Age UK Kensington & Chelsea offers dementia services which make a real difference to residents affected by the condition living in the Borough. Those services include advice, practical help, befriending, social activities and emotional support. The Dementia Support Worker is responsible for managing a caseload of individual clients, providing practical and emotional support to the most isolated and vulnerable in our community. The support takes the form of weekly or bi-weekly visits to the client’s home, as well as contact by phone and email and liaising with social care and health professionals as directed by the client’s wishes or needs.

This role requires the worker to work independently and manage their own caseload. With other members of the Dementia Services team, the Dementia Support Worker will also help with transport, group activities, trips, events, training and local forums.

To enable vulnerable people living in a variety of different tenures/property types to manage their tenancies/ licences and move towards greater independence within the community by providing a flexible and individually tailored support package.

This role will consist of four 10 hour shifts, 9 am to 7 pm.

What you'll do:

- Undertake key-working responsibilities for a caseload assigned by the Manager.
- Undertake initial and continuous assessment of needs and potential risks and agree levels of support and actions.
- Create support/action plans for providing appropriate services based on the assessment and reflecting the services and resources available.
- Support customers to undertake all domestic tasks wherever possible, including practical assistance where they have not yet developed the skills, to ensure customers enjoy a high quality of accommodation.
- Develop and maintain links with all key agencies and service providers in the local community.
- Empower customers to ensure they receive the service and benefits they are entitled to.
- Undertake all administrative work and keep accurate and comprehensive tenant/customer records to professional standards.
- Encourage and enable tenants to pay their rent and service charges.
- Ensure that rent accounts are managed effectively in conjunction with the Housing Management team with the Landlord and in the local authority.
- Ensure that referrals are chased proactively so that properties which can be used to house those in need, are filled as efficiently as possible.

An opportunity has arisen for a lead Support worker to work within our Sanctuary crisis intervention and prevention service, based in Fore Street in Enfield. We act as a short-term A&E and Emergency services alternative, providing support to individuals in the Enfield Borough aged 18+. 

This role is to provide leadership and support to the Sanctuary team on shift. The role will:

• Lead shifts during the week, providing support to team members via case management support, escalation, Health and Safety guidance
• Support the Sanctuary Manager to embed processes within the service, and ensure that these are carried out
• Work alongside the Manager and team to ensure that the service is operating to high quality standard.
• Provide support in times of emotional distress for clients. The support worker will provide a calm, supportive presence.

 The role will support clients through co-producing support plans with practical and therapeutic support offerings, as well as contributing to the development of a warm welcoming environment and ‘safe space’ to be.

This position would suit someone with a minimum of 2 years experience of working with people with mild, moderate and severe mental health issues.

LGBTQ+ Heritage Project Worker

Responsible to: Communications and Engagement Manager

Q:alliance provides support, information and representation for the LGBTQ+ people who live, work and play in Milton Keynes and Bedford. We have detailed knowledge of the issues and challenges that organisations and individuals face locally. It’s our goal to create more safe spaces and resources to ensure the LGBTQ+ community is well represented and supported.

This exciting new role will help deliver the directives of a history and heritage project funded by the Heritage Lottery. The project will support the erudition of LGBTQ+ heritage in Milton Keynes, advancing local communities’ understanding of Milton Keynes’ LGBTQ+ history and inspiring a commitment to encapsulating, celebrating, and feeling connected through shared heritage. This post will be key to extending our reach into seldom asked communities, as well as securing relationships with founding community members. This role will support our Communications and Engagement Manager to develop safer spaces for LGBTQ+ local communities through collaboration with partner agencies and delivery of LGBTQ+ cultural competency training to a range of stakeholders involved in the wider project.

You will work cross-departmentally and participate in effective monitoring systems to ensure robust reporting.

Main Responsibilities

• Project Development - Develop and implement the directives of the funded project, supporting key partners in their collaboration with Q:alliance. Communicate project aims with communities and stakeholders and establish meaningful engagement in the concept of encapsulating heritage and celebrating differing LGBTQ+ identities in Milton Keynes. Support the gathering of audio histories and the collection and succession of community artefacts through the archive. Evaluate and analyse impact.

