Role overview:
As a Lead Assessor, you'll play a pivotal role in upholding the integrity of our AP scheme. Your primary responsibility will be to conduct comprehensive assessment visits to units undergoing evaluation within the scheme. Working hand in hand with service user perspective assessors, you'll monitor all Headway AP core domains during onsite visits.
Key responsibilities:
- Review relevant documentation ahead of onsite visits.
- Collaborate with service user perspective assessors during visits, focusing on core domain monitoring.
- Facilitate post-visit debriefing meetings with assessors to ensure thorough discussion and alignment.
- Maintain open communication channels with the AP Manager to address any assessment-related queries or challenges.

Hourly rate and expenses:
The hourly rate for this role is £20.00, covering administrative tasks pre and post-visit, visit duration, and travel time. Additionally, travel expenses will be covered.

- Hold a professional qualification (e.g., OT, SaLT, Nursing, Physio) with demonstrated experience in ABI.

We are seeking a dedicated and compassionate Community Wellbeing Support Worker to join our Flintshire Wellbeing service. This role does not provide direct basic care but is focused on motivating and supporting individuals to identify their own goals as well as developing self-management strategies to improve their health and wellbeing and maintain their independence.
This role will ensure that guided conversations are led by the individual and that they are at the centre of everything we do.This is a new service commissioned by Flintshire County Council on behalf of health and social care partners through the Welsh Government Regional Integration Fund. The service has been commissioned to fill a gap identified within existing service provision.

The Flintshire Wellbeing service aims to support individuals for a period of up to 6 weeks following hospital discharge. We work collaboratively with individuals to help them identify their strengths, overcome limiting beliefs, and develop practical strategies for achieving their goals at home and within their community.
A key element of this role will be to work with the individual and co-design a personalised action plan. From this information, meaningful functional goals and outcomes are developed with the individual, to promote wellbeing, autonomy, independence, and choice. Who we are
The problem - On average, 160 young people are excluded from school each week. Once excluded, prospects for these young people are bleak, with only 4% achieving good GCSE grades, and 67% entering sustained education, employment or training compared to 96% of their non-excluded peers. This can be a pipeline to prison, with concerning statistics showing 63% of prisoners having been excluded from school.
Our mission - To ensure that young people who have been excluded from school are not excluded from society.
Who we serve - We support the hardest to reach young people in the most disadvantaged communities, each of whom are most at risk of experiencing school exclusion or are at risk of being excluded. Young people who are eligible for free school meals are 4x more likely to get excluded, along with black Caribbean boys. Young people with special educational needs are also six times as likely to be excluded as their peers.
Our work - We work in a number of different settings both during term time and after school. Our coaches lead small groups of 8-10 young people, using sport as a hook and mentorship as an anchor to develop their soft skills, as well as improve their physical & mental wellbeing. This, coupled with exposure to the work place through Career Taster Days, raises their aspirations and puts young people in a better position to enter sustained education, employment or training (EET) when they leave school.

We are recruiting Youth Workers across our delivery locations in England and Wales to join our squad in August for the new academic year. This is a frontline role where you will spend the majority of your time working with young people, building relationships with them and delivering our full programme of activities which includes rugby based activity sessions, employability workshops, career taster days and more.
You will be working with some of the hardest to reach young people who are most at risk of exclusion, in small groups of 8-10. We use rugby as a hook and mentorship as an anchor, using our four cornerstones (developing life skills, raising aspirations, improving physical wellbeing, focus on mental wellbeing) to develop young people throughout their time with us. We enable young people to be in a better position to enter sustained education, employment or training (EET) when they leave school.
The delivery team always work in pairs, running our interventions across a variety of settings (including mainstream schools, Pupil Referral Units, Alternative Provisions, SEN schools, Youth Offenders Institutes, community centres and rugby clubs) working in the school timetable and between 3-7pm. While delivering our interventions is a big part of the job, you will also be responsible for supporting the team to ensure the successful collection of data, managing school relationships and delivering on partnership projects.
You do not need to be a rugby fanatic for this role, but you do need to be passionate about social mobility of the excluded. We are looking for someone with a youth work, teaching, mentoring or sports coaching background to help strengthen our team as we work towards our forward strategy.

