-person with a passion for helping vulnerable people. You must have either knowledge or experience in energy efficiency to provide tailored energy saving advice, and the ability to adapt your approach to suit our client’s varying needs.
You must be comfortable with lone working and in managing your own workload.
A qualification as Domestic Energy Assessor (DEA), NVQ Level 3 (6049-03) Provide Energy Efficiency Services, or City and Guilds Energy Awareness (6281-01) is desirable, but relevant experience within the field or a willingness to take the qualification will be considered. (20 paid study hours).
A full clean driving licence is required.

We are looking for someone who is excited by the opportunity and challenge to deliver these improvements and can offer a clear vision and drive the creation of an efficient strategic value add finance, compliance, and governance department. As a member of the Senior Leadership Team and a key organisational leader, you will also have a strong role to play in building a positive, enabling, anti-racist, inclusive culture alongside your SLT colleagues and the Board of trustees.