• E-book creation – Support the Communications and Engagement Manager on the development of an E-book to capture the outputs of the project, including key heritage findings and personal stories.

• Event coordination – Plan, organise and support the delivery of history clubs in line with the project plan, ensuring accessibility for a wide range of invested community members. Support the curation of associated exhibition events.

• Stakeholder and community engagement – Work with the Communications and Engagement Manager to plan engagement and outreach activities in line with the directives of the funded project. Forge and support enduring relationships with key partners, strengthening our commitment to building an alliance of cross-sector compassionate advocates for the LGBTQ+ community in its diverse forms.

• Training – Support the development and delivery of LGBTQ+ cultural competency training to a range of stakeholders involved in the project.

• Data capture and intelligence - Support Q:alliance’s commitment to sustaining a contemporary data and monitoring mechanism, guaranteeing we can deliver thorough evaluations of our services and competent feedback to our funders and stakeholders on the efficacy of our work.

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. The Service provides on average 5 hours of weekly support to customers who reside in the hostel with the aim to provide them with the tools to move on within the community and live independently.

We are recruiting for a Dementia Carers Support Officer who can make a positive difference to the lives of unpaid carers in the diverse communities of Redbridge by providing them with practical, emotional and health and wellbeing related RCSS services. Services include the development and delivery of peer social and learning activities to give carers a break and reduce the risk of acute isolation. 

The post holder will focus specifically on carers of people with dementia ensuring they receive advice and information and access key support services.  The post holder will also work closely with partner organisations to facilitate cross referrals for both the carer and the person with dementia.  A key requirement of the post is to feed into local strategic work to improve the support pathway for people with dementia and their carers including being a point of contact within Redbridge as well as attendance and input to relevant partnership boards and other working groups.

This is a job share post, working in close collaboration with the existing Dementia Support Officer. The post holder will also work with colleagues, to act as a bridge between carers and statutory services, contributing towards giving carers a stronger voice in speaking up for themselves and their peers. In this context and in collaboration with the Operations Manager and front-line staff, the post holder will also pay particular attention to the needs of older, isolated, vulnerable carers, aged 65 and over, who face social exclusion, cultural isolation, and hardship. Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. Support is delivered in partnership with a number of stakeholders and partners in the community such as mental health services; substance misuses agencies, housing teams, police and social services. We are recruiting Waking Night Support Workers to join our dynamic service and team. In the role of Waking Night Support Worker; individuals will ensure continues support is provided to young people at the standards of day staff with particular focus on safety of vulnerable young people.

About the Role:

Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. Furthermore they will actively seek out ways to work with the team and stakeholders to improve the experience of young people in the service. The role requires individuals to work on a rota pattern including weekend and bank holiday working, as the service is open 365 days a year. Hours worked: 21:30 - 07:30.

The St Mungo’s Pan London Rough Sleeping Team (No Second Night Out) provide a rapid response service, intervening to ensure no one needs to sleep rough, our work is ongoing and we are currently looking for people to join our services across London. 

As an Assessment and Reconnection Worker, you will: 

• Be part of a busy team environment working closely with people new to, or at risk of rough sleeping. 

• Provide a personalised case management service to clients, turning assessment recommendations into practical action plans which fully involves the client.

• Build and maintain effective relationships to support clients throughout their recovery.

• Develop a knowledge of relevant legislation such as, welfare benefit and housing legislation.

• Work on a weekly rota including early, mid and late shifts (some evening and weekend work may be required).

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)
Team - 1X Contract Manager, 1X Team Leader, 3.2 Support Workers.
Current Vacancy - 1x FTE/1 x FTC Support Worker role who will be allocated to work with customers at both sites.
Staff work closely with Slough Community Mental Health Team (SCMHT) we also have a designated Occupational Therapist working along side supporting staff. Customers also attend Hope College which is facilitated via Slough Mental Health Team as part of their recovery process needed in preparation into independent living.
This is a great opportunity for any support worker to gain exceptional experience within the mental health field, based on the solid working relationship with (SCMHT) We have since implemented training directly from SCMHT to support staff with understanding Mental Health diagnosis and Managing Crisis with our customers in-house.

- Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