Position: S11170 Stroke Association Support Coordinator

Reporting to a Service Delivery Coach in the East of England, you will have the ability to support stroke survivors to identify their goals. Using strong assessment skills and a person-centred approach enabling them to maximise their recovery and improve communication.

· Supporting new stroke survivors and their carers to adjust to life after stroke following hospital discharge into the community.

Adult Care Home Manager
Reports to: Regional Manager
Responsible for:Team Leader, Learning Disability Nurse, Support Workers
Job Purpose
To oversee the day-to-day delivery of CQC registered high quality dignified residential care and support services promoting respect, equality and the independence of the people supported.
Principal Accountabilities
- Efficiently manage the day-to-day delivery of the adult residential care home service
- Manage staffing levels to ensure sufficient numbers of suitably qualified staff.
- Effectively manage the staff team providing information, guidance and on-going supervision and performance management reviews.
- Effectively communicate information and instructions to staff and people supported.
- Manage and coordinate the delivery of staff training and development.
- Manage and monitor the local budget.
- Provide leadership to ensure staff are clear about their duties and responsibilities.
- Responsible for the safe delivery and accurate recording of care in line with legislation, the CQC, and AFG policies.
- Being the CQC Registered Manager for the adult residential care home to ensure compliance with Health & Social Care Act 2008 (Regulated Activities) Regulations 2014 and all other relevant legislation.
- Keep up to date with best practice in social care and apply this knowledge to your work.
- Understand and monitor health and safety in the workplace.
- Manage and coordinate governance and quality assurance activity.
- Promote the interests of people supported and provide a person-centred service.
- Conduct assessments of prospective new residents and carry out all duties required to commence care and support.
- Provide clear information to all people supported and their representatives about the adult residential care home.
- Communicate effectively with people supported, their families and representatives, staff and other health and social care professionals to deliver the best possible care and support.
- Maintain confidentiality for people supported, their families and AFG employees.
- Interview prospective candidates and coordinate inductions for new staff.
- Implement AFG policies in relation to absence, disciplinary and grievance matters
- Participate in the on-call responsibilities appropriate to the role
Person Specification - Essential Requirements:
Experience
- 3 Years experience as a CQC Registered Manager in an Adult Residential Care Home setting.
- Experience of leading a CQC inspection resulting in ‘Good’ or Outstanding’ ratings.
- Sound knowledge of CQC regulations.
- Experience in managing Human Resource issues.
- Experience in interagency working.
- Experience in the management of local budgets.
- Experience of managing services using the CQC Guidance of ‘Right Support, Right Care, Right Culture’
- Significant experience of supporting adults with learning disabilities and/or complex needs, behavioural or physical support needs.
Skills and Knowledge
- Demonstrate values consistent with those of the organisation.
- Ability to implement initiatives relating to service delivery, improvement, and modernisation.
- Knowledge of, and lead on the ongoing development and implementation of systems that ensure compliance with regulatory/inspection regimes.
- Knowledge of current best practice in relation to people with learning disabilities and behaviours that challenge.
- Ability to manage change.
- Ability to manage and lead teams of staff.
- Understanding of the wider issues affecting the sector.
- Understanding of the implementation of financial procedures and managing the service budget.
- Ability to communicate clearly and concisely.

- Computer literate and experienced in using Microsoft Office packages.

This role sits within our Youth Impact Domain, which works to provide immediate one-to-one and group support for children who urgently need support. Your role will be to help us maintain and develop innovative practices to support our work in our services nationwide. As well as this, you will design, deliver and evaluate high-quality safeguarding and practice training to support and develop our direct practice colleagues.
We are currently looking for someone with a wealth of safeguarding experience and confidence in training delivery to join our dynamic, ambitious team.
A key part of this role will be your ability to:
-Provide safeguarding advice, guidance and instruction
-Design, development, review and delivery of safeguarding-related training across the organisation.
-The delivery of quality practice-related activity within the Youth Impact domain.
-Contribute to relevant cross-organisational groups, such as policy development.
-Manage internal audits, case file reviews, internal fact-finding, and leading learning reviews of safeguarding and quality practice-related matters.
In order to be successful in this role, you must have:
-SW England registered social worker or substantial safeguarding experience
-Relevant service improvement/ quality practice experience
-Experience in designing and delivering online training and learning resources
-An Ability to work alongside colleagues in a supportive way and assume the role of a critical friend, providing a challenge.
