Community Outreach Worker (Complex Needs Housing Service)

Purpose of Post

The Community Outreach Worker (Complex Needs Housing Service) provides advice, information, onward referral and holistic support to clients who are experiencing mental ill health or need support with their mental wellbeing. They ensure that the safety and wellbeing of the client is monitored and reviewed regularly, they remain a source of independent support for all clients and they will embed a person centred recovery orientated and trauma informed approach in all aspects of the role.

About the Service

The Complex Needs Housing Service works as a multi-disciplinary team (MDT) with housing teams and Drug and Alcohol Services across District and Borough councils. These teams will be working with individuals who are presenting as homeless and have a number of Complex Needs.

Service Objectives

• To improve the mental wellbeing of people experiencing mental ill health.
• To increase early access to help for people experiencing mental ill health in, by providing a clear and effective pathway to services provided by Herts Mind Network and other third sector and statutory providers.
• To contribute to an improvement in individual mental wellbeing.
• To remain a source of independent support for all clients.
• To reduce the use of police, ASB, tenancy enforcement, housing needs and environmental health services by clients whose primary need is related to mental ill health.
• To reduce the use of crisis services by people experiencing mental ill health with positive outcomes for the individual.
• To provide an assertive outreach approach for clients who are in crisis and/ or those that may have complex or multiple needs.
• To work collaboratively within a multi-disciplinary team (MDT) consisting of Change Grow Live (CGL), District and Borough councils, housing teams and other professionals.

Bridges Outcomes Partnerships (BOP) is a not-for-profit social enterprise that works alongside Government, community groups and specialist Delivery partners to design and deliver vital services that support people to improve their lives, in areas ranging from housing and employment to education and health & wellbeing.

About the opportunity

As an Impact Analyst, you will work within a team across multiple social impact projects. An example of one of the programmes you will become a key part of delivering is the Single Homelessness Prevention Service (SHPS) which is a programme that prevents the next generation of rough sleepers.

The programme supports people to sustain an existing tenancy by negotiating with landlords/ family members, or providing coaching to empower people to do so. Where needed, we help them to find and secure a new home, typically in the private rented sector. SHPS services also seek to prevent the mental trauma associated with homelessness by preventing the escalation of the homelessness journey to crisis point.

Responsibilities

Your responsibilities will include.

· Overseeing all data related aspects of the programme including data collection, extraction, cleaning, analysis, reporting, and presentation.

· Manage all Power BI reporting tables and system with support from central teams.  

· Gathering and analysing quantitative and qualitative data to measure the impact of service innovations for the programmes you are working across. Produce regular reports and insights that aid decision making to continuously improve programme design.

· Taking initiative to propose and implement relevant analyses to the project to maximise positive impact for participants with the programmes you are supporting.

· Building relationships and working at times directly with Delivery Partners (VCSE organisations who are delivering the frontline services for the programme) to ensure they are able to use the data systems accurately and effectively,

· Identifying opportunities for process automation and improving utilisation of management data by colleagues across the programme.

· Work with system developer to manage system changes and improvements.

· Lead the set-up of a new database and associated new contract mobilisation activities.

· Provide research and data to support the development of new interventions. 

· Produce monthly board packs for programmes and provide minutes of meetings.

·  Support bid submissions with development and growth leads.

· Engaging with other analysts across BOP to share learnings from your own project and implement learnings from other projects in your own.

The Royal College of Ophthalmologists (RCOphth) is a membership organisation and the voice of the profession, with over 4,000 members in the UK and overseas. Our role is to champion the specialty, set standards of training and practice, support the personal and professional development of our members and influence change to make a difference to the lives of patients with eye conditions.

Main Purpose

The RCOphth provides education, support and professional development for ophthalmologists of all grades and across all of their roles. It also contributes to education in eye care and ophthalmology for undergraduate medical students and practitioners in other medical specialties and ophthalmic professions. The RCOphth aims for all its audiences to be able to access the ophthalmology education they need, when and where they want it, and in the form most appropriate for the learner and the learning material.

• Manage the progression of materials through the necessary stages of creation and development and ensuring that materials are uploaded onto INSPIRE in a timely manner.
• Develop the functionality of the platform and managing its interaction with other software such as the membership database and collection of payments

Editors/Co-Editors/Instructors/Contributors

• Provide support to Editors and Co-Editors in loading resources onto the INSPIRE platform and integrating them to create learning materials.  This will entail formatting and editing text, images and video for house style and grammar whilst maintaining medical/scientific accuracy.
• Checking consent and clearing copyright/licensing permissions for materials used in INSPIRE.
• Organising Editor and Co-Editor meetings and workshops.
• Create mechanisms to obtain user feedback on learning materials and evaluate the platform to direct change.

Communications/Liaison

• Liaise with internal colleagues in the Department, the Communications Team and external stakeholders in relation to the development and promotion of online resources.
• Liaising with College staff from other departments about current and future projects that require support from INSPIRE or will generate learning materials for the platform.
• Liaise with those organising and delivering courses and webinars to record presentations, select and upload materials, and create specific learning pathways for delegates.

We tackle poverty and disadvantage in communities, through mental health, drug and alcohol, housing and related support.

We have almost 200 services around England – and nearly 3,500 amazing staff and volunteers who run them. Every year our services support around 125,000 people.

