About the opportunity
Fundraising is critical to delivering our mission and ensuring we can support as many children and young people facing disadvantage to open doors to their future. The Philanthropy Team as a whole has a combined target income for the current year (2023-24) of £1.83 million (growing to £2.35m as our target for next year), of which approximately £850k is from corporate donors and their foundations, £100k is from individuals, and the remaining from trusts and foundations. NHS England recently announced a new transformation fund to be allocated to pilot sites within 12 Sustainability and Transformation Partnerships/Integrated Care Systems across the country to test new and integrated models of primary and community mental health care. The role of Community Connector is fundamental to the developing of these innovative new teams and mental health services based within networks of GP Practices (Primary Care Networks). Indeed, whoever takes on this particular challenge will be instrumental in supporting the ongoing development and mobilisation of integrated primary care mental health services in the future. Along the way, you’ll work jointly with a Mental Health Practitioner to conduct assessments. We’ll also rely on you to help patients identify their socially determined needs and goals, provide self-management tools, and develop personal support plans. Delivering a range of motivational and structured psycho-social interventions, promoting independence through an enabling asset based approach and developing effective relationships with a range of agencies to facilitate a ‘joined up’ approach - these are just some aspects of this vital role.
As well as proven relevant experience, you’ll need an appropriate diploma or degree i.e., in counselling, psychology, social work, probation, mental health, Health and Social Care (level 3), NVQ’s level 3+ and/or community experience of drug, alcohol, mental health work. You’ll also need the ability to work co-operatively and flexible as part of a multidisciplinary team from a service hub in one of three locations and be willing to travel to and from a number of different locations on a daily basis. Comfortable working to the confidentiality, consent, information sharing and safeguarding policies of the integrated service, the ability to manage any challenging behaviour, anger and verbal aggression from patients is important too. And, it goes without saying that you have excellent communication skills and are proficient in the use of office IT.

Transition Together is seeking to continue the work of deepening connection and growing the voice of the Transition Movement in England and Wales through the freelance engagement of three Network Weavers. This exciting project will see this team, supported by Transition Together staff, co-design an in person Transition Assembly early next year to guide the future of our Movement in the UK as well as build relationships with Transitioners across Wales and England and connect to emerging and established networks across the UK. What is expected of you in the first six months?
- You will take on the management of at least one UK Government-funded health partnership grants programme operating across a range of countries in sub-Saharan Africa.

- You will take over project governance structures, managing the inputs of programmes team members, Country Directors and implementing partners.
- You will organise and attend a programme launch event in-country, as well as visit health institutions in sub-Saharan Africa.
MAIN RESPONSIBILITIES
Programme Management
- Coordinate staff and implementing partners to ensure the timely completion of deliverables.
- Quality assure the application of THET’s grant management processes by other team members.
- Ensure compliance with donor/country agreements, regulations and requirements.
- Identify, track and manage programmatic risk.
- Ensure effective communication with partners, donors and other stakeholders.
- Oversee project budgets, including monitoring spend against budget, undertaking re-forecasting, and authorising spend.
- Oversee the quarterly reporting process to donors.
- Coordinate and support workstream leads within the project.
Programme quality and design
- Contribute to the design of new projects: co-ordinating the proposal team; overseeing a needs analysis; liaising with stakeholders; contributing to a Theory of Change; developing content; and developing the budget.
- Develop and nurture relationships with potential and existing donors, partner organisations and consultants as appropriate.
- Contribute to the design and improvement of project and health partnership quality programme approaches, including standards, processes, tools and guidance.
- Contribute to the delivery of capacity development interventions for grant holders and THET staff.
Monitoring and Evaluation
- With support from the Research, Evidence and Learning team, oversee Monitoring and Evaluation processes for the programmes you lead, e.g. logframe and reporting mechanisms
- Lead the quarterly and annual reporting and analysis review process with institutional donors.
- Write project reports and programme reviews, and coordination of donor reporting as appropriate.
- Ensure grant holders have effective M&E plans in place and coordinate the review of their reports.
- Coordinate visits to project implementation sites for monitoring and audit purposes.
- Facilitate learning and sharing of good practices across programmes and the wider organisation.
Team Support and Line Management
- Line manage members of the Programmes Team (1-2 Programmes Coordinators).
- Oversee the contracting, payments and deliverables of in-country consultants and other suppliers.
- Lead the induction process for new Programmes Team members and other THET staff when required.
- Work with colleagues from across the team to identify areas for improvement and strengthening.
Miscellaneous
- Attend and present THET’s work at UK and international events, including the THET annual conference.
- Join your choice of THET’s and external thematic working groups, such as on gender equality and social inclusion, on the climate crisis, etc.
- Contribute to the strategic direction of THET and play a leadership role in the Programmes Team

About the Role
Community Connectors work as part of an integrated, blended mental health offer to support people with emotional, social and practical needs to access a range of local, non-medical and non-clinical services, to improve confidence, social inclusion and independence.
This role will involve working as part of a multi-disciplinary team with statutory, third sector and NHS colleagues to support clients who are suffering from moderate to severe mental health difficulties. You will need to have experience working with those suffering with mental health illness as well as being able to demonstrate in depth knowledge of the services, support networks and local resources that may assist in improved outcomes for those dealing with complex and enduring mental health issues. The ability to be adaptable, work independently and the resilience to work in some challenging environments are key attributes for this role.
Service Delivery
- Assess suitability for community connection support as part of a blended team approach and support individuals to access the service by utilising in depth knowledge of local voluntary and community services and networks in your respective area
- To work with individuals to coproduce a plan to address their mental health and wellbeing goals. Am enable people to achieve these goals through facilitating access and connecting with appropriate resources
- Enable people to access community resources which may include accompanying them.
- Offer support, guidance and help where possible to people to gain confidence, overcome initial fears and seek solutions to any barriers inhibiting attendance.
- Take a holistic approach, based on the individuals’ priorities and wider determinants of health that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
- To work as part of an integrated team to ensure the best outcomes for each person accessing the service.
- Identify and build good working relationships with the community groups, relevant services and activities for partnership work and signposting as well as keep updated information about them.
- Competently work as part of a blended team, under direction from Mind BLMK and ELFT, working with independence and in an agile way to support people with complex mental health challenges.
