We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

What you'll do:
- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions
- Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation
- Develop and maintain links with all key agencies and service providers in the local community
- Empower customers to ensure they receive the service and benefits they are entitled to
- Encourage and enable tenants to pay their rent and to ensure that rent accounts are managed effectively in conjunction with the Housing Management team
- Ensure that referrals are chased proactively so that properties which can be used to house those in need are filled as efficiently as possible
- Adhere to Look Ahead's Policies and Procedures
- Engage in learning and development activity to increase knowledge and skills
- Undertake any other duties consistent with the grade and nature of the post as may be reasonably assigned

About the Service
The Multiple Disadvantage role is aimed at providing specialist support services to women living in refuge with multiple needs, who have experienced domestic, or sexual violence and who have mental illness or use substances problematically. You will liaise with partner agencies ensuring services delivered are of the highest standard in line with the organisation aims and objectives.
About the Role
You will work with women living in refuge within 7 Pan London boroughs, to devise a support plan that meets their immediate practical and emotional needs and start the process of recovery, linking women into more specialist support services where required. The Multiple Disadvantage Worker will undertake risk assessments, safety plans and identify support needs of women such as accessing benefits, budgeting, and explore re settlement pathways from the refuge. You will promote social inclusion, encourage independence and development of personal resilience. You will build up support network for women, promoting and enabling engagement with other services.
About You
Relevant experience and direct work within specialist support services to women/families with multiple needs. Case work experience, providing proactive and emotional support to women who have experienced domestic abuse and provide the support to enable them to recover and rebuild their lives. Relevant experience around working multi agency and being proactive in relation to multi agency working, with a professional manner.

The West London Women’s Service is a specialist gender and trauma informed Assessment Centre for women with complex needs who are at risk of, or have experienced domestic abuse, violence against women and girls (VAWG); and have multiple disadvantages.
We have a great opportunity for someone interested in working as part of an innovate women’s only service in an Assessment and Reconnection role and becoming part of this exciting team achieving positive outcomes to support women in their recovery and move on journey.
In this role you will:
- Be part of a busy team environment working closely with women new to, or at risk of rough sleeping.
- Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.
- Build and maintain effective relationships to support clients throughout their recovery.
- Develop a knowledge of relevant legislation such as, welfare benefit and housing legislation.
- Work on a weekly rota including early, mid and late shifts (some evening and weekend work may be required).
For this role staff can claim an additional 20% pay premium for hours worked between 8pm and 8am Monday to Friday, and all hours worked on Saturday and Sunday.
About you
To be successful in this role you will:
- Experience managing complex and difficult situations in relations to people, have good interpersonal and listening skills, patience and resilience and the ability to adapt to new situations
- Great communication skills, with the ability to network and build effective relationships with a variety people.
- Understanding of the issues involved in providing support to people with complex support needs including mental and physical health needs, substance dependency and who may be on low (or no) income.

We provide a range of services for those affected by drugs and alcohol. Our skilled and compassionate people work closely with services users to change and save lives. We combine proven expertise in treatment and recovery with innovative approaches and actively involve those we help in improving the design of the services we provide.
You will be based in our psychosocial team working with adult males (over 18 years old). You will be responsible for delivering a range of elements of our substance misuse work including case management, assessments, care planning, the delivery of one-to-one interventions to include harm reduction and motivational work, group work, and engagement support as well as other associated duties. You need to be committed supporting clients to make positive changes in order to prepare them for release into the community, and you will arrange continuity of care. You will work in collaboration with the clinical team and integrated working is a key element of this role. You will work within the prison setting and contribute towards the wider drug strategy.
Having experience of supporting people with complex health needs including substance misuse, you will bring knowledge and commitment to deliver drug and alcohol treatment. You will have a non-judgemental attitude and a desire to support the lives of our service users to be healthier and safer by delivering a high-quality service.
Experience of working with drug and alcohol misuse and individuals is essential. You will be a competent communicator, with the ability to work independently and be comfortable and enthusiastic, both with the highlighted client group and working in partnership with other service providers. Previous experience or knowledge of working with clients on opiate replacement treatment, and/or involved in the criminal justice system will be advantageous.

Working in partnership across Berkshire Healthcare Foundation Trust, you will support the testing of new and integrated models of primary and community mental health care.
Community Connectors will support patients to access a wide range of community services and resources including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing.

- Work jointly with the Mental Health Practitioner in conducting assessments, including risk assessment

About this role…
Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
Thames Reach welcomes applications from people with transferable skills.
You will:
• Complete outreach shifts, providing a rapid response to referrals for people who are rough sleeping across four districts (Mole Valley district council, Tandridge district council, Epsom and Ewell Borough Council, Reigate and Banstead borough council).
• Support our clients with practical tasks that will enable them to move off of the streets or sustain accommodation.
• Directly support clients, identifying activities that will assist in their journey towards recovery and away from life on the streets.
• Maintain and support links with community programs and resources.
• Ensure that all paperwork is completed and work is recorded and monitored.
To succeed as the Assistant Support Worker you will have:
• A strong commitment to providing a high standard of service.
• Have an interest in homelessness and a desire to support us to have a positive impact on the lives of homeless people.
• Excellent organisation and time management skills in order to remain strongly outcome focused, with the ability to coordinate a high number of cases and complete tasks promptly and efficiently.
• The ability to learn and an enthusiasm for training and acquiring new skills.
• Confident in building strong working relationships with partner agencies, being able to challenge them when required.
• Strong IT skills with the ability to use Outlook, online recording systems, Excel, and Word, using a tablet and a smart phone.

This is an exciting opportunity for an individual who has experience of working in the social housing sector and would like a new challenge. You should have Knowledge of Housing Law in relation to housing management and understand Welfare Benefits, especially Housing Benefit and the current climate of how supported housing is funded via Housing Benefit and the scrutiny around evidence of the support offer.
You will be the main lead for housing management in your patch. Experience of managing income recovery, neighbour nuisance/conflict and estate management is essential, ideally this has been within a supported housing context.

- Preparing documentation, including notices, witness statements and court documents for any enforcement or to end a tenancy.
- Representing Nacro at court.
Income recovery you will be responsible for recovery of accommodation charges from beginning to the end of the process.

- You will ensure the Association’s conditions of tenancy and licences are enforced.
- You will provide training and induction support to new starters, so they understand their role around housing management and tenancy creation including dual visits to provide additional training where issues are highlighted, or new staff need it.
- Supporting and advising on less complex anti-social behaviour to promote prevention and early intervention, and the correct recording and monitoring of cases.
Estate Management/Property Standards
- Though the Occupancy Sustainment Programme (or similar) deliver training to local teams’ duties on property standards including documentation and recording in systems, to include reporting a repair, completing monthly fire and health and safety checks and support they need to provide around gas compliance.
Support the team through training to help workers know how to access our policies and procedures around property standards including knowing how to report and escalate issues.
