Hospitals need to free up beds by discharging patients who do not need to remain in hospital. Despite the pressures we want discharges to be safe, successful and as painless as possible for patients. We also want to ensure a patient is not readmitted into hospital. 

The Hospital Discharge Community Support Coordinator will match Age UK Lambeth (AUKL) volunteers and partner organisation volunteers to support a patient's discharge. Volunteers could provide support with transport, practical support such as food shopping or completing home safety checks. Upon discharge volunteers may continue supporting patients for up to 4 weeks, providing support in person (at the individual's home or in the community) and over the phone, linking them into support services and helping them access the community to prevent hospital readmission.

What you’ll be doing?

1. Oversee the development of a new, inclusive volunteer programme that offers a range of diverse and challenging volunteer roles to support patients leaving hospital and once settled home. 

2. Work in partnership with the VCS to create 100 volunteering opportunities to support hospital discharge.

3. Using the volunteer platform, Care for Me, track allocated tasks to ensure they’re completed. Monitor volunteer reports to review the support and work delivered under the project.

4. Develop and maintain strong working relationships with King’s College Hospital and Guy’s and St Thomas’ Hospital to ensure appropriate referrals are made into the service.

Support and Connect is a short-term service, offering up to 10-12 sessions of emotional, social and/or practical support. You would be expected to meet with clients either at Likewise, in one of our partner offices across Camden or within the community spaces across the borough.

About the Project – Door of Hope (East London)

The Door of Hope Project provides support to women who sell sex in Tower Hamlets.  This includes journeying alongside women, whilst providing advocacy, information, emotional and practical support.  The project also has a strong team of around 15 volunteers, who deliver street outreach to women in Tower Hamlets on a Thursday morning and Friday night. 

About the Role

Beyond the Streets is a UK charity with over 20 years’ experience of partnering with women in the sex industry, to see them safe from coercion, violence and abuse. We deliver trauma-informed, person-centred support, provide training, and create resources and reports informed by research, lived experience, and practitioner experience.

Our ‘Direct Work’ is central to the organisation, and we are looking to recruit a new Women’s Support Worker.

Taking a woman-centred approach, your role is to provide holistic support for women with complex needs who want support, or to exit the sex industry. We are looking for someone with energy, motivation and experience of working with vulnerable adults; someone who a highly developed sense of self-awareness who can work in a gender and trauma informed way.

This role will be part of a small team of support workers, and a service manager, who deliver an in-person support service, with women who sell sex on-street in East London. You will have frontline experience of working with individuals who experience multiple disadvantage. This might be in the VAWG sector, domestic abuse, substance misuse, mental health or counselling or direct experience of women in the sex industry. You will have a good understanding of working with vulnerable people and be aware of the need for Safeguarding.

You will be a champion of the cause, have a passion for supporting women who face multiple disadvantages and be familiar with the Violence Against Women and Girls (VAWG) sector.

RESPONSIBILITIES

Programme & Operations Support

• Support the coordination of OSH’s residentials, events, and programmes, ensuring logistics run smoothly.

• Support with phoning participants and referrers to support them to complete sign up forms where necessary

• Track and monitor referral processes, ensuring smooth communication and follow-ups with referrers to maximise participation at our events

• Maintain good relationships with external partners such as venues, facilitators, transport companies and other organisations we work with. 

Finance & Compliance Support

• Assist with financial administration, such as processing invoices, logging donations, and tracking expenses.

• Ensure OSH meets data protection and safeguarding requirements, keeping records up to date.

Other duties

• Getting involved in OSH events in the evenings and at weekends, including residentials. These are overnight trips from 2 to 5 nights

With the support of the Head of Digital and Marketing you’ll work across the organisation to ensure that the Target Ovarian Cancer brand is front and centre, develop and deliver marketing and communications strategies, plans and campaigns to achieve the organisation’s business objectives and KPIs, focussed on attracting and engaging more people who want to use our supportive services, campaign for change, partner with us, and donate or fundraise.

We are an ambitious charity, building on our achievements and targeting what’s important to stop ovarian cancer devastating lives - symptoms awareness, early diagnosis, better treatments and support for all. It’s an exciting time to be joining Target Ovarian Cancer as we move into our ambitionus new three year plan.

The role is a full time, 35 hours per week, one year fixed-term contract position.

We currently offer a hybrid-working model. This means you may usually work from your home or remote location, but are expected to attend meetings and pay for your own travel to the office in Angel, London when required.

