This is an excellent opportunity to truly make a role your own and get involved in the project from the start, which will be supporting 10 adults with mild/moderate to severe learning disabilities, as well as additional mental and physical health needs.
You will play a pivotal ambassadorial role, spearheading Centre’s 404 service expansion into the London Brough of Brent and bringing our expertise, vision and values to families in need.
This varied and dynamic role will lead on the set up, coordination and provision of excellent person-centred care and support to service users, enabling them to live independently in their home and be supported in all aspects of day-to-day living. As Project Manager, you will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet individual needs. You will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of support workers.
The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would be an advantage. An opportunity has arisen for a lead Support worker to work within our Sanctuary crisis intervention and prevention service, based in Fore Street in Enfield. We act as a short-term A&E and Emergency services alternative, providing support to individuals in the Enfield Borough aged 18+.
This role is to provide leadership and support to the Sanctuary team on shift. The role will:
- Lead shifts during the week, providing support to team members via case management support, escalation, Health and Safety guidance
- Support the Sanctuary Manager to embed processes within the service, and ensure that these are carried out
- Work alongside the Manager and team to ensure that the service is operating to high quality standard.
- Provide support in times of emotional distress for clients. The support worker will provide a calm, supportive presence.
The role will support clients through co-producing support plans with practical and therapeutic support offerings, as well as contributing to the development of a warm welcoming environment and ‘safe space’ to be.
This position would suit someone with a minimum of 2 years experience of working with people with mild, moderate and severe mental health issues.

A bit about the role:
As a Outreach Support Worker, you will provide a service where planned support and crisis intervention is delivered to vulnerable people with low, medium and high support needs living in independent accommodation with the aim of them maintaining their home.
In conjunction with colleagues and Senior Managers, you will be responsible for the effective operation of the Support Service, in line with its policies and procedures, including encouraging a co-operative and supportive environment within the Service, listening to customers’ views and developing new services to meet these needs.
Some of the key results for the role include:
- To provide pre tenancy guidance and ongoing support to customers on all tenancy matters, including assistance to furnish and move into properties.

Thames Reach’s Outreach teams operate across London and helps those who are rough sleeping to find routes away from the street. Summary of Role
Working under the direction of our Centre Managers at Arc in the Park and Abbey Hub, Support Workers are key to ensuring the successful delivery of our provision for children and young people.
