We want to ensure that we are easy to access, working with local communities and providing the best support we can to residents and our partners such as secondary care. Hospitals need to free up beds by discharging patients who do not need to remain in hospital. Despite the pressures we want discharges to be safe, successful and as painless as possible for patients. We also want to ensure a patient is not readmitted into hospital. 

The Hospital Discharge Community Support Coordinator will match Age UK Lambeth (AUKL) volunteers and partner organisation volunteers to support a patient's discharge. Volunteers could provide support with transport, practical support such as food shopping or completing home safety checks. Upon discharge volunteers may continue supporting patients for up to 4 weeks, providing support in person (at the individual's home or in the community) and over the phone, linking them into support services and helping them access the community to prevent hospital readmission.

What you’ll be doing?

1. Oversee the development of a new, inclusive volunteer programme that offers a range of diverse and challenging volunteer roles to support patients leaving hospital and once settled home. 

2. Work in partnership with the VCS to create 100 volunteering opportunities to support hospital discharge.

3. Using the volunteer platform, Care for Me, track allocated tasks to ensure they’re completed. Monitor volunteer reports to review the support and work delivered under the project.

4. Develop and maintain strong working relationships with King’s College Hospital and Guy’s and St Thomas’ Hospital to ensure appropriate referrals are made into the service.

We are looking for someone who is outgoing and enthusiastic to join our Supporter Care team, who will carry out essential supporter engagement and administrative activities associated with marketing campaigns, donations, fundraising events and requests for materials. With previous experience of providing excellent customer service, you will have the ability and confidence to communicate with a variety of people, engaging with supporters by phone, email and in writing about a wide range of enquiries.

The Supporter Care team at Alzheimer’s Research UK is often the first point of contact in the charity for supporters, potential donors and members of the public. The Supporter Care Administrator works alongside five other team Executives handling supporter enquiries, processing donations, and thanking supporters to ensure an excellent standard of customer care and to generate the maximum amount of income for dementia research. This role reports to the Senior Supporter Care Officer (SSCO) and ensures that the team successfully deliver an efficient and a first-class supporter experience for the charity.

This role will be known internally as Supporter Care Executive.

Main duties and responsibilities of the role:

Supporter engagement

· Answering inbound telephone calls; responding to a wide range of enquiries including marketing campaigns, donations, fundraising events and requests for materials.

· Managing email enquiries, either by responding or forwarding as necessary.

· Responding to written enquiries received in the post.

· Making outbound telephone calls in order to increase the value of, or to retain supporters; including thanking, obtaining Gift Aid declarations and reactivating lapsed donors.

· Recording feedback received by the charity and assisting in the resolution of complaints.

· Champion high quality supporter care, ensuring that all supporters and members of the public have an excellent experience when contacting the charity.

Donation processing, database and general administration

· Processing donations from telephone calls and mailings, acknowledging supporters appropriately.

· In Memoriam giving; sending out materials, processing donations and sending appropriate acknowledgements to donors, Funeral Directors, and the Next of Kin.

· Regular giving; processing new Direct Debit instructions and Standing Order payments, amending and cancelling gifts as necessary.

· Processing and thanking donations relating to regional fundraising and sporting events.

· Ensuring the integrity of the database (currently Salesforce), by amending supporter details as necessary.

· Opening and batching incoming post.

· Updating team procedures as required.

· Archiving, scanning and filing documents.

Other duties

· To take personal responsibility for ensuring a good working knowledge of Alzheimer’s Research UK when communicating with supporters. This will require attendance at marketing campaign briefings and other staff presentations and continuously building knowledge by taking the time to read daily news summaries, press releases and the staff intranet.

· To recognise fundraising, engagement and retention opportunities when communicating with supporters using a range of initiatives to explain how donations positively contribute towards the cause.

