GPIMHS is an emotional and wellbeing service that supports adults aged 18+ in primary care who are experiencing a range of mental health difficulties. Clients are offered extended consultation times, quick and easy access to practical advice, and tailored support for their mental health needs from a multidisciplinary team.
Clients can explore the situation affecting their wellbeing – Whether that is an ongoing mental or physical health problem, loneliness, debt, or other issues. They can then be guided to appropriate resources that may help, including talking therapies, benefits advice, or an introduction to a local community group.
The role of the Community Connector is fundamental to the development of these innovative teams and mental health services based within networks of GP Practices (Primary Care Networks – PCNs). The role supports the ongoing development and mobilisation of integrated primary care mental health services. GPIMHS is delivered in partnership between Surrey and Borders Partnership NHS Foundation Trust (SABP) and the voluntary, community and social enterprise (VCSE) sector.
The Role
As part of a multidisciplinary team based in primary care, the Community Connector will:
- Collaborate with the Mental Health Practitioner to conduct assessments (including risk assessments), assist with triaging incoming referrals, support clients in identifying socially determined needs and goals, provide self-management tools, and facilitate personal support plans.
- Deliver motivational and structured psychosocial interventions to promote recovery and wellbeing.
- Encourage independence through an asset-based approach that builds on individuals’ strengths, preferences and natural support networks.
- Establish and maintain effective relationships with GPs, practice staff, health practitioners, and a wide range of agencies to ensure coordinated support and continuity of care.
- Support clients in accessing community services including Community Connections, housing providers, family and carer support, debt advice, and employment services.
- Facilitate engagement with key services such as Safe Havens, CMHRS (Community Mental Health Recovery Services), and local wellbeing activities.
- Connect clients with peer support and volunteering opportunities and provide support to peer support workers within the PCN.
- Assist with community resource mapping, maintain a database of local resources, identify gaps in provision, and collaborate with VCSE organisations to develop needed services.
- Broker and build partnerships between public and voluntary sector agencies to enhance service delivery and access.
- Complete quality outcome measures with clients and conduct regular reviews (i.e. DIALOG+).
- Attend regular supervision with the Mental Health Practitioner (case management) and Team Manager (line management).
About You
Qualifications and Experience
- A diploma or degree in an appropriate subject e.g. counselling, psychology, social work, probation, mental health, Health and Social Care (Level 3) or NVQ Level 3+.
- A minimum of 2 years’ experience of mental health work.
- Proficient in Microsoft Word, Outlook, and basic Excel skills.
- Experience of using electronic patient record systems/case management systems (e.g. SystmOne, Halo, IAPTus) and/or ability to learn how to use the electronic patient record system.
- Ability to enter and manipulate data on a database as required by the Service and Commissioners to include in monitoring and reports.
- Ability to work from home and maintain confidentiality and professionalism when and if required using Microsoft Teams and other virtual platforms.

The Role

The Team
IPS Grow is a national programme designed to support the expansion of Individual Placement and Support (IPS) services in mental health, primary care, and drug and alcohol teams across England. The programme is led by the organisation in partnership with the Centre for Mental Health. It is funded by NHS England and Improvement (NHSE/I), the Department for Work and Pensions (DWP), and the Office for Health Improvement and Disparities (OHID).
A core element of the programme is the expert assistance provided by a team of IPS professionals, known as IPS Grow Leads, to support providers to improve their fidelity to the IPS model and achieve better employment outcomes for their clients.
About You
Successful candidates will be able to demonstrate:
- Experience of designing and leading data strategy at a senior level. This encompasses how we collect and manage data, as well as using analysis to produce relevant and actionable insights
- Experience of successfully managing senior stakeholder relationships, including communicating technical concepts to non-technical audiences

You will be central to designing, conducting and writing up this ground-breaking research project with the support of the wider policy team. This would be an ideal role for recent master’s or PhD graduates or someone completing a research degree, though we welcome candidates from all backgrounds.

In this role, you will be an integral member of the Events and Community Fundraising Team. You will work closely with a team of field-based colleagues providing support, ensuring the growth of income and development of long-term relationships with supporters.

Job Description
- To recruit, manage, motivate, coach and develop a staff team including carrying out annual appraisals and regular supervision meetings. To manage their health, safety and wellbeing.
- To promote and embed Catch22’s vision and values in local service delivery
- To ensure all service user information, interventions and other data as required by the contract are recorded on the relevant data management system in line with Catch22’s policy.
- Weekly review of service data to identify any risks to service delivery, and outcome performance and put in place corrective actions.
- To be the designated safeguarding lead in the service, ensure that all safeguarding processes are followed and to be Catch22’s representative on local safeguarding boards where appropriate.
- To manage operational risk, health and safety, governance and safeguarding issues in line with Catch22 and Commissioner policies.
- Carry out regular case file audits as per the Audit Policy and Procedure and implement any service improvement or process change as a result of audit outcomes.
- To monitor referrals to the service ensuring they are in line with the volumes and eligibility criteria for the service; manage engagement with key stakeholders to promote the service and generate appropriate referrals to ensure the service / funding reaches the most appropriate young people.
- Be responsible for the allocation of referrals and ensure caseload management structures are in place including specialist female and mental health caseloads to maximise impact for young people.
- To ensure that the diverse needs and aspirations of all groups of service users are considered in service delivery and to enable all service users to fulfil their potential.
- To carry out such other relevant duties as may be required and as are commensurate with the nature and level of this post.
- When necessary, hold a small case load of high-risk clients ensuring appropriate support is given to young people according to their need who are referred by means of effective assessment and support planning.
- Provide consultation, information and where required, training and presentations, to other professionals, family and young people from a range of settings.
- Work alongside and ensure effective relationships with Children and Families services and other local agencies and treatment providers
- Liaise effectively with local partner organisations and other relevant agencies to ensure effective referrals, care and risk management as well as ease of transition and referral for young people and to promote the continued joint work of the service and other partners. Attend relevant meetings or delegate to the most appropriate caseworker to ensure Catch22’s service is fully integrated with other services.
- Work with service delivery partners to identify other professionals, statutory and non-statutory stakeholders to include in service training offer to improve awareness and impact of support for women and girls.
- To ensure meaningful participation and active co-production with service users and stakeholders.
- To promote services at local and national level, taking every opportunity to advertise positively the work of Catch22.
- Maintain and update professional knowledge and competencies ensuring that you and staff you manage are up-to date on all relevant Catch22 policies and procedures and are working in line with local and governmental frameworks such as Public Health England, Safeguarding etc. ensuring these are implemented in the service
