This is an excellent opportunity to truly make a role your own and get involved in the project from the start, which will be supporting 10 adults with mild/moderate to severe learning disabilities, as well as additional mental and physical health needs.
You will play a pivotal ambassadorial role, spearheading Centre’s 404 service expansion into the London Brough of Brent and bringing our expertise, vision and values to families in need.
This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.
The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. About the role:
One role is based at Kings College Hospital the second role is based at Princess Royal University Hospital. As a Hospital Independent Domestic Violence Advocate you will:
- implement effective ways of working with male victims and those supporting them to increase safety and reduce harm
- provide a high-quality, front-line service to male victims of domestic abuse, delivering a premium service prioritised according to risk, primarily focusing on victim/survivors aged 16+
- initiate, develop, maintain and monitor multi-agency links through procedures and protocols to keep safety central to all services for victims of domestic abuse
- specialise and be leading practitioner for Male victims of domestic abuse; providing advice, guidance and single point of contact for a male victims
You will need:
- Have a knowledge of help-seeking barriers and support needs of victims of domestic abuse (in heterosexual or same-sex relationships).
- Have a good understanding of domestic abuse including the impact of domestic abuse on victims and their children.
- Understand the principles of risk assessment, safety planning and risk management for victims of domestic abuse and their children
- Understand safeguarding issues, and the legal responsibilities surrounding these issues.
- Direct service delivery to victims of domestic abuse or other vulnerable people.

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. Assessment & Reconnection Worker - Rough Sleeping Prevention Service

Are you interested in supporting people at immediate risk of rough sleeping and making a real difference to the lives of people who are homeless?
Our client's exciting pilot is a new approach to preventing rough sleeping, through the provision of accommodation with onsite specialist move-on support to those who are at immediate risk of sleeping rough. The service will provide a route and support for single homeless clients who are presenting to the Local Authority Housing Options or day centres and at imminent risk of rough sleeping, but not yet rough sleeping.
Where their homelessness cannot be prevented by the council and they are at imminent risk of sleeping rough, a referral can be made into the Rough Sleeping Prevention service.
The service will be based across three London locations in Islington, Lambeth and Kensington and Chelsea and staff will be expected work across all three sites.
- 7 day rolling rota including early, mid and late shifts.
- (K&C): medium – high support accommodation.
- (Lambeth): medium support accommodation.
- (Islington): Assessment hub space for up to 15 clients. Site will operate 24/7.
Each site will have staff delivering casework and support, providing each person with a comprehensive assessment of their needs and circumstances, and intensive specialist support to secure move on.
The service will work in partnership with the local authority to secure onward move-on options e.g. access the council PRS scheme, advocating for access to supported housing.
As an Assessment and Reconnection Worker, you will:

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 7th anniversary this year. We are seeking an individual to join our successful service in Southwest London. This role will help to strengthen the links between Primary Care, voluntary and public services to improve patients’ health and wellbeing.
You will support patients within Primary Care GP practices in Merton, and you will support each patient to access appropriate services provided by voluntary and community organisations and other agencies. 132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Undertake initial and continuous assessment of needs including support needs and daily living skills. This would include potential risks and an assessment of the customers independent living skills as a whole.
The initial and co-produced assessment will incorporate models of co-production, observation, and person-centred planning. This is with a view that it is solution focused as to provide a framework for progression towards independent living.
Liaise with partner agencies regarding the assessment process seeking advice and guidance as to specialist input and clinical input as part of multi-disciplinary assessment process.
Create support/ action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
Empower customers to be part of the local community and active participants in the local community as part of a Recovery Capital model.

Oaklands & St Kilda is a pair of 6-unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.
Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.
This is a Waking Night role working to support young people throughout the night at the service. This role completed 10 hour shifts 4 days a week across all days of the week.