We’re seeking a Philanthropy Lead to join our fantastic Philanthropy and Partnerships team. You will provide leadership to a team of fundraisers and secure transformational funding from philanthropic supporters to ensure Pancreatic Cancer UK can deliver its strategy to double survival rates of pancreatic cancer. In this role you will:

• Lead the growth of a robust pipeline of prospects with capacity to give significantly, working with key stakeholders to solicit and close asks as necessary.
• Work collaboratively with senior leadership to maximise high value fundraising opportunities and further a culture of philanthropy within the organisation.
• Be responsible for leading the development and delivery of the philanthropy fundraising strategy.

About You 

• Proven experience as a philanthropy fundraiser, with a strong track record of securing 6-figure gifts from philanthropic supporters.
• Proven experience of developing the skills of a fundraising team.
• Proven experience of building and delivering a major donor fundraising growth strategy.
• Highly skilled at influencing and negotiating with a variety of senior stakeholders.

SHAK (South Hampstead and Kilburn Community Partnership) is a resident led charity based on the Alexandra and Ainsworth estate in the Kilburn Ward of Camden, London, dedicated to supporting residents and young people living in an area of high deprivation.

If successful you will be based at “The ARC Youth Club”, an estate based youth led centre with integrated music studio, kitchen, games and "chill-out" space. The ARC engages approximately 250 young people per year and plays a vital role in their welfare and development outside of school. In holidays we offer a full program of activities and free meals for young people who may otherwise miss out.

You will deliver a range of activities e.g. cooking, arts & crafts, sports, trips, music, events etc. You will also offer support and guidance for local young people and enable them to develop life long skills while designing and running their own projects, forums etc. In school holidays you will be required to work an expected 28 hrs a week as we offer a full program of activities and free meals for young people

This role would be a floating officer based in the Havering borough supporting across the numerous services providing Intensive Housing management support. 

Shift Pattern: 37.5 hours per week, 9:00AM -17:00PM or 13:00PM - 21:00PM Monday - Sunday, Onsite. 

Salary: £26,100

Funded by the Money and Pensions Service (MaPS), this team delivers expert guidance and practical solutions to help clients manage and reduce their debt. The team's advisors are trained to navigate complex financial situations, offering tailored advice that empowers clients to regain control of their finances. Their work is vital in fostering financial resilience within the community, ensuring that individuals have the tools they need to achieve long-term financial stability.

Scope of role

The Debt Technical Supervisor is responsible for supporting Debt Advisers to ensure the delivery of accurate, effective, and tailored advice to over-indebted clients. This role involves conducting debt-advice interview observations, reviewing case files, and identifying training needs for advisers. The Debt Technical Supervisor provides real-time support and constructive feedback, helping advisers navigate complex cases and ensuring they have access to the necessary resources and information. Additionally, the role includes auditing delivery partner organisations to prepare them for external reviews and supporting them in meeting compliance and quality standards. The supervisor will collaborate with the Quality Assurance Manager to promote best practices, address risks, and contribute to the continuous professional development of advisers.

Key Responsibilities

Support Debt Advisers in their roles and ensure that the advice provided to over- indebted Clients is accurate, effective and tailored to individuals’ circumstances.

• Conducting debt-advice interview observations.
• Undertaking debt advice file reviews.
• Identifying advisers’ soft skill and technical training needs.
• Providing constructive feedback, and pastoral support, to advisers and their line managers.
• Providing real-time support to advisers, which will include but is not limited to providing guidance in complex matters and provide advisers with sources of information.

Support, and regularly audit delivery partner organisations, ensuring that they are prepared for external reviews of their service in line with the Grant Agreement.

• Ensure partner organisation are kept up to date with the requirements for external assessments.
• Supporting partner organisations with the preparation and review of files and documentation ahead of an external assessment.
• Support partner organisations with the implementation of any remedial actions following an external review and support advisors to identify suitable training to improve areas highlighted.

Ensure project compliance and quality requirements are achieved

• Working closely with the Quality Assurance Mangers to ensure Advisors based at delivery partners have a good awareness and understanding of the quality and service delivery requirements and have active involvement in the delivery of partnership events related to both quality improvement and CPD.
• Working collaboratively to share good practice in performance and quality improvement and to progress key priority areas of work.
• Escalating risks promptly and appropriately and contributing to wider monitoring and reporting requirements.

Contribute to Team

• Work to an agreed work-plan meeting targets and milestones
• Prioritise and manage your own workload.
• Be an active member of the team, identifying opportunities for your development. Work in collaboration with our funder, and with colleagues from across their other funded organisations, contributing to key projects and meetings.
