Job description

Working Hours: 5 days per week, 37.5 hours per week

Ruils is a user-led charity based in the Richmond borough that supports disabled children and adults and the elderly to live independently, be part of their community and to live life to the full.

We are working with the Richmond General Practice Alliance (RGPA) and our local Primary Care Networks to deliver and further develop Social Prescribing in Richmond. Social Prescribing is part of the NHS Long-Term plan and enables GPs and other health care professionals to refer patients to a link worker who can work with the individual to connect them with services and activities in their local community.

This post empowers people to take control of their health and wellbeing by giving them time to focus on ‘what matters to me’.

We are now recruiting for talented, dedicated individuals to join our outstanding team of 16 and help develop the service further.

We are looking for people from any previous walk of life with experience of working with individuals to work with and advocate for them in gaining access to the support they need.

We are particularly interested in individuals who have experience of supporting clients with long-term mental health conditions or those involved in end-of-life care.

In this role you will:

  • Work with individuals and their carers on a one-to-one basis to plan for their needs and find the support and activities desired;
  • Engage with community organisations and specialist services that provide that support to ease the path for your clients;
  • Work closely with Primary Care Networks, GPs, health care professionals, and other wellbeing services in Richmond to find the need and locate the support;
  • Have responsibility for managing a caseload and dealing with referrals;
  • Work with people who might be experiencing complex issues, so you will need outstanding listening and communication skills, empathy and enthusiasm.

You’ll need excellent IT and organisational skills, be adept at seeking out the information for relevant community services, and have a tenacity to help people achieve sustainable improvements in their wellbeing.