· To actively and enthusiastically promote supporter care at Alzheimer’s Research UK and be willing to take part in various initiatives to help maintain this such as department updates and trainings and being involved in inducting new starters to Alzheimer’s Research UK on the work of the Supporter Care team.

About the role

This role is a great opportunity for someone who is passionate about supporting those who struggle with complex needs, substance use, and dual diagnosis. We are looking for someone who can work flexibly to support residents and participants who struggle to engage with the support available to them through various providers. You will be at the heart of engaging our participants to regain motivation and the strength to step down from acute psychiatric wards and other supported living, by empowering them with the relevant skills and knowledge to support their journey. You will support them towards achieving their personal goals including independent living. You will be based across various homes, hospitals, wards, and supported living accommodations based around Lewisham.

You will provide pathways, support, and appropriate interventions that will enable successful resettlements, and deliver to achieve more safer, integrated communities for our stakeholders. The areas of support will include: Prevention, Transition, Activities of Daily Living (ADL), Support, and Social Inclusion. 

This role will work alongside South London and Maudsley NHS Foundation Trust (SLAM) who provide the widest range of NHS mental health services in the UK. They also provide substance misuse services for people who are addicted to drugs and alcohol with 4,600 staff serving a local population of 1.3 million people.

• Support a caseload of participants, contribute to the development of support plans, risk assessments and reviews.
• Positively promote the pathways available towards independent living within the community. Ensure participants understand what tools and resources are available to them to regain independence.
• Participate in, and encourage R&Ps to participate in the running and development of projects, initiatives, training and volunteering.
• Recognise signs of deteriorating mental health and initiate appropriate interventions to prevent a crisis.
• You may need to attend external appointments when required.

Peabody is launching a brand-new supported housing service in Lambeth for young people aged 19–25. We’re looking for a part-time Housing Support Worker to join this exciting new service and make a real difference in young people’s lives.

You’ll support young adults who have experienced homelessness, trauma, or complex life circumstances—including mental ill health, substance misuse, or time in care or the criminal justice system. This is your chance to help them build independence and a brighter future.

• Providing one-to-one support to a small caseload of young people

Prostate Cancer Research aims to develop and deliver breakthrough medicines and treatments to achieve a world where people are free from the impact of prostate cancer. The organisation believes that it is only through research that we can build a future in which no family will have to fear losing a loved one to prostate cancer. In the last four years, we have expanded the amount of research it funds by four times. 

Prostate Cancer Research is seeking a an experienced, dynamic and driven Trusts & Statutory Executive. The role will take a lead on identifying and securing collaboration and funding opportunities with both existing and prospective partners, developing and managing a pipeline of funding partners that align with PCR’s mission and core programmes of work. 

Since 2018, PCR has significantly increased income from Trusts & Foundations and Statutory sources, as well as working in collaboration with other medical research charities and non-profits. This is a newly created and integral role within the Partnership Fundraising team to build on this work and will be key to identifying and nurturing relationships with prospective partners and funders, as well as developing and implementing comprehensive partnership strategies to engage existing and prospective partners. 

The ideal candidate will have demonstrable experience of developing and nurturing relationships with funding partners (with experience of identifying and securing at least five-figure income), understanding complex issues and working with others to identify solutions, and producing compelling funding proposals in support of this work. They will also have excellent communication and interpersonal skills with the proven ability to build and manage strong, impactful relationships. 

Finally, candidates must have a passion for securing funding that will help improve the lives of people with prostate cancer and a commitment to advancing healthcare through strategic partnerships and collaboration. 

Key Responsibilities 

• Conduct proactive prospecting and qualification of Trusts & Statutory funding opportunities to build a robust pipeline of potential funders. 

• Identify and nurture relationships with prospective Trust & Foundations and Statutory funding partners aligned with the mission and values of PCR. 

• Develop and implement comprehensive partnership strategies to engage existing and prospective funding partners. 

• Stay informed about PCR’s current and future work and translate larger, complex programmes of work into relevant, compelling funding asks.
