This is an exciting opportunity to join the team in Herefordshire and Worcestershire.  We’re looking for an enthusiastic, motivated and well organised individual to join the team in delivering a comprehensive Stroke Recovery Service across the region.

This is an exciting opportunity to work with stroke survivors and their families to support them following a stroke.

Position: S11266 Stroke Support Coordinator

Location: Home-based, Herefordshire and Worcestershire. However, extensive travel will be required as part of this role (travel will include team meetings or other work-related meetings).

Hours: Part-time, 28 hours per week

Salary: Circa £21,948 per annum (FTE circa £27,435 per annum)

Contract: Services are contracted and there is currently funding for this contract until 31 March 2026.

The Role

Reporting to the Stroke Service Delivery Coach, the coordinator will work with the local Health and Social Care services to receive referrals and support stroke survivors and their carers. As a coordinator you will provide personalised information, advice and support with regular reviews throughout.

Key responsibilities will include:

• Working with the local Health and Social services to receive referrals to support stroke survivors and their carers.

• Providing personalised information, advice and support to address any needs identified.

• Completing Needs Assessments with stroke survivors and supporting them to develop a stroke recovery plan.

• Providing regular reviews to support people in establishing and achieving their own personal goals. 

• Completing 6 month Post Stroke Reviews.

About You

You will have experience and have a proven record:

• In a caring and/or charity profession supporting people with disabilities.

• Be able to demonstrate previous experience of working collaboratively with other organisations to achieve joint ambitions. 

• Have experience of working with people who may have additional communication support needs.

• Have the ability to use basic Microsoft system.

This role requires extensive travel across a large geographical locality to visit people at home and in community settings. Candidates must be able to demonstrate how they can meet this requirement of the role

To support charities’ (critical) adoption of AI we need to invest in supporting sector-experiments that create demonstrable impact (acting as showcases), working alongside the peer groups, charities and working groups to help move conversations into practical action.

To achieve that, we are looking to recruit a Head of Design for AI to focus on driving forward this work in a number of different ways:

• Blending expertise in AI technologies with human-centered design principles to foster innovation and inclusivity across the social sector;
• Providing direct support to organisations and specific product design advice;
• Designing a range of experiments and tools alongside the sector and CAST team;
• Supporting the design and development of specific programmes, training and interventions that will help shift the sector on AI

About the Role

Length and Salary

This is a full-time role, on a permanent contract. The annual salary is £42,193 full-time per annum, less any required deductions for income tax and national insurance.

Reprieve operates a hybrid working model and we ask staff to work two days per week from the London office and the rest of the week from home. Job Description

The Youth Violence Intervention Practitioner role is to be part of the youth work team within Redthread with a primary focus on youth violence interventions in a health care setting. In this role you will be part of the Youth Violence Intervention Programme team at St George's Hospital. You will support young people aged, 11-25 years old, who present to hospital following an incident of violence of for whom them there are concerns around exploitation or extra-familial farm. You will work with the young person in the hospital as well as supporting them post discharge to ensure they have the support in place that they need and want. Other tasks include:

• Hold a caseload and work with young people in a range of settings, including in A&E and on the wards, in the community, in face-to-face and group contexts, and promote their personal, educational, health and social development through all interactions.
• Assist with the on-going development of the service model to ensure that clients gain the maximum benefit from Redthread’s interventions.
• Ensure information resources such as client forms, databases and contact files are kept fully up-to-date and secure, in line with GDPR, the Data Protection Act and our Confidentiality Policy and Consent Policy. Maintain high quality recording of interventions with young people on our database, Threads, in line with Redthread’s best-practice requirements. (Training will be provided.)
• Collect and record evidence of project outcomes and young people’s achievements, to ensure the completion of monitoring forms and project progress reports as required.
• Maintain a good level of knowledge of the issues around youth violence and Child Exploitation.
• With guidance and support from the Team Leader, develop skills to further your work with the young people.
• Under the direction of the Team Leader, develop an imaginative programme of events, activities and other interventions which support Catch22’s mission.
