Job Purpose
This role is within our crisis alternative service Safe Space - an out of hours service providing a safe and welcoming space for people who are feeling distressed and experiencing crisis. The service operates 365 days a year from 12:00pm (midday) - 11:30pm across all of our boroughs. The role will consist of two shift patterns to ensure the service is covered: 11:30am-6:30pm and 4:30-11:30pm. This will be on a rota basis across support workers.
Staff within the service will provide person-centred, practical and emotional support, face to face or via telephone on a one-to-one or group basis to individuals experiencing mental health crisis. This will consist of 1:1 support; de-escalating and providing interventions, signposting for specialised or longer-term support, and delivering collaborative self-management plans with clients to improve mental wellbeing. The aim of this service is to de-escalate and prevent further crises.
The objectives of the service include:
- To improve the mental wellbeing of people experiencing mental health crisis in HFEH.
- To provide support to clients accessing the service- for instance: signposting, de-escalation, planned wellbeing activities etc.
- To contribute to an improvement in individual mental wellbeing.
- To remain a source of independent support for all clients.
- To treat service users with respect, dignity and personalised support
- To raise awareness of mental health services available with the goal to improve long term mental health and reduce social isolation
- To increase self-management skills of those accessing the service
- To reduce the use of police, ambulance and statutory mental health services whilst experiencing crisis.
- To reduce the use of statutory crisis services by people experiencing mental ill health without positive outcomes for the individual.
The Role
The role of the support workers will be to support the rest of the team in delivering interventions on a one-to-one and group basis. The role will also involve triaging and assessing all clients accessing the service, signposting and delivering collaborative self-management plans with clients to improve mental wellbeing.

Livingstone House is a 24 hour staffed 92 bed mix gender hostel, which provides short term supported housing for single homeless vulnerable people with Low to Medium support needs. Look ahead provides the support function and Riverside provides the housing management and night concierge. The Service provides on average 5 hours of weekly support to customers who reside in the hostel with the aim to provide them with the tools to move on within the community and live independently.

What you'll do:
To provide support to peers and management, being flexible and responsive and help bespoke support for each customer's abilities and aspirations
To implement the principles of personalisation, as part of a local service plan.

To support up to 14 customers to sustain their tenancies
To promote customer involvement in all aspects of your work
To take on a project that will help assist the service move forward

Campbell Rd Young Peoples service provides support and housing for 55 young people in Tower Hamlets that are either leaving care of affected by youth homelessness. The Support Worker role will be to manage a caseload of approx 7-10 young people .You will be expected to deliver positive outcomes for all our young people and support them to reach independence, keep safe and maintain their tenancy's. Support Worker

We provide support for 16 customers, we have two sites Hope House (10 bed supported housing) Doddsfield Rd (6 flats independent living with floating support).
Hope House - Support hours 24 hours 7 days per week, with sleep- in based on a shift rota pattern - (sleep-ins staff are paid additional to salary advertised).
Doddsfield Rd - Floating support (during working hours)

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. The Service encompasses a mix of units across six sites supporting 57 vulnerable young people and 19 teen parents. The aim of the service is to empower Young People to move towards independent living, supporting their successful transition from childhood; laying the foundations for healthy, ambitious, and safe adult lives with minimal ongoing support. 132 Assessment Centre is for single homeless men and women that have links within the borough. The accommodation provides 6 separate self-contained units with on suite bedrooms, and 1 bedroomed flat. The scheme is staffed 24 hours, with Night Support Workers and Concierge cover during the evenings.
The aim of the project is to assess the individual over a 12-week period with the aim of identifying their support and housing needs. In that period, we will provide a bed for the customer to assist staff to carry out a detailed assessment to ensure that the recommendation for move on into alternative housing is captured correctly to assist move on for the individual into more suitable accommodation which is correct reflective of their needs based on our assessment/recommendation.
The service operates a model of personalised approach in order to address a wide range of presenting complex needs, which could range from mild learning difficulties, substance use (alcohol and drugs), offending, mental health, dual diagnoses, etc. We work closely with outside agencies in order to ensure that the individual is linked into getting the correct support whilst they remain at the assessment centre.
A major part of the Assessment Support Workers role is putting in the foundation with our customers around basic living skills and preparing the individual to be ready to move into alternative supported accommodation.

Oaklands & St Kilda is a pair of 6 unit supported accommodation mental health rehab services working with young people between the ages of 18-25 who may have experienced a mental health related hospital admission to gain and develop skills for future independent living. The service will support individuals transitioning from Child and Adolescent Mental Health Services to Adult services.
The service forms part of a pair of services supporting up to 12 individuals at any one time across the sites, providing a welcoming and supportive environment for young people across London on a spot purchase basis.
Support workers will manage a number of young people to help them to identify, plan and achieve their goals, develop relationships meaningful to them and achieve greater independence by providing a flexible and individually tailored support package.
This role consists of consists of day time working patterns including early shifts from 8am and late shifts from 2pm. Shifts are 8 hours long and 5 days a week.

- Building supportive, trusting relationships with young people and creating a positive atmosphere
- Working proactively with other members of the team to handle the service caseload and administrative responsibilities

Hounslow Young People is Supported Accommodation Service for young people and young parents aged 16-25 in Hounslow. About the Role:
Waking Night Support Worker will work in line with person centred risk assessments; support plans and liaise with Support Workers to provide comprehensive handover and partnership working. They will be observant of changes in behaviour and presentations ensuring appropriate actions are taken to document and report changes in support needs of young people. Waking Night Support Worker will be able to carry out all necessary health and safety actions including light cleaning tasks and environment management. This is an exciting opportunity for a Support Worker to join our friendly team and help support the growth of our LifeSkills Plus offer across London. You will identify and provide intensive support to adults with complex needs who are further away from employment or training by using a strength based approach, ensuring those accessing the programme receive appropriate practical and emotional support using a mixed programme of home visiting, informal and structured support groups.
Your impact:
You will deliver a range of face to face support and interventions to individuals with complex needs that focus on their strengths and which enable them to consider options and think differently, helping them to feel more empowered and able to face new challenges.
You will support and deliver face to face activities that increase confidence, improve mental health, widen social networks, help with budgeting and enable individuals to seek training, work or volunteering opportunities.
Your skills (for details check the job description and person specification):
You will have experience of working in services that deliver positive outcomes for individuals and their families.
You will be able to demonstrate excellent interpersonal skills and the ability to develop and sustain relationships with colleagues, partners and stakeholders.
You will have the ability to work collaboratively with individuals to identify their support needs, plan, implement and monitor emotional and practical support.