Bringing together two big organisations is not a small task and it’ll take a while to fully integrate our systems, processes and approaches. The Role

An exciting opportunity has arisen for a Building Recovery coordinator (HMYOI Deerbolt being the community) to support Waythrough’s service delivery within Reconnected to Health in HMYOI Deerbolt.As a Building Recovery coordinator, you will provide 1:1 support for service users, who have completed most of their recovery journey with a Recovery Coordinator and are now in sustained recovery. You will take a “whole person” holistic approach to ensure that every service user’s package of care encompasses all areas of their wellbeing, to achieve the best possible outcomes for every person. 
As a Building Recovery coordinator, you will also recruit train and supervise peer mentors who will support the service alongside yourself with group delivery, mutual aid, focus groups and any project work.

• Manage a service user caseload, devising, implementing and reviewing on-going recovery plans and treatment, liaising closely with the full range of workers and skills within the service team, including family members and significant others as appropriate.
• Carry out focus groups and promote equality diversity and inclusion through project work and promotion of the service
• Recruit, train and supervise peer mentors who are an integral part of our service delivery and lived experience
• Work collaboratively with partner agencies to undertake shared assessments, key working and interventions to promote individual service user progression from engagement, through structured treatment to self-sustained recovery.
• Facilitate service user access to treatment and community resources to increase recovery and social capital, supporting service users and their families within their local community To lead effective case management for allocated service users, including on-going processes such as risk assessments, risk management plans and safeguarding assessments and plans. This includes the effective use of an online case management system.

The Research Officer role at Marie Curie is a key position within the Research Team and the larger Policy and Research Directorate. The role is focused on driving evidence-informed policy and practice in palliative and end of life care. This position offers an exciting opportunity to apply research experience and passion to make a significant impact on the lives of people affected by dying, death, and bereavement.

You will be responsible for:
- Supporting the associate directors with ongoing and new research projects, including data analysis (qualitative and quantitative), preparing manuscripts for publication, and creating academic posters for research conferences
- Conducting or supporting research such as scoping reviews, rapid reviews, and qualitative or quantitative analysis, and summarizing information to inform research and policy work
- Developing summaries and impact reports from funded Marie Curie research and other research that supports Marie Curie's ongoing work
- Organizing events and conferences, including the annual Marie Curie Palliative Care Research conference, and planning new local conferences and workshops focusing on knowledge mobilization and quality improvement
- Providing general administrative support to the research team, including arranging meetings, taking minutes, and tracking actions

Key Criteria:
- An interest in end-of-life care research.
- Experience in undertaking or supporting research, including literature reviews.
- Highly motivated, positive, proactive, and solution focused.
- Ability to work independently and manage own workload, work calmly under pressure, and meet tight deadlines.
- Effective team member with excellent written and verbal communication and interpersonal skills.
- Excellent administrative and IT skills, including proficiency in Word, Excel, Outlook, and web-based databases.
- Ability to develop effective working relationships with Marie Curie staff and external stakeholders.
- Qualitative or quantitative research skills and experience of presenting research and capturing data in compelling ways.
- Experience of supporting committees, organizing meetings, and taking minutes.

About the Role

Acorns is currently the UK’s largest children’s hospice charity, in terms of both numbers of children and families supported and annual expenditure on our care work.  We need to raise more than £13 million each year to run and support our care services and we rely heavily on the goodwill and generosity of our local community. 

As Partnership Fundraising Manager, you will:

• be responsible for the creation and submission of bespoke applications to trusts and foundations, including high value and multi-year funding organisations.
• maintain, develop and support existing relationships with grant making trusts and foundations
• deliver engagement events to motivate and inspire trusts and foundations
• develop and use a bank of Acorns stories and case studies
• build good relationships and work closely with the wider Acorns teams including Care Teams, Finance, Data Team and Marcomms

About You

• Experience of trust and foundation work in the voluntary sector

• Fundraising experience

The aim of the Hertfordshire Mind Network Domestic Abuse Casework Service is to provide advice, information, and support to survivors of intimate partner or familial violence living in the community about the range, effectiveness, and suitability of options to improve their safety and that of their children. All advice will be based on a thorough understanding and assessment of risk and its management.

The purpose of the Hertfordshire Mind Network Community Outreach Worker role is to:

• To provide support and advocacy services to clients experiencing domestic abuse
• To ensure direct contact is made with the client within a specified time of an incident being reported to the police and to carry out a risk assessment.
• To ensure that the safety and wellbeing of the client is monitored and reviewed regularly.
• To maintain and enhance service delivery standards and effectiveness.
• To support the team with case management and volume of referrals to ensure a short waiting time and referrals are contacted promptly and assessed appropriately.
• To collate and obtain feedback regarding the effectiveness of the service.

Substance Misuse Practitioner (Mid area)

The Children's Society has been helping children and young people in this country for over 140 years. We run local services that support young people when they are at their most vulnerable and in desperate need of help. We're there for young people, every step of the way. 

This role sits within our children and families directorate, which works to provide immediate one-to-one and group support for young people who urgently need support. Your role will be to help us maintain and develop innovative practice to support our work in our services within the Essex area. 

We wish to recruit a Substance Misuse Practitioner to join the established All Age Recovery Co-ordination Service (AARCS), which we deliver in partnership with Open Road, supporting young people, adults and families affected by substance misuse.

You will undertake individual assessments and risk assessment with young people up to the age of 25, developing a person-centred care plan specific to that young person and their needs and supporting them through their recovery journey with the service. This will be delivered through a range of tailored interventions, in both a one to one and/or targeted group work capacity.

-You will have substantial experience in working with vulnerable young people either in a paid or unpaid capacity.

-You will have experience of carrying out individual and group work interventions with young people & adults.

-You will have experience in using Microsoft Office Applications and other relevant data systems.

-You can demonstrate the ability to work on your own initiative as well as part of a multi-disciplinary team.
